The clinical features and the long-term results of surgery for anal fistula were studied. Fistula distribution was subcutaneous (13%), intermuscular (14%), low anal (55%) or high anal (18%). There were no pelvirectal fistulas. The location was mostly posteriorly in the anus, except for the low anal fistulas, which were most frequent anteriorly. Follow-up examination was carried out on 199 patients, on average 9 years after fistula operation. The recurrence rate was 11% after laying open of fistula, with the highest rate (26%) in high anal fistula. The most common causes of recurrence were undetected internal opening and incomplete laying open of the fistulous tract proper. Most (91%) of the recurrences appeared within 18 months after surgery. Minor defects in anal control were found in 34% of the patients, and in 9% of controls matched for age and sex but with no anal surgery. Multiple operations for anal fistula, and also a gutter-shaped and a firm or hard scar in the anal canal adversely affected anal control. The amount of divided sphincter musculature did not influence the incidence of postoperative anal incontinence.
In 31 adults consecutively undergoing surgery for anal fistula (opening of fistulous tract), anorectal manometry was performed before and 7 months after the operation. The resting pressure was significantly reduced in the distal 3 cm of the anal canal postoperatively. Voluntary sphincter contraction was less markedly affected. Maximal squeeze pressure and maximal contractile power were significantly reduced, however, especially in women and after division of the external sphincter muscle. The pressures were significantly lower in women than in men, particularly after operation, and defective anal control was associated with reduced squeeze pressure. It is therefore suggested that in selected cases, primarily women, anal pressure should be measured preoperatively and division of the external sphincter muscle avoided if the pressure is low. Constant rectoanal inhibitory reflex was elicited by a significantly smaller distending volume and lower rectal pressure postoperatively than preoperatively which, like the reduced resting pressure, indicated impaired function of the internal sphincter muscle.
Anorectal function was manometrically studied in 199 adults on average 9 years after laying open of anal fistula, in order to determine the factors adversely affecting anal continence. Resting anal pressure, and especially squeeze and voluntary contraction pressures, were significantly reduced in the 67 patients with defective anal control. Maximal squeeze and contractile pressures were significantly lower in women than in men but, like maximal resting pressure, were uninfluenced by age. Fistula type significantly influenced maximal resting and squeeze pressures, with tendency to low pressures and high incidence of defective anal control after operation for high intermuscular fistula. Maximal contractile power was unrelated to extent of external sphincter division. Rectal sensation and activity of the rectoanal reflexes did not appreciably differ between the continent patients and the others. Digital assessment of sphincter tone at rest and at maximal contraction correlated well with the respective anal pressures, but was an unreliable indicator of anal continence. The manometric findings warrant maximal preservation of the anal sphincter musculature, but fistula healing must not be thereby endangered.

The incidence and epidemiology of anal fistula were studied among the inhabitants of the City of Helsinki (population 510,000) during a 10-year period, 1969 to 1978. The mean incidence per 100,000 population was 8.6 for nonspecific and fistula, 12.3 for males and 5.6 for females. Of a total of 458 anal fistulae diagnosed during the period under study, the nonspecific fistulae accounted for 90.4%, the tuberculous fistulae for 0.2%, the postoperative and traumatic fistulae for 3.3% and fistulae originating in anal fissure for 3.3%. Anal fistulae associated with ulcerative colitis comprised 1.5% of the total series and fistulae associated with Crohn's disease 1.3%. At the time of diagnosis of the nonspecific anal fistulae, the mean age of the patients was 38.3 years; the male-female ratio was 1.8: 1. All the patients younger than 15 years of age were male. 35% of anorectal abscesses of nonspecific anal origin developed into a fistula. Sedentary occupations did not show any significance in anal fistula formation.

The first case of epidermoid carcinoma of the tongue found in a retinoblastoma survivors is described. The patient, 10-year-old girl, had been treated successfully for bilateral retinoblastoma 9 years earlier. According to the literature, second malignant epithelial tumours are, in contrast to mesenchymal tumours such as osteosarcoma, very rare in retinoblastoma survivors. Initially one eye of the patient was treated with irradiation using Stallard's disk (radioactive cobalt), but we consider that Stallard's disk was unconnected with the genesis of the carcinoma of the tongue.
A recurring adenocystic carcinoma originating in the floor of the maxillary antrum in a 75-year-old man is described. The primary tumour had been surgically removed from the mucous membrane of the hard palate 22 years earlier. Histological examination showed deposits of calcification in the tumour tissue. The authors believe this feature to be a sign of the slow-growing variant of adenocystic carcinoma.
