A biometric study of higher alcohol production in Saccharomyces cerevisiae.

A hundred strains of Saccharomyces cerevisiae were examined for the ability to produce higher alcohols. In the strains tested the production of higher alcohols was found to be an individual strain characteristic and, as such, was statistically significant. The characteristics of the strains used (flocculation ability, foaming ability, killer character, and non-H2S production) were found to be uncorrelated to isobutanol and isoamyl alcohol production, whereas the production of high levels of n-propanol was found to be related to inability to produce H2S. This, in turn, suggests a link to methionine biosynthesis.

[Subcutaneous tunnelization of the subclavian vein. A bacteriological study of 400 catheters in hematologic intensive care].

Infectious complications are the highest risk in hematology. In this study, subclavian approach and tunnellization are reviewed. Four hundred (400) catheters were evaluated by the Isolator Culture method. Catheters are left in place a long time (mean time 30 days). Colonization occurs rarely (about 10.5%). In hematology, subclavian tunnelized catheter occurs to be a safe and reliable central venous catheter.

[Experience of health personnel at an organ procurement center].

Organs' procurement oblige to take care of a great number of brain death patients. This study concerns the psychological influence of such a situation on teams, nurses and physicians of this specific activity of harvesting. It took place in three organs' procurement intensive care units and interested sixty three men and women. These teams reveal the difficulty to live "the brain death", according to brain death patients themselves and their families, but also to transplantation: to work on brain death patients for the life of other patients. So it seem necessary to give a place at a psychologist in this specific intensive care units not for the patients, they are dead, but for the teams.

Intramedullary osteosynthesis 3. Küntscher nailing in the humerus.

With regard for the principles that govern the correct application of intramedullary osteosynthesis in the long bones of the lower limb, the authors also support its use in the humerus. After an exhaustive survey of the specific problems at this site, a critical review of 130 cases observed over the last 15 years is reported. Extremely favourable results emerged in the treatment of the humerus with this means of synthesis, thanks to its surgical, mechanical and biological advantages.

Intramedullary osteosynthesis. 4. Küntscher nailing in the ulna.

Intramedullary osteosynthesis with Küntscher nailing is an appropriate and extremely effective operation in the treatment of diaphyseal fractures of the ulna. Like the other long bones of the limbs (femur, humerus, tibia), in the ulna, as well, the advantages of the method, particularly when performed in closed surgery, are undeniable, so much so that this bone has a shape which is particularly favourable to nailing. The authors present and discuss their personal series of cases reporting results which are excellent. Thus, once again, the great reliability of this simple means of synthesis, in use for more than 20 years, is emphasised, and the subject is completed with a series of previous technical suggestions on how to obtain the best results.

[Percutaneous angioplasty in the treatment of iliac artery stenosis].

PTA represents a slightly invasive method for treatment of iliac arteries stenoses. The Authors present their experience about this topic and discuss current indications in the use of this technique. They believe that beyond the already defined indications, such as treatment of short stenoses, transluminal angioplasty can be employed for long stenoses too, eventually combined with limited surgical procedures. They stress the importance of combining traditional PTA with new percutaneous or intraoperative Argon Laser technology, that surely will contribute to broad its indications and to improve its results.

Intramedullary osteosynthesis. 1. Küntscher nailing in the femur.

Based on more than 20 years experience, the authors support the effectiveness of intramedullary nailing in the treatment of diaphyseal fractures of the femur. They review the history and advantages of this method and discuss the biomechanical principles on which it is based. Recent advances in technique have made it possible to secure proximal and distal anchorage in cancellous bone. This allows the method to be extended to more proximal and distal fractures than in the traditional method which is applicable only to the middle third. It also facilitates progressive axial compression, early loading and early mobilisation of the limb, thus creating the optimum biological conditions for consolidation and joint function. The closed method also reduces blood loss and the risk of infection. The techniques adopted to deal with more complex situations are described. These include moderately comminuted oblique and spiral fractures and certain bifocal fractures. Finally, the results of treating 450 cases are reviewed, with a longterm analysis of 198 patients.

The association of two or more surgical methods in the tibia.

The authors propose the association of two or more surgical methods in the treatment of aseptic pseudarthrosis of the tibia with the object of increasing the probability of union and reducing the period of disability. Based on the anatomo-pathological features of the pseudarthrosis, methods aimed at obtaining mechanical stability (fibula pro tibia, Küntscher) and methods which stimulate biological repair (decortication, transplants) are associated. The results obtained confirm the theoretical presuppositions, revealing a relatively short amount of time required for healing.

Intramedullary osteosynthesis. 2. Küntscher nailing in fractures of the tibia.

The purpose of this study is to discuss diaphyseal fractures of the tibia treated by intramedullary Küntscher nailing. This is based on 368 cases treated over the last 20 years. Closed intramedullary osteosynthesis favours the healing process, resulting in the early formation of callus which is of good quality and abundant. A careful review of the cases showed a 93.7% incidence of good results.

The problem of carcinogenic hazard evaluation for new chemicals: general considerations about the carcinogenic process.

We discuss the problem of oncogenic hazard evaluation for new chemicals. In the recent past, assessment of global carcinogenicity in rodents was considered the most significant type of information. It has recently emerged that this type of information is not adequate to distinguish initiation and genotoxicity from promotion-like effects. In this report we suggest that hazards from initiating agents and hazards from promoting agents should be treated separately. In this perspective, longterm experiments for carcinogenicity in rodents should still play an important role but a less central one. Initiation-promotion experiments in different target organs are recommended. We suggest a strategy of hazard evaluation related to the initiating potential as distinct from overall carcinogenicity. The problem of utilizing not only the qualitative component of the available information, but also the quantitative component, is considered. Finally, we discuss a possible hazard evaluation for promoting effects, but conclude that this area is very much in its infancy. Possible contributions of computer science technology to the problem of oncogenic hazard evaluation are briefly introduced.

[Surgical treatment of thrombosis of the inferior vena cava].

Personal experience of thromboses of the inferior vena cava is presented and pathogenetic and symptomatological aspects stressed. In addition, in relation to the results presented and reported data, it is concluded that the role of surgical therapy is fundamental to integrate the thrombolytic and anticoagulant treatment of these severe forms of deep venous thrombosis.

Pharmacological bridge to cardiac transplantation.

From September 1985 to August 1988, 32 patients were referred from various intensive care units throughout Paris for urgent cardiac transplantation or for a mechanical bridge to transplantation. At time of admission, under maximal sympathomimetic therapy, the cardiac index (CI) was 1.81 +/- 0.26 l/min per m2, the pulmonary capillary wedge pressure (PCWP 31 +/- 7 mmHg), systemic vascular resistances (SVR) 2053 +/- 469 dynes s cm-5. In 25, diuresis was less than 25 ml/h. Five were anuric. Prior to any final decision, a new inotropic agent, enoximone, was infused in addition to previous treatment as a 10 min bolus iv 1.5-2 mg/kg every 8 h. In 3, the situation further deteriorated, leading to a Jarvik 7-70 implantation within 12 h. In 29 however, within 3 h, the Cl increased to 2.69 +/- 0.56 as SVR dropped to 1410 +/- 453 and PCWP to 18 +/- 7. Diuresis increased to more than 100 ml/h in all. This permitted an indepth evaluation of the transplant candidates leading to contraindications to transplantation in 16. Nine patients could be weaned off iv enoximone. Four of these are still living (NYHA class III) with a follow up of 6-17 months. In 11, transplantation was performed within 2 days. Four died within a month, 2 with multiple organ failure. One patient died after 5 months. Six are back to normal life, NYHA class I (follow up 10 months-2.5 years). This protocol suggests that in patients with extreme heart failure, immediate survival may be increased by iv enoximone therapy, permitting a better selection of the recipients, more efficient pre-transplantation intensive care and consequently a decrease in the indications for a temporary mechanical bridge to a staged transplantation.

Combined heart and kidney transplantation using the same donor.

A 45-year-old man received a combined heart and kidney transplant provided by the same donor. The patient was in the terminal stage of renal failure and was submitted to 3 sessions of haemodialysis per week for 2 years before transplantation. A dilated cardiomyopathy with severe impairment of left ventricular (LV) function was discovered, precluding renal transplantation alone. In the postoperative course, a cardiac rejection episode was detected by echocardiography and documented by endomyocardial biopsy; no simultaneous rejection of the kidney could be detected. The patient was discharged after 2 months, and he resumed a normal and active life. The specific problems raised by the management of such multi-organ transplantation are briefly reviewed.

Alignment of the extensor apparatus of the knee by alar and tuberosity transposition in recurrent subluxation of the patella and in external patellar hypertension syndrome.

In consideration of the multiple anatomical structures involved in recurrent subluxation of the patella and in external patellar hypertension syndrome, the authors propose an operation which allows for complete realignment of the extensor apparatus by transposition of the anterior tibial tuberosity, division of the external alar ligament, tightening of the internal alar ligament, and correction of the muscular insertions. In particular, removal of a capsulofascial band medially and its reimplantation into the external incision is proposed, thus avoiding the formation of retractive scar tissue. The long-term results, which were overall satisfactory, confirm the effectiveness of the operation proposed.

[A new type of small calibre carbon fiber arterial prosthesis].

A new type of vascular prosthesis, made of pyrolytic carbon fibres, was tested on growing pigs. Fourteen grafts (4 x 33 mm) were implanted on the infrarenal aorta and removed 15 s, 60 m and 7, 15, 30, 60 and 120 days after surgery. Specimens were examined by light and scanning electron microscopy. As controls ten pigs underwent excision and re-implantation of a 30 mm segment of infrarenal aorta. Autografts were removed 7 to 120 days after surgery. All but one carbon grafts, as well as all autografts were patent at the time of removal. Basic characteristics of carbon grafts, as shown by the present study, may be summarized as follows: 1) thromboresistance of the inner surface, at the time of implantation, 2) development of a thin neo-intima, 3) rapid and complete endothelialization of the flow surface, 4) good viability of the inner layers and 5) solid anchorage to peri-graft tissues.
