[Pure ovarian dysgerminoma. Evaluation of experiences and definition of a therapeutic strategy].

The authors present a series of 10 pure dysgerminoma of the ovary treated between 1976 and 1984; mean age = 19.5 years, range 15-46 years. All patients had initial surgery: 8 annexectomies for 5 stages Ia and 3 stages IIIc nodal disease, 1 hysterectomy with bilateral adnexectomy for a 46 year old stade Ib patient and 1 case of salvage surgery for a progressive disease after a single adnexectomy. Two patients had no adjuvant therapy after initial adnexectomy (stage Ia tumors less than 10 cm in diameter). Four patients received a prophylactic subdiaphragmatic radiation therapy (3 stages Ia tumors larger than 10 cm, 1 stage Ib disease). Three patients received an irradiation for a subdiaphragmatic bulky disease and a prophylactic supradiaphragmatic irradiation (stage IIIc nodal disease). Two patients received chemotherapy, one for recurrence, the other for progressive disease. The authors discuss the therapeutic indications of pure dysgerminoma of the ovary, especially the conservative management of young patients wishing to preserve an hormonal and obstetric future and the value of radiologic and serologic follow-up. Recent data in the literature underline the efficacy of new combinations of cytotoxic agents and their role as an adjuvant of surgery in early as well as in advanced stages.

[Treatment of cervical lymph nodes in lip cancers].

From 1970 to 1985, 299 patients with lip cancer were examined, treated and followed-up at the Bergonié Foundation. In most cases, interstitial radiotherapy was used and the local control rate reached 94%. Local recurrences could usually be treated again, so that the final local control rate was 99%. However, the management of neck nodes remains controversial in some cases: 5.6% only of T1-2 N0 stages developed neck nodes, which were successfully controlled in 78.9% of cases and it was therefore considered that the preferred option should be to maintain the patients under close follow-up. For T3 N0 cases, of which 17.6% would evolve, it was considered that a sub-mental and sub-maxillary neck node dissection was advisable when performance status was satisfactory and when a close follow-up was difficult. The recurrence rate was 40% for patients with palpable neck nodes who were often submitted to a combined radio-surgical treatment. In such cases, recurrences were controlled in 22% of patients who died 7 times out of nine.

[Leukocyte removal in a closed system of human red-cell concentrates: a technic coupled with the automated extraction of the buffy coat using a sterile connector from a filtration kit].

A technique, integrally run in a closed system, for leucocyte depletion of human red cell concentrates is described. It associates two complementary processes: buffy-coat removal and filtration. The first step is carried out with an automated system for blood component preparation (Compomat, NPBI); its efficiency is improved by a custom made blood collection set with ACD anticoagulant solution in the primary bag. The second step is simplified by a filtration kit requiring only one sterile connection for operation (SCD 312, Dupont de Nemours) and allowing a standardised rinsing of the filter. Quality control of 33 units so prepared shows principally: --an intensive leuko-depletion (4 logs) enabling leukocyte contamination to be kept below 10(6) per unit; --a moderate red cells loss (15 ml for the first step and 20 ml for the second one). This technique provides a permanently available and very pure blood component. Moreover it offers new potential for standardisation and mastery of quality control.

[Viability of human red blood cells preserved for 35 days after leukocyte depletion (in vitro study)].

24 leukocyte poor red cells concentrates (L.P.R.C.) were prepared by sterile connection of a leucocyte filter between the primary bag and the SAGM bag of a blood unit after centrifugation. Their quality was followed up to 42 days by means of a panel of tests including, ATP and 2,3-DPG levels, hemolysis, plasma potassium, lactate and glucose, and counts of the microaggregates. 24 standard units acted as a control group. Results showed better preservation of LPRC and especially less hemolysis, higher ATP levels and at least equal oxyphoric capacity (explored by 2,3-DPG). Microaggregate formation was dramatically reduced and bacteriologic checks (48 at day 25 and 48 at day 42) were all negative. Leucocyte depletion appears as a new way to improve functionality of erythrocytes during storage in the SAGM medium. 35 days shelf life will allow this blood product to be more available and its preparation more standardised.

[Brachytherapy of cancer of the penis. Analysis of a series of 53 cases].

From 1970 through 1984, 53 patients with squamous cell carcinoma of the penis have been treated by interstitial irradiation with iridium 192 wires; in this group 33 patients have been followed for at least ten years. There were 7 T1, 31 T2, 15 T3 and 37 N0, 7N1, 6 N2, 3 N3 (WHO classification, 1979). Forty eight patients were treated by interstitial radiotherapy alone, after previous circumcision for 35 of them, and five by an association of external and interstitial radiotherapy. Eleven patients presented a local recurrence; all but one were controlled by penile amputation. Fifteen patients developed severe complications (necrosis, urethral stenoses treated by surgery) and ten of them underwent a secondary total or partial penile amputation. Complications are strongly correlated with the irradiated area and the dose (over 65 grays). Recurrences and complications may develop very late after the treatment, beyond ten years. They required 12 partial and 10 total amputations. Interstitial radiotherapy is the first line treatment for carcinoma of the penis and it is well accepted by the patients. However, to keep a reasonable rate of complications and recurrences we limit the indications of interstitial radiotherapy to the small lesions (T1-T2) and we suggest to decrease the dose under 65 grays. To avoid some local failures we treat now the whole glans.

Localized follicular lymphomas: prognosis and survival of stages I and II in a retrospective series of 103 patients.

From 1966 to 1985, 103 patients with a localized (stages I and II) follicular lymphoma (Kiel classification) were treated at the Fondation Bergonié. Clinical staging was performed using, after physical examination, chest X-rays (100%), bipedal lymphangiography (98%) and unilateral bone marrow biopsy (BMB) (51.5%). The patients were then treated by radiotherapy with or without chemotherapy. Overall survival (OS) at 5 and 10 years is 69 and 56.3%, respectively. Relapse-free survival (RFS) is 53.7 and 49%. Unifactorial analysis shows three prognostic parameters to be independently significant in terms of OS: age, stage and B symptoms. In terms of RFS, only 2 factors are significant: age and B symptoms. Multivariate analysis (Cox model) shows that age is a more important prognostic factor than stage.

Clinical study of an LH-RH agonist (ICI 118.630, Zoladex) in the treatment of prostatic cancer.

Eighty patients with prostatic cancer have been treated with an LH-RH analogue (Zoladex). Ten had no metastasis, and hormone therapy was used as an induction treatment before curative radiotherapy. The others had metastatic disease and, in some cases, had already received some form of endocrine therapy. Patients received a monthly injection of Zoladex (3.6 mg). No progressive disease was noted among patients with nonmetastatic tumors; of the patients with metastases, those who were previously untreated had a higher response rate (14.8% complete response) and longer progression-free and overall survival. Toxicity was mild in spite of two cases of disease flare.

Comparison between a solid-phase low-ionic-strength solution antiglobulin test and conventional low-ionic-strength antiglobulin test: assessment for the screening of antierythrocyte antibodies.

A solid-phase low-ionic strength salt antiglobulin test (LISS-SPAT) has been developed using a microplate coated with dried sera as a solid phase. Before coating, the in vitro C3d fragment generation was activated by adding heat-aggregated immunoglobulin. The LISS-SPAT was compared with low-ionic strength conventional antiglobulin test (LISS-AGT) and also with a test using polybrene or papain microplates. When detecting the IgG and IgM antierythrocyte antibodies the reaction was developed in the same way in LISS-SPAT and LISS-AGT. In routine work, the LISS-SPAT provides a fast, reliable, handy and inexpensive screening of antibodies. This method appears to be an additional method to the papain and polybrene tests in microplates.

[Contribution of the combination of radiation and surgery in the treatment of invasive pharyngo-laryngeal carcinoma].

On the basis of the retrospective analysis of 269 cases of pharyngo-laryngeal carcinoma, the authors recall that the degree of tumour invasion is one of the essential prognostic factors for this disease. The macroscopic appearance of the tumour should therefore play a determinant role in the choice of treatment, as invasive lesions are generally responsive to radiotherapy.

[Cancer of the prostate: estrogen therapy].

From 1941, estrogens are usually used in the treatment of prostatic carcinoma, wrongly in case of adjuvant hormonotherapy and exceedingly because of cardiovascular toxicity from over dosages. A recall of their mechanism of action and toxicity allows to argue about the palliative treatment and to look to their use for the locally advanced cancers in order to get a tumoral reduction before radiotherapy. A pilot study realised by the authors shows that this hormono-radiotherapic protocol is achievable.

[Metastatic cancer of the prostate: phase II study of spirogermanium (NSC 192965)].

A phase II study of spirogermanium was conducted in a series of 15 patients with metastatic prostatic carcinoma. All the patients have previously received multiple hormonal therapies. The drug was administered at the dose of 200 mg/m2 by a continuous infusion for five days, and 120 mg/m2, three times a week subsequently. The side effects were mainly neurological toxicity and phlebitis at the injection points which were dose and schedule dependent. Only one partial response for two months was noted in this series. Thus, spirogermanium seems to have a limited value in patients with prostatic cancer.

[Conservative loco-regional treatment of soft tissue sarcoma in adults after induction chemotherapy].

Between June, 1979 and December, 1983, 23 adult patients with non-metastatic soft tissue sarcoma were treated with induction chemotherapy since local spread precluded primary conservative surgery. Eighteen patients received a combined cyclophosphamide, vincristine, adriamycin and dacarbazine treatment and 5, a combined adriamycin, cis-platinum and vindesine treatment. Both drug combinations exhibited toxic effects, notably on blood and digestive tract. However, tumoral volume was reduced by 50 p. cent or more in 14/23 patients, including 2 complete remissions; the course of the disease remained unchanged under treatment in only 3 cases. Chemotherapy facilitated surgical excision in 14 patients, and subsequent radiotherapy resulted in complete remission in 13 of these. None of the 13 patients relapsed during a mean follow-up period of 12 months (range: 3 to 48 months). Among the 9 patients who could not be operated upon, 5 benefited from radiotherapy with satisfactory local control, but only one still remains in complete remission. Thus, induction chemotherapy made it possible to achieve local control in two-thirds of patients with locally advanced sarcoma. However, a more prolonged follow-up is necessary to confirm that these results are long-lasting and to determine the effects of chemotherapy, if any, on the metastatic potential of these tumours.

Cytochrome c oxidase in plant mitochondria. Electron transfer after flash photolysis at low temperature of the CO-bound half-reduced complex.

Rebinding of CO after flash photolysis of the carboxy mixed-valence cytochrome c oxidase in plant mitochondria has been monitored in the visible range at low temperature (160-210K). At 586/630 nm, the photodissociation of the CO-saturated suspension was followed by a largely incomplete rebinding of CO to Fea3. After a series of three to four flashes, the unbound species accumulated up to 85% of the total enzyme, the remaining 15% CO-bound form undergoing recombination repetitively. In the Soret region, the absorbance level after the first flash came back to the initial one, as if all the CO-bound form were restored. However, the amount of complex which could be photodissociated by a second flash was only part of the starting material, in agreement with observations in the alpha range. The unbound species exhibited optical features very similar to the ones of compound C. The lack of CO rebinding is interpreted as due to an electron redistribution induced by the flash photolysis. This electron transfer has been tentatively assigned to a concomitant CuB oxidation and CuA reduction on the basis of the 655 nm band as a probe for a3+3 [Beinert et al. (1976) Biochim. Biophys. Acta 423, 339-355] and of the optical features in the visible range. Results are discussed with respect to analogous investigations with beef heart mitochondria and the studies by Boelens and Wever (1979-1982) of electron transfer after photolysis of the carboxy mixed-valence state when using the isolated mammalian enzyme at room temperature.

[Cancer of the oral cavity without palpable adenopathy: value of external irradiation exclusively].

Between 1970 and 1978, 99 patients with a squamous carcinoma of the buccal cavity without palpable lymphadenopathy were treated at the Bergonié Foundation by radiotherapy only and regularly followed up. All received a minimal dose of 40-50 Grays either to the primary node groups or the entire cervico-supraclavicular areas. Eight patients developed an isolated lymph node recurrence, in two cases only in the irradiated zone. The other 6 lymph node recurrences occurred either below the radiotherapy fields when the latter had been limited to the neck (4 cases) or at the posterior border of the field (2 cases). Secondary control was possible in two patients of the 8 by combined radiotherapy and surgery. Exclusive radiotherapy for lymph node areas up to a dose of 45-50 Grays thus protected 92% of patients from a lymph node recurrence. The methods of such irradiation and in particular the extent of radiotherapy fields are then discussed on the basis of the clinical aspects of the tumour.

A near-infrared investigation of cytochrome c oxidase in higher plant mitochondria.

Optical features of cytochrome c oxidase in potato mitochondria have been characterized in the near-ir region. In order to discriminate the respective properties of the various redox centers, the redox state was monitored from free and inhibited, bound species. Appropriate comparisons singled out difference spectra which can be attributed specifically to CuA and CuB. The CuA difference spectrum (red-ox) exhibits a negative band centered at 812 nm and, analogous to its mammalian counterpart, the so-called 830-nm band (delta epsilon red/ox = -2.0 mM-1 cm-1). The unusual difference spectrum (red-ox) assigned to CuB is characterized by a broad positive band also centered at 812 nm with an extinction coefficient of delta epsilon red/ox = 4.3 mM-1 cm-1.

[Prognostic factors in cancer of the endometrium and therapeutic indications].

Between 1970 and 1979, 270 patients were treated at Fondation Bergonie for carcinoma of the endometrium (169, for the whole treatment). The cases were classified according to the FIGO staging system. The mean age of the patients was 62.2, with a range from 31 to 95. 62% of patients had stage I carcinoma, 16% stage II, 18% stage III and 4% stage IV. The operability rate, according to the patients ages was considered a main factor in the prognosis. The 5 years survival was 64% for stage I, 60% for stage II, 47% for stage III and 0% for stage IV. Histologic grade of the tumor and depth of myometrial invasion are also important prognosis factors; both influencing survival. This study compared with others, provides the main prognosis factors: age and operability, stage of the tumor, histologic grade, depth of myometrial invasion and pelvic and/or para-aortic lymph nodes involvement. The authors conclude by specifying the indications for treatment.
