Visceral pathology of hereditary tyrosinemia type I.

The major pathological findings in 23 patients with hereditary tyrosinemia type I seen at the Hôpital Sainte-Justine over a 23-year period are reviewed in combination with findings in the literature. Hepatic and renal alterations are given special emphasis. Hepatic changes differ in the acute and chronic forms of the disease. The former is characterized by alterations shared by several hepatopathies of infancy, whereas the latter is characterized by established cirrhosis, frequently of a mixed macro- and micronodular type, with a frightening propensity for the development of hepatocellular carcinoma. Renal changes reflect tubular injury, resulting in Fanconi syndrome, with tubular dilatation, nephrocalcinosis, and involution of epithelial cells. A significant proportion of patients also reveal some degree of glomerulosclerosis and interstitial fibrosis, indicating at least the need for careful assessment and follow-up of renal function, particularly in light of the adverse renal effects of immunosuppressive regimens used in liver transplantation.

Studies on DNA binding of caffeine and derivatives: evidence of intercalation by DNA-unwinding experiments.

Caffeine and derivatives are compounds with pleiotropic effects on the genetic material which are supposed to originate from drugs binding to DNA. Here we show, by using two different topological methods, that methylated oxypurines, at biologically relevant concentrations, unwind DNA in a fashion similar to known intercalators. Methylated oxypurines could be ranked by decreasing unwinding potency: 8-methoxycaffeine greater than 8-ethoxycaffeine greater than 8-chlorocaffeine greater than caffeine greater than theophylline. These findings confirm, with a different assay, interaction of caffeine with DNA and add additional support to an intercalative mode of binding of these drugs to DNA.

Pulmonary sequestration with histologic changes of cystic adenomatoid malformation.

Pulmonary sequestration and congenital cystic adenomatoid malformation (CCAM) are two infrequent congenital pulmonary diseases. The combination of these two entities is rare. We report a case where the antenatal ultrasonography showed a left pulmonary mass suggesting CCAM. The US done after birth revealed an aberrant vascularisation. Pathologic examination confirmed the association of both lesions.

DNA damage in mouse and rat liver by caprolactam and benzoin, evaluated with three different methods.

Benzoin and caprolactam were examined for their capability of inducing alkaline DNA fragmentation in mouse and rat liver DNA after treatment in vivo. Three different methods were used. With the alkaline elution technique we measured an effect presumably related to the conformation of the DNA coil. With a viscometric and a fluorometric unwinding method we measured an effect presumably related to the number of unwinding points in DNA. For both compounds only the alkaline elution technique was clearly positive. The results suggest that both caprolactam and benzoin can induce an important change in the conformation of the DNA coil without inducing true breaks in DNA.

Malignant small-cell tumor of the thoracopulmonary region: 'Askin tumor'.

Malignant small-cell tumor of the thoracopulmonary region (MSCT) is an uncommon neoplasm in children. We describe five cases diagnosed since 1981 that fulfill the criteria put forth by Askin et al. Surgery was performed for diagnosis or therapy in all patients. Two patients underwent open lung biopsy, only because of tumor extent. The other three had chest wall resections. All patients received radiotherapy and chemotherapy. Three patients presenting initially with extensive disease died at intervals of 2.5 to 7 months after diagnosis. Two patients are alive and disease-free at 16 and 24 months postdiagnosis. All five cases were reviewed for standard histology and differential immunohistochemistry. Electronmicroscopy and tissue cultures were done in 3 of the 5 patients. All five patients were neuron-specific enolase-positive. MSCT is an exclusion diagnosis not always readily made. Clinical course and pathologic expertise may point to the correct diagnosis.

Hormone-secreting metastatic interstitial cell tumor of the testis.

A patient is described with a hormone-secreting interstitial cell tumor, manifesting metastasis 7 years after removal of a reportedly benign primary tumor. The late course of the disease was followed by computerized tomography and serum hormone assays. Previously reported cases of metastatic interstitial cell tumor are reviewed, and prognostic features and treatment are discussed.

Transcription terminates near the poly(A) site in the CYC1 gene of the yeast Saccharomyces cerevisiae.

A 38-base-pair region required for normal CYC1 mRNA 3' end formation in Saccharomyces cerevisiae was shown to be necessary for the termination of transcription in vivo by examining the stability of CEN3 plasmids. CEN3 plasmids were stably maintained during vegetative growth, unless a GAL1 transcript impinged on the CEN3 region. Transcription from the GAL1 promoter was terminated, and plasmid stability was restored by the insertion of a fragment containing the 38-base-pair region of CYC1. In contrast, a similar fragment lacking the 38-base-pair region had no such stabilizing effect. Furthermore, CYC1 mRNA transcription terminated in a region less than 100 nucleotides downstream from the normal poly(A) site, thus establishing that CYC1 mRNA 3' end formation does not involve overly extended precursors as are observed in higher eukaryotes.

Relationship between urinary kallikrein excretion and plasma atrial natriuretic factor in essential hypertensives.

Relationships between renal kallikrein and plasma atrial natriuretic factor were investigated by measuring urinary kallikrein excretion (by spectrophotometric assay) and plasma atrial natriuretic factor (by radio-immunoassay after extraction from plasma) in 84 normal subjects and in 104 essential hypertensives with a normal renal function. The atrial natriuretic factor was significantly higher in the essential hypertensives than in the normal subjects (38.5 +/- 1.3 versus 29.0 +/- 1.3 pg/ml, P less than 0.01), whereas urinary kallikrein excretion was significantly lower in the hypertensives than in the normal subjects (11.1 +/- 0.9 versus 15.3 +/- 0.6 nkat/24 h, P less than 0.01). Taking the 95% confidence limits of urinary kallikrein excretion in the normal subjects (from 4.5 to 29.9 nkat/24 h), we divided our essential hypertensives into two subgroups, patients with a normal kallikrein excretion (n = 80; mean urinary kallikrein excretion 13.8 +/- 0.8 nkat/24 h) and patients with a low kallikrein excretion (n = 24; mean urinary kallikrein excretion 2.3 +/- 0.3 nkat/24 h) The patients with a normal kallikrein excretion had a mean plasma atrial natriuretic factor value of 31.9 +/- 1.2 pg/ml, which was similar to that found in normal subjects. In contrast, the mean plasma level of atrial natriuretic factor in the patients with a low kallikrein excretion (50.7 +/- 2.2 pg/ml) was significantly higher than that measured in the patients with a normal kallikrein excretion and in the normal subjects (P less than 0.01 versus patients with normal kallikrein excretion and normal subjects, respectively).(ABSTRACT TRUNCATED AT 250 WORDS)

Spectral analysis of pendular waveforms in congenital nystagmus.

We analyzed quantitatively electronystagmographic recordings of pendular waveforms via spectral analysis methods. We found (1) symmetrical pendular oscillations in orbital positions spanning a range as wide as 40 degrees, with frequencies between 2 and 8 Hz; (2) stable and small pendular oscillations in patients without a null zone and unstable and large pendular oscillations in patients with a neutral zone; (3) a non-decreasing relationship between the amplitude and the frequency of the eye oscillations, and (4) pendular components in orbital positions at the two sides of a neutral zone. It is argued that pendular nystagmus can be generated independently of normal (jerk) nystagmus in the same orbital position.

On the variations of the time constant of the slow-phase eye movements produced by surgical therapy of congenital nystagmus: a preliminary report.

Quantitative analyses of the effect of nystagmus surgery have been based previously on the pre- and postoperative comparison of the nystagmus intensity at various gaze angles. On the other hand, a recent hypothetical model of the defective oculomotor system producing congenital nystagmus emphasizes that the time constant of the (increasing velocity) slow-phase eye movements should represent a useful parameter in the quantification of the surgical effects, since it is related to the potential good visual acuity of the patients in the presence of nystagmic cycles with extended foveation. By computer analysis of the pre- and postoperative electro-oculographic recordings from 5 patients with congenital nystagmus we show experimentally that the above theoretical indication is a valid tool in the assessment of the effects of classic nystagmus surgery.

Amount of surgery in congenital nystagmus.

By analyzing the clinical data on the amount of surgery on the extraocular muscles and resulting angular shift of the null zone in 24 patients with congenital nystagmus, a linear dependence was found to exist between these two variables. Hence, the amount of surgery can be confidently established in advance of treatment in order to obtain the shift of the null zone. Many cases of partial surgical successes are discussed and explained a posteriori by a lack of a proper amount of surgical rotation.

Biochemical and enzymatic aspects of caffeine and caffeine derivatives induced DNA strand breaks.

The results presented here point to the difficulties that exist in connection with the identification of the molecular target of caffeine. Our data support the evidence that caffeine and caffeine derivatives cause DNA-protein cross-links (DPC) in whole mammalian cells or in isolated nuclei. These DPC have the same properties (saturability, reversibility and temperature-dependence) as those produced by an enzymatic inhibition. The experiments performed in reconstituted systems, in the presence of purified DNA topoisomerase II, do not support the original hypothesis that this enzyme might be a possible target for this class of drugs. We suggest the possibility that other DNA metabolism enzymes are involved in the biological effects of caffeine and caffeine derivatives. Further biochemical and molecular data are necessary to identify which of these enzymes is in fact affected.

DNA damage induced by some bile acids, evaluated with alkaline elution technique.

Bile acids are promoting agents in colon carcinogenesis. In this work we have tried to characterize the DNA alteration induced by bile acids in Sprague-Dawley male rats. Confirming previous findings, a clear increase in elution rate was observed at alkaline pH. No effect could be observed when the nuclei were washed before the elution, in condition totally unsuitable for the repair of the type of DNA damage induced by typical genotoxic agents. We advanced the hypothesis that the increased alkaline elution rate observed with bile acids could be independent of DNA fragmentation and related to changes in chromatin structure.
