Unscheduled re-attendances of patients to the A&E Department were examined over 50 days. A re-attendance rate of 1.9 per 100 new patients was found. Of 102 return visits, only 35 resulted in no change in the diagnosis or treatment. Common faults in the initial consultation were failure to explain the condition and prognosis adequately and failure to provide adequate analgesia. Review of patients reattending unexpectedly by a more senior member of staff is a means of identifying faults in management and advising and educating junior staff.
In order to improve inpatient facilities for terminal care for elderly people, a special ward has been opened to maximize the quality of remaining life and to achieve 'death with dignity'. The ward is based within a geriatric department and in a District General Hospital. The work of the first year is described. It is considered to have been successful.

Two hundred and twenty-one patients in slow-stream geriatric wards in the Portsmouth district were questioned about their vision; 159 of these were examined. It was possible to give help with vision to 21 per cent of patients and to give a further 6 percent help with other eye problems. Eighteen months later, when half the patients examined had died, 14 per cent of those still alive were continuing to enjoy their improved vision, and 6 per cent were benefitting from the help they had received with other eye problems. As a result, we recommend that all admissions should be assessed with regard to their visual ability and that all those with adequate mental faculties should be examined by an ophthalmologist or an optician on admission and when necessary therafter.
