Extreme variability of clinical symptoms among sibs in a MELAS family correlated with heteroplasmy for the mitochondrial A3243G mutation.

In a family with mitochondrial myopathy, encephalopathy, lactic acidosis, and stroke-like episodes with extremely varying clinical expression, we have identified the A3243G heteroplasmic point mutation in mitochondrial DNA. The degree of severity of the clinical symptoms in the various family members was reflected in the relative quantity of mutated mitochondrial DNA in different tissues. The biochemical activity of complex I of the respiratory chain in muscle was decreased in some members of this family.

Nutrition services and the Hamilton-Wentworth Home Care Program: a current overview.

The structure of and services provided by the Hamilton-Wentworth Home Care Program are described. The goal of the Home Care Program is to provide coordinated multidisciplinary care for the patient in the familiar home setting, to facilitate early discharge from hospital, and/or prevent admission to a health care institution altogether. Nutrition counseling for patients and consultation for the multidisciplinary health care team is provided by the Home Care nutritionist, a registered dietitian. Her duties and activities are further delineated, as they exist in Hamilton-Wentworth. This unique position provides stimulating and rewarding work for the nutritionist who desires the challenge of therapeutic nutrition counseling in a community setting.

Protective efficacy of pneumococcal polysaccharide vaccines.

Clinical studies of 6- and 12-valent pneumococcal capsular polysaccharide vaccines were carried out in controlled studies among novice gold miners in South Africa. In the studies 1,523 persons received pneumococcal vaccine, and 3,171 were included as controls. In the great majority of subjects given pneumococcal vaccine, antibodies developed against the capsular types included in the vaccine. The 6-valent vaccine afforded 76% reduction in cases of laboratory-verified pneumococcal pneumonia caused by the homologous types, and there was 92% reduction in the cases afforded by the 12-valent vaccine. The vaccines were well tolerated and offer great promise for effective control of illnesses caused by pneumococci.
