[Special diagnostic procedures in atrial tumours of the heart (author's transl)].

A vascular myxoma which prolapsed into the mitral valve was found in the left atrium of a 47-year old man. Two-dimensional ultrasonic tomography yielded relevant non-invasive diagnostic information on the pattern of movement of the pedunculated tumour. Highly accurate determination of the tumour volume was achieved via angiocardiography and videometry. Coronography of the tumour vessels allowed identification of an unusual point of insertion of the myxoma. The risk of obstruction of the mitral orifice was documented haemodynamically by a steep gradient above the mitral valve when holding breath during the expiratory phase.

[Suture of flexor tendons and substitution of flexor tendons in the "no-man's land" in children].

From 1969 to 1976, there were 14 primary, 10 secondary one-stage, and 14 secondary two-stage repairs of injured flexor tendons in the so-called 'no-man's-land'. Functional results of these three operational methods are compared in this paper and show that, in children, flexor tendons in uncomplicated wounds can be sutured primarily, even in the 'no-man's-land'. Flexor tendons in complicated wounds also may be primarily sutured, but not as effectively. In the latter case, according to the results under review, secondary two-stage repair, not secondary one-stage repair, must be applied.

[Complications caused by the plastic material used in a 2-stage flexor tendon grafting operation].

After the use of the silicone-rod method for the two-stage flexor tendon grafting without complications in more than 120 cases, there were found -- although an identical technique was used -- some cases of inflammatory reaction compelling the earlier removal of the rods and leading to poorer results. Tests showed that there are silicone-rubber rods on the market which differ widely in their physical qualities and, therefore, also show differences in their compatibility with tissue.

[Evaluation of regional myocardial dysfunction by echocardiographic pressure-dimension analysis (author's transl)].

Ecg, left ventricular pressure measured by micromanometer-tipped catheter, and dimension measured by echocardiography were simultaneously recorded in 24 patients with coronary artery disease and 9 normal subjects. In analogy to pressure-volume diagrams, pressure-dimension diagrams were constructed, the area in systole and diastole and the cycle efficiency calculated. In patients with coronary artery disease the normal rectangular shape of the pressure-dimension diagram shows two typical deviations: 1. a dimension decrease during isovolumetric contraction and dimension increase during isovolumic relaxation in 42% of the patients, in 31% in patients with stenosis of the right coronary artery, in 56% in patients with stenosis of the left anterior descending coronary artery, in 85% in patients with a reduced ejection fraction. 2. An exact reflected image was found only in patients with stenosis of the right coronary artery and reduced ventricular function in 23%. The abnormal dimension changes are the result of an asynchronized contraction and relaxation with inward movement of one part and outward movement of another part of the left ventricle. These changes are caused by ischemic or fibrous areas of the ventricle. Independent of left ventricular pressure and dimension changes we found that the cycle efficiency was useful to study regional myocardial work. It ranged from 81.7 +/- 2.5% in the normal subjects to 74.8 +/- 1.8% in patients with reduced ventricular function. From the diastolic part of the pressure-dimension diagram the regional compliance was calculated. Dependent on coronary artery disease the regional compliance was decreased even at rest.

[End-to-side mesenteric caval in children - significance of blood flow physics in defining the surgical procedure preoperatively (author's transl)].

Elevated blood pressure of the portal circulation in children may be lowered to defined levels by end-to-side shunting of the mesenteric and the portal vein. The question is, whether an optimal geometry of the ellipsoid anastomosis can be calculated preoperatively. A simplified model of the shunt system is presented using electric and hydraulic analogs of the real blood flow physics. It is not taken into account in this model, that there are two capillary beds connected in series in the portal circulation, which both exhibit high blood flow resistance. Shunt resistance as being calculated on the basis of this model in 6 children, and real shunt resistance as being measured in these patients intraoperatively actually turned out to be in the same order of magnitude inspite of the handicaps of the model. Thus to our opinion the calculations presented are suited to determine preoperatively the optimal geometry of an ellipsoid shunt anastomosis intended to lower blood pressure in the portal circulation to levels as desired.

[Familial asymmetrical septal hypertrophy with and without obstruction: clinical findings in patients with echocardiographically confirmed diagnosis (author's transl)].

Obstructive and non-obstructive asymmetrical septal hypertrophy (ASH) is a relatively common disease which has no characteristic clinical symptoms. In only 14 of 71 patients in whom the diagnosis had been confirmed echocardiographically had it been possible to make the diagnosis by clinical means alone. Most of the patients had no specific clinical symptoms (angina, dyspnoea, systolic murmur, non-specific ECG changes) indicating a cardiomyopathy. Every patient with such uncharacteristic signs should therefore be studied by echocardiography in order to exclude ASH. In 17 patients there were no clinical symptoms at all, the diagnosis being made entirely by echocardiography.

[Changes of left ventricular transverse diameter and of contractility after haemodialysis (author's transl)].

In 26 patients (8 women, 18 men; mean age 47 +/- 3 years) who participated in a regular dialysis programme twice weekly the left ventricular diameter, the mean velocity of fibre shortening, the ventricular wall thickness, and the muscle mass were determined echocardiographically. Left ventricular muscle mass (187 +/- 13 g), left ventricular posterobasal wall thickness (14,7 +/- 0,6 mm) and septal thickness (16,4 +/- 0,8 mm) were pathologically increased due to chronic pressure and volume overloading. After an average dialysis time of 12 hours the body weight was reduced by 2,6 +/- 0,4 kg (+ 1,0 to -7,8 kg). The left ventricular transverse diameter changed from 55,3 to 50,2 mm (P less than 0,001) which correlated with a decrease of the enddiastolic volume from 160 +/- 11 to 125 +/- 10 ml (P less than 0,01(. Despite increased cardiac frequency and unchanged diastolic and systolic pressure no significant increase of the mean velocity of fibre shortening (0,93 +/- 0,06 vector 1,13 +/- 0,09 circ/s) occurred. The results show that loss of fluid during haemodialysis led to a significant decrease of the enddiastolic transverse diameter and the enddiastolic left ventricular volume. However, dialysis does not lead to an increase of the mean velocity of fibre shortening as a parameter of improved contractility.

[Peculiarities of acute appendicitis in women].

A complete documentation of the clinical and operative findings and a comparison of these with the histologic diagnosis in nearly 900 appendectomy patients from a period of more than two years is presented. In this publication an account of the specific results of nearly 500 women is given. The influences of age, menstruation, and gynecologic illnesses are reported in detail. Sixty-five percent of all women were treated unnecessarily with appendectomy. However, the indications for this operation should not be limited on these grounds.

[Echocardiographic criteria of asymmetrical hypertrophy of the ventricular septum without outflow tract obstruction (author's transl)].

Asymmetrical septal hypertrophy (ASH) without outflow tract obstruction is a genetic variant of idiopathic hypertrophic subaortic stenosis (IHSS). Without difficulty and risk echocardiography can distinguish both diseases. Comparison in 33 patients with ASH but no outflow tract obstruction and in 29 with IHSS revealed following identical findings: (1) excessive hypertrophy of the ventricular septum when compared with the posterior wall of the left ventricle (asymmetrical septal hypertrophy); (2) decreased systolic motion of the septum; (3) slowed early diastolic closure of the anterior mitral leaflet; (4) relatively small end-diastolic and systolic diameter of the left ventricle. But the important distinction was that, while in IHSS there was a systolic forward movement of the anterior or both mitral leaflets, this not recorded in patients with ASH without outflow tract obstruction. The posterobasal portion of the left ventricle is thickened in patients with IHSS, due to the outflow tract obstruction.

[Diagnostic criteria of idiopathic hypertrophic subaortic stenosis in the echocardiogram(author's transl)].

Idiopathic hypertrophic subaortic stenosis (IHSS) is morphologically characterized by ventricular septal hypertrophy. It is asymmetrical because there is no corresponding hypertrophy of the posterior wall of the ventricle. The proportion between septal thickness and posterior wall thickness is more than 1.2. In addition, the anterior mitral leaflet moves towards the ventricular septum during the ventricular systole. Finally, the aortic cusps may close prematurely, even during ventricular systole, if there is a marked outflow-tract obstruction. The thickness of the septum and posterior wall, as well as the movement of the mitral and the aortic valves, can be easily registered by the echocardiograph. IHSS is, therefore, more easily diagnosed by this non-invasive method than by any other method. The echocardiogram demonstrates (1) asymmetrical septal hypertrophy, (2) anterior movement of the anterior and frequently also the posterior mitral leaflet in midsystole, (3) partial or complete closure of the aortic valve in mid-systole, (4) relatively small end-diastolic and systolic diameters of the left ventricle, (5) delayed early-systolic closure movement of the anterior mitral leaflet in the sense of a functional mitral stenosis, (6) decreased systolic septal movement.

Aging and its relation to cell growth and differentiation in Drosophila imaginal discs: developmental response to growth restricting conditions.

The development of Drosophila organ discs can be arrested by culturing them in adult male flies. These aged discs lose some of their differentiation competence and form incomplete imaginal structures. The more advanced the aging, the greater the loss of competence. Discs made to differentiate prematurely show deficiencies similar to aged discs. The age-induced defects can be repaired in a larval milieu. Special hormonal conditions, but not cell multiplication, are apparently involved in the recovery process.
