Urothelial atypia and survival rate of 500 unselected patients with primary transitional-cell tumour of the urinary bladder.

In a consecutive series of 500 unselected patients with primary urinary bladder tumours the influence of urothelial atypia on the 5 years survival-rate was examined. All tumours were transitional-cell tumours categorized according to the T-classification. Mucosal biopsies from 7 pre-selected sites were taken at the initial cystoscopy in 391 patients (78%) to identify urothelial atypia. The over-all cumulative 5 years survival-rate was 48%. Submucosal and muscle invasion had major influence on survival, whereas tumour grade was less important. Patients with urothelial atypia fared significantly worse than those with normal bladder mucosa (5 years survival 42% versus 62%). This difference in survival-rate became apparent first after two years of observation. Grade II atypia in the bladder mucosa and grade III (carcinoma in situ) had equal significance assessed by the survival-rates.

Control for recurrences of urinary bladder tumours by transabdominal ultrasonic scanning.

By transabdominal ultrasonic scanning of the filled bladder it is possible to examine the surface of the bladder urothelium in the outpatient clinic. In a "blind" study of 129 patients controlled for recurrences of urinary bladder tumours the results of dynamic transabdominal ultrasonography were compared with the results of cystoscopy. The ultrasound could identify the recurrences of 5 mm or above in size significantly. Below 5 mm in dimension, when positioned in the dome of the bladder or at severe trabeculation the ultrasonic scanning lead to a misdiagnosis. No invasive recurrences were overlooked by ultrasonography. We will advocate that transabdominal ultrasonic scanning replace routine cystoscopy in low-risk patients with superficial bladder tumours of the Ta category and a low grade after the primary transurethral treatment and no or few recurrences of the same Ta category at the first control cystoscopy. On the other hand we will recommend to continue cystoscopy of patients with frequently recurring urinary bladder tumours, the high grade Ta tumours, carcinoma in situ because of the risk of invasive growth, and patients with primary invasive tumours including those with the superficial invasion (category T1).

Colorectal bleeding localized with gamma camera.

Localization of colorectal bleeding using a gamma camera was attempted in a prospective study of 24 patients after in-vivo labelling of their erythrocytes with 99mTc-pertechnetate. In 7 of 8 patients with bright-red rectal bleeding a correct localization was given by scintiphotos, thus helping the surgeon to plan the operative procedure. Of 11 patients with recently acute colorectal bleeding but no signs of actual haemorrhage, 8 had no scintigraphic bleeding. Five investigations had to be excluded due to technical errors. The scintigraphic classification was correct in a significant number of patients (p less than 0.02). It is concluded that bleeding can be identified in colorectum at brisk-haemorrhagic stools and be correctly localized to the right or left side of the colon. This non-invasive scintigraphic procedure should therefore be undertaken before an abdominal 2-3 vessel arteriography is performed.

Ureterocele in adults. A follow-up study of 28 adult patients treated with transurethral diathermy incision.

During an 18-year period, 28 adults with ureterocele were admitted to hospital with uncertain urological or abdominal symptoms. All of the ureteroceles were treated with transurethral diathermy incision. At follow-up examination after a mean observation time of 7 years, 16 of 20 patients were free from symptoms and the other 4 had only minor loin pain or dysuria. Significant vesico-ureteric reflux or depression of the renal filtration rate did not develop in any case. It is concluded that simple transurethral incision is adequate for treatment of small or medium-sized ureteroceles in adults.

The urodynamic patterns and pathophysiology of the canine allotransplanted upper urinary tract.

The urodynamic patterns of pelvic pressure, pelvic volume and frequency of ureteral peristalsis and the pathology of the upper urinary tract after renal allotransplantation were investigated in 41 dogs. The findings were compared with previous measurements of both normal and renally autotransplanted dogs. The urodynamic patterns of the allotransplants were unchanged and not influenced by variations of renal function or degree of rejection. Morphologic alterations of collagen infiltration in the smooth muscle tissue changed the viscoelastic behaviour of the system. Finally, the results support that myogenic impulses govern the urinary transport from the canine kidney to the bladder.
