[Anguilluliasis of the small intestine and AIDS. A case and review of the literature].

The authors report about one patient with an Acquired Immune Deficiency Syndrome (AIDS) presenting with strongyloidiasis involving the small bowel. On the basis of this observation, they describe the appearances of small bowel involvement by strongyloidiasis in patients with AIDS. They emphasize the main features differentiating these cases from the usual affection, namely the severity and diffusion of the lesions, as well as the presence of a very extensive associated lymphoid nodular hyperplasia.

[Magnetic resonance imaging of malignant hepatic tumors in adults].

Recently, magnetic resonance imaging (MRI) had an important place in imaging techniques of malignant primitive tumors of the liver. For many authors, this technique appears as the reference modality for hepatic tumor evaluation. Nevertheless, MRI is considered to be primarily a problem-solving rather than a screening modality. This specific goal of this technique is due to a more precise characterization of the lesions compared to other imaging modalities. The authors, according to recent data in literature and their own experience, present MRI findings of the more frequent malignant hepatic tumors encountered in adult.

[Nonresectable fibrolamellar hepatocellular carcinoma: outcome of 4 cases treated by intra-arterial chemotherapy].

We evaluated the role of locoregional intraarterial treatment in the management of non-resectable fibrolamellar hepatocellular carcinomas (FLHCC) in 4 women. Three patients underwent transcatheter oily chemoembolization (TOCE) (7 courses), and one received intraarterial iodized oil with chemotherapeutic drug after surgical intraarterial catheter placement (1 course). The latter procedure resulted in marked discomfort and was therefore not repeated. The courses were performed in average every 4 months. TOCE was well tolerated and resulted in a decrease in tumor size in 2 patients, which subsequently permitted a safe hepatic resection. In one patient, TOCE provided a stabilization of tumor size. We conclude that TOCE is a valuable therapeutic tool for the management of non-resectable FLHCC, in particular as it may be followed by hepatic resection.

[Amebic abscess of the caudate lobe. An uncommon topographic form].

Amebic liver abscess is the most common extraintestinal complication of invasive amebiasis. Amebic abscess is most often located in the right lobe of the liver which is affected four times as often as the left lobe. Location in the caudate lobe is exceptional. We report herein a case of amebic abscess located in the caudate lobe.

Transalar sphenoidal encephalocele. Uncommon clinical and radiological findings.

A case of basal encephalocele protruding through a defect in the greater wing of the sphenoid bone is presented. The transalar type of encephalocele is uncommon and unclassified. This case, documented by computed tomography and magnetic resonance imaging, is unique in that it presented with trigeminal neuralgia and was associated with an angioma of the soft palate.

Percutaneous ethanol injection for the treatment of symptomatic cystic metastases from ovarian carcinoma. Work in progress.

Two patients with symptomatic cystic metastases from ovarian epithelial carcinoma underwent ultrasound (US)-guided percutaneous aspiration and temporary injection of 99% ethanol into the cyst. In the first case, the patient initially underwent surgical resection of the mass and received systemic chemotherapy, but the cyst recurred 2 months later. Percutaneous aspiration and ethanol sclerotherapy were performed twice in the second case; fluid reaccumulated 2 months after the initial procedure. No side effects occurred. During the follow-up period (8 months in the first case and 4 months in the second), no clinical recurrence of the initial symptoms was noted. At the end of that period, a recurrent but asymptomatic cystic lesion was revealed at US examination in the first case. In the second case, a minimal asymptomatic residual collection was depicted with computed tomography. The results indicate that this technique should be considered in patients with symptomatic cystic metastases from ovarian carcinoma and may have potential benefit in the palliative treatment of such lesions.

Preoperative segmental localization of hepatic metastases: utility of three-dimensional CT during arterial portography.

To evaluate the accuracy of a fast three-dimensional (3D) reconstruction technique in determining the segmental location of hepatic metastases, 14 patients (40 metastases) were prospectively investigated before surgery with dynamic two-dimensional (2D) computed tomography during sequential arterial portography (CTAP). All patients underwent subsequent hepatic tumor resection within 4 days. After computer-generated mapping of hepatic venous structures with high attenuation and metastases with low attenuation, 3D reconstruction was performed. Thirty-six of the 40 lesions (90%) were detected with 2D and 3D CTAP. The accuracy in determining the segmental location of hepatic metastases was 78% (28 of the 36 metastases) for 2D CTAP and 94% (34 of the 36 metastases) for 3D CTAP. The difference in localization rates between 2D CTAP and 3D CTAP was statistically significant. The 3D CTAP technique provides vital data unattainable with other imaging modalities that improve the preoperative assessment of the resectability of hepatic metastases and allows planning a safer surgical approach.

[Hemoperitoneum caused spontaneous rupture of hepatocellular carcinoma. Role of hepatic artery embolization in the therapeutic procedure].

A 73-year-old man with intraperitoneal haemorrhage due to spontaneous rupture of hepatocellular carcinoma was successfully treated by emergency transcatheter hepatic artery embolization. A safe hepatic resection was performed 30 days after the procedure. The patient was still alive four months after surgery. We concluded that emergency arterial embolization is an effective alternative to surgery to control hemorrhage resulting from spontaneous rupture of hepatocellular carcinoma and permits, when possible, a safe subsequent surgical treatment of the disease.

[Percutaneous transluminal angioplasty of portosystemic shunts].

Percutaneous transluminal angioplasty was performed in 3 patients with stenotic portosystemic shunt (one splenorenal, one mesocaval, and one arterialized splenoportal shunt). In the first two cases, the procedure was successful, followed immediately by widening of the shunt and a decrease of the splenorenal and mesocaval pressure gradient. Shunt patency was confirmed at long term as the patients showed no evidence of esophageal varices or recurrent variceal bleeding. In the third case, results were disappointing as thrombosis of the shunt occurred at the end of the procedure. Nevertheless, we conclude that percutaneous transluminal angioplasty should be the first intention procedure for treating stenosis of portosystemic shunts.

Primary carcinoid tumor of the seminal vesicles: CT and MR findings.

A case of primary carcinoid tumor of the seminal vesicles is presented. To our knowledge, this case documented by CT and MRI is unique in that it is the first described in this location. The tumor appears homogeneous and intensely enhanced after IV on CT, and hypointense on MRI on both T1- and T2-weighted images. On T2-weighted images, the tumor is heterogeneous.

[Evaluation of the role of MRI at 1.5 T in the staging of the initial extension of cancer of the uterine cervix].

To evaluate the sensibility and specificity of MRI in the initial staging of carcinoma of the uterine cervix, a retrospective study of 41 patients (48 MR scans) was undertaken. The sensitivity of T2-weighted images in the detection and determination of tumor location was 90% and specificity was 100%. On T2-weighted images, the signal intensity of the tumor was high in 64%. Determination of tumor size was correct in 80%. The ability of MRI to demonstrate the extension to the uterine corpus was acceptable with a sensitivity of 63% and a specificity of 100%. The extra-uterine extension was difficult to appreciate especially for parametrial, vaginal and bladder involvement with a sensitivity of 38%, 43 and 67% respectively. Concerning the lymph nodes involvement, the sensitivity was 38%. The results of this study point out the ability of MRI to evaluate the size and location of the tumor. The accuracy of MRI to appreciate the involvement of the parameters, vagina and bladder is disappointing.

[Unresectable hepatic adenomas. Management by selective hepatic artery embolization].

Four patients with unresectable hepatic adenoma underwent selective hepatic artery embolization. Non-operability was related to the size and the situation of the tumor, and the clinical background. The only subsequent postembolization syndrome was one case of acute renal failure, reversible after diuretic treatment. In 3 cases the decrease in tumoral size was delayed and appeared six months after the first embolization, allowing surgical resection in 2 cases. Because of lack of tumoral devascularization and presence of residual nodules on the resected specimen, this technique is not a satisfactory alternative to surgery, at least with the employed technique. However, preoperative selective hepatic artery embolization can play an important role in the management of hepatic adenomas as it may permit safer surgical resection.

[Prevention of intolerance to iodized contrast media ].

The exact mechanisms of adverse reactions to contrast media are still imperfectly known. However, these reactions may be classified as idiosyncratic. Non-idiosyncratic reactions can be prevented by using new, non-ionic contrast agents. Idiosyncratic reactions can be prevented by specific premedication. Patients with a history of idiosyncratic reaction may benefit from corticosteroids and antihistamines administered prophylactically. Patients who seem to be more likely than others to react to contrast media must be premedicated, the risk of reaction being identified and evaluated by questioning. It has recently been suggested that all patients about to receive an intravascular injection of contrast medium should also be premedicated.

[Differential diagnosis of ulcerative mucous membrane diseases of the mouth cavity and pharynx].

Nonmalignant ulcerous diseases of the mouth often require repeated histological and clinical observations for several weeks to establish a diagnosis. One patient with Wegener's disease, one with a necrotizing sialometaplasia, and a case of Sutton-type aphthosis outline these difficulties. In all three cases, only a synopsis of histology, laboratory findings, and clinical appearance led to the respective diagnosis.

Transalar sphenoidal meningoencephalocele associated with buccal angiomatosis. One case.

Basal encephaloceles are uncommon congenital malformations occurring with an estimated frequency of one case in 35,000 births. They are frequently occult since they protrude into the orbit, nasal cavity, nasopharynx or paranasal sinuses. Basal encephaloceles are divided into five anatomical types. We report a case of transsphenoidal encephalocele which differed from the classical type in that it protruded through a congenital defect in the greater wing of the sphenoid bone. This case had an unusual clinical presentation, being responsible for trigeminal neuralgia and associated with an angioma of the soft palate.

[Imaging of cancer of the uterine cervix].

Recently, magnetic resonance imaging (MRI) and transrectal or transvaginal ultrasound (TRUS, TVUS) had an important place in imaging techniques of cervical carcinomas and raise the question of modifying the imaging strategies. For the diagnosis of primitive tumor, those techniques cannot take the place of clinical examination and gross examination. In the assessment of parametrial involvement, TRUS which has better accuracy than clinical examination, and MRI which is considered as the most accurate technique, have an important role to play. In the follow-up and the detection of recurrences, MRI is actually considered as the best imaging technique. The authors, according to recent data in literature and their own experience, present basic concepts of imaging strategies for staging and follow-up of cervical carcinomas.
