Microglia generate external proton and potassium ion gradients utilizing a member of the H/K ATPase family.

An ion selective electrode in self-referencing mode was employed to detect ionic concentration gradients at the vicinity of microglia isolated from newborn rat brains. At 5 mM extracellular potassium concentration, a gradient of -9.43+/-4.2 microM (n=48) was recorded dissipating over a distance of 10 microm from the outer surface of the cell membrane. Pharmacological studies indicated that neither the Na+/K+-ATPase nor the inward rectifier potassium channel makes significant contributions to generation of this gradient. The recorded potassium gradient was found to be augmented by increase in extracellular potassium or proton concentrations and could be inhibited by Omeprazole (10 microM) and by the specific H+/K+-ATPase blocker SCH28080 (1 microM). These, along with the coexistence of a gradient of excess of protons, strongly suggest that a K/H ATPase is the major generator of both the potassium and the proton gradients. The Kd of the glial transporter for K ions is an order of magnitude higher (3.7 mM) than that of the epithelial H+/K+-ATPase. This is a first report of an H+/K+ transporter in microglia cells with a Kd in the physiological range of [K+]out. Implications of the H+/K+-ATPase on potassium homeostasis in microglia under high extracellular potassium and low pH, as found at the site of brain injury, are discussed.

The role of the general health questionnaire in general practice consultations.

BACKGROUND: The patient self-rating questionnaire is commonly used as a research tool to identify patients with 'unrecognized' depression. There is no evidence to support its use as a clinical tool in general practice. AIM: To determine whether use of the 30-item general health questionnaire (GHQ) is a practical means of increasing identification of 'new' episodes of emotional distress among patients consulting their general practitioner (GP). METHOD: A randomized controlled trial was carried out in a Scottish new town practice with eight partners. In the waiting room, 1912 patients aged over 14 years and consulting over a 10-month period attempted to complete the GHQ. The 'clinical judgement' group posted the questionnaire into a box then attended the doctor as normal. The 'screened' group presented the questionnaire to the doctor. After the consultation, the doctor completed an assessment questionnaire. The main outcome measures were GHQ scores and doctors' assessments of mental health. RESULTS: In total, 1589 patients were eligible to participate. However, 207 patients in the screened group were excluded because the doctor did not look at the questionnaire. The clinical judgement group (59.7% patients) and the screened group (40.3%) were compared. Although the doctors' diagnoses of distress were low in the clinical judgement group (8.1%), they were significantly greater in the screened group (13.9%) where the diagnosis of depression was doubled. The percentage of patients scoring greater than or equal to 9 (GHQ+) was 21.5% and 21.0% respectively. The level of agreement between the doctors' diagnoses of distress and the questionnaires scoring GHQ+ rose from 19% in the clinical judgement group to 35% in the screened group. CONCLUSIONS: The general health questionnaire used in a practice setting increases the identification of patients with emotional distress. However, the use made of the questionnaires in the screened group raises questions of doctor and patient acceptability.

Quantitative assessment of trabecular bone pattern identification.

The results of the research described in this paper demonstrate that the trabecular architecture is unique to each individual and stable enough to be used as a forensic marker for positive identification of human remains. The trabecular bone architecture depicted on radiographs is often used as an individualizing forensic marker for positive identification of human remains. The aim of the present study was to ascertain the reliability of the trabecular pattern in forensic identification. The trabecular pattern is potentially the best radiographic forensic marker since its presence on a radiograph doesn't depend on a previous pathology or traumatic event. A sample of 305 radiographs of the left wrist of 103 postmenopausal women was studied using an image analyzer. The uniqueness and stability over time of the trabecular architecture was examined by creating line maps or "densitographs" of the ultra-distal point of the radius of each roentgenogram. Pearson's correlation coefficients were calculated for all possible combinations of pairs of radiographs. The correlation coefficient of pairs of radiographs of the same individual, taken at different times (2 to 6 years apart), was always higher than 0.72, while the correlation coefficients of radiographs of different individuals was always below 0.62.

The emotional impact of shift work on the children of shift workers.

OBJECTIVES: This study compared the emotional state of children of shift and day working fathers. METHODS: One hundred and ninety children (8 to 11 years of age) took part in the study. Ninety-one came from "shift working" homes, and 99 came from "day working" homes. Each child completed 2 questionnaires, The Harter Self-Perception Questionnaire and the Children's Depression Inventory. RESULTS: The daughters of shift working fathers reported a significantly poorer perception of their ability in school-related activities and significantly greater discrepancies between their perceived overall level of competence and their ideal level of competence. In addition, more depressive symptomatology and a lower level of self-esteem were also found for the daughters of shift working fathers in comparison with the daughters of day working fathers. No such effects were found for the sons of shift working fathers. CONCLUSIONS: Parental shift working may be experienced as stressful within the family, and this stress may affect the emotional state of the child. However, the nature of expression of these emotional difficulties may vary according to the gender of the child. Further research is needed.

p53 immunohistochemical staining and survival after adjuvant chemotherapy for breast cancer.

We have investigated the relationship between immunohistochemically determined p53 status and outcome in 277 women with node-positive primary breast cancer who, following tumour excision and axillary clearance, were randomised to receive either 6 cycles of cyclophosphamide/methotrexate/S-fluorouracil (CMF) (n = 130) or no such post-operative treatment (n = 147). Follow-up data (median = 9 years) were available on all patients. A significant association was found between p53 status and survival. Patients with p53-positive tumours had a less favourable outcome than those with p53-negative disease. Women receiving adjuvant CMF chemotherapy had a significantly more favourable outcome compared to those who did not. The effect was seen both in women with p53-positive and p53-negative tumours; multivariate analysis showed relative risks for overall survival attributable to chemotherapy of 2.3 (95% CI 1.2-4.3) for women with p53-positive tumours and of 2.1 (95% CI 1.4-3.0) for those with p53-negative tumours. Thus, adjuvant chemotherapy with CMF is associated with a survival benefit in women with node-positive breast cancer irrespective of immunohistochemically determined p53 status.

Magnetic properties experiments on the Mars Pathfinder lander: preliminary results.

Many of the particles currently suspended in the martian atmosphere are magnetic, with an average saturation magnetization of about 4 A. m2/kg (amperes times square meters per kilogram). The particles appear to consist of claylike aggregates stained or cemented with ferric oxide (Fe2O3); at least some of the stain and cement is probably maghemite (gamma-Fe2O3). The presence of the gamma phase would imply that Fe2+ ions leached from the bedrock, passing through a state as free Fe2+ ions dissolved in liquid water. These particles could be a freeze-dried precipitate from ground water poured out on the surface. An alternative is that the magnetic particles are titanomagnetite occurring in palagonite and inherited directly from a basaltic precursor.

Genomic imprinting in the rat: linkage of Igf2 and H19 genes and opposite parental allele-specific expression during embryogenesis.

Igf2 and H19 are closely linked imprinted genes in both mice and humans that are expressed from opposite parental alleles. In this study we demonstrate that these two genes are also closely linked in the rat, with the H19 gene mapping to within 145 kb of Igf2 on rat chromosome 1. We identified polymorphisms in H19 and Igf2 transcripts in two inbred rat strains and determined the expression of the parental alleles in F1 offspring. The H19 gene was shown to be expressed exclusively from the maternal allele in all fetal and neonatal tissues. Monoallelic expression of Igf2 from the paternal allele was found in all tissues except the leptomeninges and choroid plexus. Igf2 in the choroid plexus was monoallelic at days 13.5 and 15.5 of gestation with a switch to biallelic expression by day 18.5, demonstrating a loss of imprinting after the choroid plexus has differentiated. Biallelic expression of Igf2 was observed in the leptomeninges at all fetal and neonatal stages analyzed. These studies demonstrate conservation of imprinting of two closely linked genes transcribed from opposite parental alleles in a species other than human or mouse. A comparative approach between different species will be important in defining the mechanisms that regulate the tissue-specific expression of imprinted genes.

Phosphorylation of T-lymphocyte plasma membrane-associated proteins by ectoprotein kinases: implications for a possible role for ectophosphorylation in T-cell effector functions.

Extracellular adenosine triphosphate (ATPo) has been suggested to play a role in lymphocyte effector functions. Recently, it has been suggested that MgATP2- may be the molecular species which is involved in modulating the lytic interaction between cytotoxic T-lymphocytes (CTL) and their target cells. In this study, we provide evidence that ATPo mediates the phosphorylation of extracellular proteins on T-lymphocytes through the action of ectoprotein kinases. The ectophosphorylation is temperature-dependent, supported by Mg2+ and Mn2+, and both ATP and GTP, whereas kinase activity and/or substrates were removed by pretreatment of intact lymphocytes with trypsin. We show the presence of extracellular ATP/GTP-binding sites, indicating the presence of ectoenzymes on intact lymphocytes. The major ectoprotein kinase was identified as a casein kinase II-like protein kinase and could be inhibited by heparin, whereas its activity was enhanced by spermine. The ectoprotein kinase showed remarkable substrate specificity, phosphorylating the serum protein vitronectin, but not fibronectin. In experiments with the cell-impermeable protein kinase inhibitor K-252b, we demonstrate the possible functional importance of ectoprotein kinase in CTL-mediated cytotoxicity, i.e., target cell death was completely blocked by K-252b without affecting intracellular phosphorylation. These results suggest that ectoprotein phosphorylation may possibly be an important event in immunologically relevant cell-cell interactions.

Liver-directed gene transfer and prolonged expression of three major human ApoE isoforms in ApoE-deficient mice.

Apolipoprotein E (apoE) plays a key role in lipoprotein metabolism and may have other important biological functions. In humans, there are three common, naturally occurring isoforms of apoE that are associated with differences in lipid levels and atherosclerosis. However, the direct in vivo effects of the apoE isoforms on lipoprotein metabolism and atherosclerosis are not yet fully understood. To investigate the effect of the apoE isoforms in vivo, we constructed second-generation recombinant adenoviruses encoding each of the apoE isoforms. These recombinant adenoviruses were injected intravenously into apoE-deficient mice fed a Western diet (mean baseline cholesterol level 1401 mg/dl) in order to study their effects in the absence of endogenous mouse apoE. Hepatic expression of apoE3 and apoE4 completely normalized the lipoprotein profile; 3 d after injection, mean plasma cholesterol levels were 194 and 217 mg/ dl, respectively, and this effect was maintained for at least 6 wk. Expression of apoE2 had much less effect on lipoprotein levels (mean cholesterol level 752 mg/dl 3 d after injection), despite much higher plasma levels of apoE2 compared with apoE3 and apoE4; by 6 wk after injection the cholesterol levels had returned to baseline levels in the apoE2-expressing mice. Expression of all three isoforms significantly increased HDL cholesterol levels by approximately threefold and was independent of the cholesterol-lowering effect. ApoE transgene expression was substantially prolonged compared with that achieved using a first generation adenovirus and apoE was readily detected in plasma 3 mo after virus injection. These studies demonstrate: (a) prolonged in vivo expression of human apoE isoforms in apoE deficient mice after second-generation recombinant adenovirus-mediated somatic gene transfer; and (b) significantly impaired ability of apoE2 in vivo to mediate clearance of remnant lipoproteins in apoE-deficient mice fed a Western diet compared with apoE3 and apoE4.

Elevated expression of calcium-binding protein p9Ka is associated with increasing malignant characteristics of rat prostate carcinoma cells.

Northern and Western blotting techniques were used to study expression of the mRNA and corresponding protein product of the S100-related calcium-binding molecule p9Ka in 6 different metastatic cell lines of the Dunning R3327 rat prostate cancer model. In cells with the lowest metastatic capability (G cells), p9Ka mRNA was barely detectable. In 2 weakly metastatic cell lines (AT-1 and AT-2), p9Ka transcript amounts were, respectively, 6.29 +/- 0.74 and 5.55 +/- 1.11 times that detected in the G cells. In 3 highly metastatic cell lines (AT-3, MAT-LyLu and MAT-Lu), the amounts of p9Ka mRNA were, respectively, 12.85 +/- 2.82, 13.06 +/- 1.69 and 11.62 +/- 1.81 times that expressed in the G cells. Western blot analyses detected no p9Ka protein in the G cells. The amounts of p9Ka protein expressed by tumour cells of intermediate metastatic capability (AT-1 and AT-2) were 3.4 +/- 1.3 microg and 3.3 +/- 1.4 microg, respectively, per 1 x 10(6) cells. The amounts of p9Ka protein expressed by the tumour cells of highest metastatic capability (AT-3, MAT-LyLu and MAT-Lu) were 8.3 +/- 1.1 microg, 8.7 +/- 1.6 microg and 9.6 +/- 1.7 microg, respectively, per 1 x 10(6) cells. Our data reveal a direct association between the elevated expression of mRNA and the p9Ka protein amounts and the increased metastatic capability of individual prostatic cancer cell lines. We suggest that calcium-binding protein p9Ka may play an important role in the metastatic behaviour of rat prostate cancer.

[Treatment of heart failure with beta-blockaders].

Since 1975 several studies have indicated that treatment with beta-adrenergic blocking drugs has a positive effect on prognosis in patients with left ventricular dysfunction. After myocardial infarction, treatment with timolol and propranolol improves prognosis in patients with symptoms of cardiomegaly and heart failure. In patients with idiopathic dilated cardiomyopathy, treatment with metoprolol improves the left ventricular ejection fraction and symptoms of heart failure, and may have a positive effect on prognosis. Recent studies of patients with chronic congestive heart failure also indicate that carvedilol has a positive effect on mortality and morbidity. The authors review some relevant studies, to stimulate the use of beta-adrenergic blocking drugs to treat certain types of heart failure.

Model for the examination of evolutionary trends in tooth development.

Through the use of serial computerized tomography (C-t) scans, two distinct developmental stages can be identified in mature teeth. C-t scans thus provide a non-destructive method for assessing growth within individual teeth, as well as for comparison of the development of modern and fossil teeth. The second deciduous molar (DM2) and first permanent molar (M1) resemble one another morphologically, despite differences in size and developmental rates. Thus, they provide an excellent model for studying variation in growth within an individual. To test the C-t method, we first examined a recent archaeological sample and then examined teeth from Skhul I. Serial C-t scans were used to compare two distinct developmental stages represented by the dentine-enamel junction (DEJ) and outer enamel surface (OES), respectively, in mandibular DM2 and M1 of 31 archaeological specimens. The difference in form and size between these two surfaces in and between teeth was calculated from intercusp distances measured at the DEJ and OES using the form distance matrix. Intercusp distances at the DEJ and OES of these teeth were then compared to their counterparts in the DM2 and M1 of Skhul I, taken here as representative of early anatomically modern Homo sapiens sapiens. Form differences between paired DM2 and M1 at the DEJ were smaller than those at the OES, supporting the hypothesis that differences between the two teeth increase throughout development. The increase in intercusp distances from the DEJ to OES was found to reflect the angulation of cusps relative to one another, rather than enamel thickness. Form differences between the Skhul DM2 and M1 were smaller than those observed in the recent series, and the recent M1 differed more than the DM2 from its fossil counterpart. The similarities found between the Skhul permanent and deciduous teeth and the recent DM2, may reflect a similar growth pattern. This would contribute to earlier crown completion in the fossil M1.

The localization of the insulin-like growth factor receptor 1 (IGFR-1) in benign and malignant breast tissue.

Insulin-like growth factors (IGFs) play an important role in normal cellular growth and development and have been implicated in the regulation of tumour growth. Two receptors are recognized, IGFR-1 and IGFR-2, of which one, IGFR-1, is a transmembrane heterodimer structurally similar to the insulin receptor. Studies using ligand-binding assays have suggested that the proportion of human breast carcinomas expressing IGFR-1 varies between 39 and 93 per cent and all suggest a lower level of IGFR-1 expression in benign mammary epithelia. As there is this variation between studies and since no study appears to have examined the immunohistochemical localization of IGFR-1 within breast tissue, a series of 79 infiltrating ductal carcinomas, 11 infiltrating lobular carcinomas, three cases of pure ductal carcinoma in situ (DCIS), seven fibroadenomas, and eight normal breast specimens have been studied utilizing the monoclonal antibody alpha IR3. IGFR-1 localized to the epithelial component of 90 per cent of the carcinomas, with only cytoplasmic (21 per cent), only membrane (5 per cent), or a mixture of both cytoplasmic and membrane (64 per cent) staining patterns. In some tumours, distinct basolateral distribution of the receptor was observed. Invasive lobular carcinoma showed significantly less labelling than ductal (P = 0.0009). There was a significant correlation between the level of IGFR-1 immunostaining with both oestrogen receptor (P < 0.001) and progesterone receptor (P = 0.0018) positivity within the malignant group. All normal mammary epithelium showed strong labelling, which was often at an intensity matching that of the most strongly labelled carcinoma and occasionally visualized as basolateral staining of the luminal cells. Weak to moderate staining of endothelial cells was also observed. It is concluded that IGFR-1 immunoreactivity is found in the majority of breast carcinomas, where it correlates most closely with oestrogen receptor status. The high intensity labelling of normal cells seen in this study contrasts with the low levels inferred from ligand-binding-based techniques and emphasizes the importance of the morphological approach in the investigation of novel molecules.
