[Limitations and perspectives of postoperative radiotherapy in bronchial cancer].

The role of postoperative irradiation for lung cancer remains a controversial issue. The available data suggest a reduction in local relapse in cases of positive mediastinal lymph node, but how this benefit translates into survival is not known. The current indications include tumors with positive mediastinal lymph node and incomplete resection with micro- or macroscopical residue. Nevertheless, postoperative irradiation requires a meticulous technique to avoid inducing life-threatening complications to vital organs such as the heart or the lung.

[Four cases of hemorrhages after surgical repair of iatrogenic tracheal stenosis (author's transl)].

In our series of 23 patients, operated 27 times, for a iatrogenic stenosis of the trachea, there were 4 deaths by hemorrhage of the innominate artery. Three resulted from shearing of the artery by the tracheal sutures. One came from erosion by a tracheostomy cannula. This fatal complication should be prevented by preserving healthy tissues between the artery and the trachea.

[Thoracic empyema. Focus on bacteriology and therapy].

Treatment of empyema thoracis is a difficult one, especially when it occurs in a large residual pleural cavity or with a bronchopleural fistula. Sixty three patients have been studied: 39 post-surgical cases (pneumonectomy 21, lobectomy 14, therapeutic pneumothorax 4) and 24 cases without previous surgical procedure. Bacteriologic findings were as follows: 19% of the cultures were negative; Staphylococcus aureus has been isolated in 43% of the positive cultures, Gram negative bacilli in 40%, anaerobic germs in 21.5% and Streptococcus pneumoniae in one case. In 55 patients (87%) initial treatment was pleural drainage and local irrigations associated with systemic antibiotherapy. Enzymatic debridement with streptokinase was necessary in 25 patients (40%) and intrapleural antibiotherapy in 19 patients (30%). Twenty two patients needed subsequent surgical treatment: 12 pleural debridements, 2 pleuropneumonectomies and 12 open-window thoracostomies were performed. Fourty three patients were cured; 11 died (7 related to the primary disorder) and 8 developed chronic infections. Effective pleural drainage must always be the first treatment in empyema thoracis. When this fails, surgical treatment is imperative. Pleural debridement is the procedure of choice when there is no residual pleural cavity and when the lung can be reexpanded. Pleuropneumonectomy must be reserved to extensive pulmonary destructions. Open-window thoracostomy is the best technique when large residual pleural space or bronchopleural fistula occurs (e.g. after pneumonectomy), and after the failure of all other treatments.

Combination chemotherapy with cisplatin, etoposide, and vindesine in non-small cell lung carcinoma: a clinical trial of the EORTC lung cancer working party.

A combination of cisplatin (60 mg/m2 on Day 1), etoposide (120 mg/m2 on Days 3, 5, and 7), and vindesine (1.5 mg/m2 on Days 1 and 7), repeated every 3 weeks, was administered to 73 patients with non-small cell bronchogenic carcinoma. After two full courses, the results could be evaluated in 62 patients, 25 (40.3%) of whom responded (five complete responses, 20 partial responses). The median survival for the responding patients (12 months) was significantly superior (P = 0.02) to that of the nonresponding patients. There were three early toxic deaths from sepsis associated with granulocytopenia, and seven patients presented peripheral neuropathy. Although active in non-small cell bronchogenic carcinoma, the combination of cisplatin, etoposide, and vindesine does not appear to be superior to cisplatin-etoposide or cisplatin-vindesine when our previous experience and the results reported from other institutions are considered.

Training in surgery: our constructive propositions.

Several suggestions are presented in order to achieve or to maintain a level of excellence in surgical training, i.e. 1. Interuniversity theoretical teaching; 2. National evaluation; 3. Effective limitation of the number of residents; 4. Residents must be represented in critical organs; 5. A yearly evaluation of the quality of training. These requirements are fulfilled in the USA and Canada.

[Should we still operate small cell carcinoma of the lung? (author's transl)].

Two hundred and nineteen cases of small cell carcinoma of the lung are reviewed. The 5-year survival rate is less than 5%. Four patients survived more than 5 years, among 9 tumors resected with curative intention. Palliative resections do not enhance the survival rate. Combined radiotherapy and polychimiotherapy have a high response rate and tend to extend the median survival interval. Thus, small cell carcinoma require an accurate pathological diagnosis and extensive staging. Only small, localized peripherical tumors (T1N0M0, T2N0M0) should be resected.
