Gut endocrine cells in the snail Helix aspersa.

A microscopic study of the endocrine cells present in the gut of the snail Helix aspersa is made. Electron microscopy is necessary in most cases to identify the enteroendocrine cells, since neither silver impregnations nor immunocytochemical staining have rendered positive results. Endocrine cells are scarce and rest on the basement membrane. They display a clear cytoplasm and variable amounts of small (143 nm) secretory granules of diverse electron-density. They are ovoid or rounded and possess apical processes which extend into the lumen of the gut. The nucleus, located in the basal region of the cell, presents characteristic cytoplasmic indentations. Intraepithelial nerve bundles in contact with endocrine cells are present.

Endocrine cells of the gastric mucosa of Rana temporaria L.

The endocrine cells of the gastric mucosa of Rana temporaria have been studied according to the ultrastructure, the staining properties of the granules with Masson Fontana's and Grimelius' silver methods, silver impregnation of Davenport on deplasticised semithin sections and immunocytochemical techniques. Seven different types of endocrine cells have been described. Six were regarded as belonging to known types: G, A, EC, ECL, D and P cells. One type was considered as unclassifiable.

Giemsa stain applied to deplasticized sections to identify pancreatic islet cells.

A new application of the Giemsa stain to demonstrate endocrine cells in deplasticized sections of Epon embedded material is described. Its application to the pancreas of Rana temporaria is illustrated. The technique does not require postfixation with OsO4 and is easily performed in 30 min. It allows the easy identification of three types of endocrine cells (A, B, and D). A cells, preferentially located at the islet periphery, stain purple-blue. B cells, which occupy the interior of the islet, display a lilac color. D cells give a strong purple color; they are located both in the periphery of the islets and scattered among acinar cells. Positive identification of the cell types was made by immunocytochemistry and electron microscopy.

Myelopoiesis in lymph nodes of mice. Enhancement effect by the injection of trypan blue or India ink after birth.

Myelopoiesis in lymph nodes was studied in newborn mice under normal conditions and after treatment with trypan blue and carbon particles. In the mouse lymph node, myelopoiesis normally takes place between the fourth and twelfth day after birth, then decreases while an intense proliferation of the lymphocytic population simultaneously takes place. Myelopoiesis manifests itself as clusters of cells, mainly of the neutrophilic line, located around post-capillary venules (PCV's) in the medulla. The injection of trypan blue or carbon particles (India ink) causes an enhancement of myelopoiesis and a delay in the development of the lymphocytic population. Myelopoiesis includes not only granulopoiesis but also the formation of erythroid clusters and occasional megakaryocytes. Three possibilities should be considered to explain the enhancement of myelopoiesis: 1) blockade of the reticuloendothelial system caused by overloading with particulate matter; 2) induction of humoral myelopoiesis-stimulating factors; and 3) stimulation of pluripotent stem cells through dye injection.

The tubular system of human endometrium.

The so-called "intranuclear" tubular system (TS) of human epithelial endometrial cells has been investigated in 50 specimens and has been found in some instances of secretory endometrium and in one case of hyperplastic endometrium with secretory change. The TS was found both in nucleus and cytoplasm. The intranuclear TS can be associated with the nucleolus, connected to the nuclear envelope or lying free in the karyoplasm. When it is connected with the nuclear envelope an intimately related cytoplasmic invagination containing many ribosomes is frequently found. The cytoplasmic TS is smaller than that found in the nucleus and is usually connected with rough endoplasmic reticulum profiles. These findings allow us to make the suggestion that the TS could have a cytoplasmic origin and subsequently migrate into the nucleus. The function of the TS is unknown.

[Excretion in basommatophora and stylommatophora pulmonata (author's transl)].

A comparative study on the kidney of Pulmonata living on land (Stylommatophora) and in fresh water (Basommatophora) was made. The sections performed in three different planes allowed us to know the 3-dimensional pattern of its components: renopericardial duct, kidney sac and ureter. Renopericardial duct runs far into the kidney sac. The duct of Basommatophora is longer and bears also longer cilia than that of Stylommatophora. Kidney sac is similar in both groups and is made up of a wall from which a complex lamellar system spreads into the cavity. Lamellae are longer and more numerous at the dorsal wall, being the lumen of the organ eccentric. The cavity is lined by columnar epithelial cells (nefrocytes) with brush border. Characteristic of the nefrocytes is the presence of a large apical vacuole containing an excretion granule. In Lymnaea there are also lamella lined by cubic epithelium without excretion granule. The union between kidney sac and ureter is different from one another in the three species studied. In Lymnaea the kidney sac narrows and forms the ureter; the connexion in Cryptomphalus is achieved through a 200 micrometer window; and in Helicella there is a duct between both parts. The ureter has a columnar epithelium with very deep infoldings of the basal cell membrane, according to its osmotic regulative function. In Stylommatophora there are primary and secondary ureters, while in Basommatophora there is only one type.

Development of lymphocyte populations in lymph nodes of the mouse.

Lymphocytic colonization of lymph nodes in mouse is studied. Lymphocytes are first seen through the walls of post-capillary venules in diapedesis and nodal parenchyma on the second day after birth. Large lymphocytes are observed in nodal parenchyma between the 4th and 6th day. The nodular pattern appears on the 24th day with development of primary nodules.

[Myelopoiesis in lymph nodes of mice treated with trypan blue (author's transl)].

When mice are injected with trypan blue inside the peritoneal cavity immediately after birth, a delay of the lymphocyte colonization and an enhancement of the lymph node myelopoiesis are observed. In normal mice some granulopoiesis takes place in lymph nodes between 4th and 12th days after birth, but after trypan blue treatment granulopoiesis increases and erythroid colonies appear. These findings indicate that the blockade of RES enhances the hemopoiesis in lymph nodes.

[What is the cost of inappropriate admission of pneumonia patients?].

BACKGROUND: The number of patients admitted with community-acquired pneumonia (CAP) varies greatly from one hospital to another. Prognostic models for CAP can help physicians decide which cases to treat on an outpatient basis. Our aims were: a) to validate a model for predicting low-risk CAP, and b) to estimate savings that would have resulted if the low-risk patients identified by the model had been treated at home rather than in hospital. PATIENTS AND METHODS: All CAP cases diagnosed by the emergency room physicians of a hospital in northwestern Spain (Ferrol) were enrolled prospectively over a period of 19 months. The prediction rule of Fine et al was used to classify all patients. Mortality in each category was compared with the mortality predicted by Fine's system. Patients in the lowest risk categories (I and II) were considered to have been inappropriately admitted unless they were hypoxemic or had significant comorbidity. Costs were figured based on data provided by our accounting department. RESULTS: Of 192 CAP patients enrolled, 131 were admitted and 61 were treated as outpatients. Ten patients died, none of whom was in classes I or II. The costs of the apparently unnecessary hospital stays of the 34 patients in these classes was 6,979,756 pesetas. The estimated savings that would have derived from treating these patients out-of-hospital was 6,133,292 pesetas (36,862 euros; 322,804 pesetas/month). CONCLUSIONS: a) The predictive model used has been found useful for identifying patients at very low risk of dying from CAP; b) Using this model can improve CAP admission criteria, and c) Application of the model can lead to savings.

[Isolation of Mycobacterium tuberculosis with primary resistance to chemotherapeutic agents in patients with HIV infection].

BACKGROUND: To evaluate the resistance of Mycobacterium tuberculosis in subjects with simultaneous infection by the human immunodeficiency virus. METHODS: Isolation of Mycobacterium tuberculosis was carried out in a University hospital over a period of 7 years with selection of those corresponding to subjects with HIV infection. The isolation of Mycobacterium tuberculosis with resistance to one or more chemotherapeutic drugs was identified and is described herein. RESULTS: A total of 287 subjects were identified having tuberculous disease, 39 (13.59%) of them presented a simultaneous infection by the human immunodeficiency virus. In 4 of the latter (10.26%) Mycobacterium tuberculosis resistant to one or more chemotherapeutic drugs was isolated with the most frequent resistance being to isoniazide (3/4). CONCLUSIONS: A high degree of diagnostic suspicion must be maintained in patients with the human immunodeficiency virus with respect to tuberculosis, not only because of frequent endogenous activation but also because of the danger of in-hospital transmission. The possibility of resistance makes systematic antibiograms in these cases advisable. In these patients recommendations regarding treatment should be followed avoiding shortened or simplified therapeutic periods which have occasionally been demonstrated as ineffective.
