The authors describe their experience of war surgery from a period of three months. A total of 635 patients, most of whom were young men, were treated. The journey to hospital was frequently very long, often lasting for several days. The lesions and types of surgery are described. Ketamine was found to be very useful for war anaesthesia.
A quantitative scintigraphic method was introduced to examine the proximal, middle and distal third of the carpal scaphoid bone. 99mTc-Sn-pyrophosphate scintimetry was obtained by the use of a gamma camera equipped with a pinhole collimator. Of six patients in whom a unilateral fresh fracture of the scaphoid was clinically suspected, radiology showed a fracture of the scaphoid in four and no fractures in two. The method may be suitable in early diagnosis and further localization of clinically suspected fractures with initially non-diagnostic radiographs.
The results of truncal vagotomy and pyloroplasty for gastric, prepyloric and combined ulcers in 215 patients were evaluated 9-15 years after operation. The operative mortality was 1.9 per cent, and the recurrence rates in gastric, prepyloric and combined ulcer patients were 8, 13 and 0 per cent respectively. Recurrent ulceration was significantly related to acid secretion. The functional results were not different in patients with gastric and prepyloric ulcers, but significantly better in the small group with combined ulcer than in the group with prepyloric ulcer. No long term weight changes were seen. Gastric cancer was the cause of death in 2 men and 1 woman treated for gastric ulcer.
