Evaluate the contribution of social services.

Performance measures are generally easiest to determine for social work activities that have clearly defined goals. According to Patrice Spath, RHIT, by using a patient and family satisfaction survey, the case management team can collect data about patient and family perceptions of staff performance, set goals, and make constant improvements in quality.

Find out what's causing unplanned admissions.

Hospitals have seen a tremendous increase in the number of surgical procedures performed in the outpatient setting. This change in practice has implications for patient care quality and hospital resource use. Does ambulatory surgery impair quality? How to formalize your grievance response. Part 2.

It's unlikely that the mechanism used in many facilities to handle patient complaints is as well-defined as it must be in order to meet the Health Care Financing Administration's Conditions of Participation. In this month's column, part two of a three-part series, we describe the first steps in the grievance process, when a grievance is filed and the hospital responds.

When does a complaint become a grievance? Part 1.

Included in the Health Care Financing Administration's new Conditions of Participation (CoPs) is the requirement that a hospital's governing board ensure there is a well-defined process that patients can use to file grievances and receive feedback. But because the CoPs do not define the term "grievance," the regulations are unclear.

Continuity of care means better instruction on meds.

Patients' lack of compliance with therapeutic treatment regimens is often overlooked as a continuity-of-care problem, argues columnist Patrice Spath. In this month's Quality-Co$t Connection, Spath offers advice on determining whether your facility offers patients clear and complete medication information and how to improve the transfer of information to patients.

Analyze process changes to identify system failures.

In this month's Quality-Co$t Connection, consulting editor Patrice Spath, RHIT, emphasizes that undesirable patient incidents are often caused by slight changes in the usual process of care. Deviations in system operations can be planned and desired or unintended and unwanted. Change analysis can help you analyze the effects of such deviations and can be a useful tool during sentinel event investigations.

Does your facility have a 'patient-safe' climate?

Although it's difficult to measure an institution's culture, it is possible to measure the factors that affect patient safety, says quality consultant Patrice Spath, RHIT. Areas to evaluate include management systems, safety systems, and individual attitudes and perceptions about patient safety improvement. In this month's Quality-Co$t Connection, Spath examines the elements that contribute to a patient-safe climate.

Surveyors looking for well-designed processes.

The Joint Commission's performance improvement standard requires that organizations do an effective job of designing new processes. New processes may be those that have been revamped in a way that makes the process dramatically different or new services/activities that never existed before in your organization. Consulting editor Patrice Spath, RHIT, discusses how to approach systematic process development.

How to assure appropriate peer evaluations. Part two.

Peer review within the organized medical staff is a systematic effort to evaluate and analyze medical care services delivered to patients and to assure the quality and appropriateness of these services. The goal of peer review is to maintain and improve the quality of medical care. To adequately fulfill their responsibilities, the medical staff should have clear definitions of how peer evaluations will be conducted and what constitutes an adequate review process.
