[Bone and joint transplantation. Alternatives to bridging segmental bone defects and a model for allogenic bone transplantation].

Fresh allogeneic diaphyseal bone segments were transplanted into 12 dogs. We attempted to improve the immunogenity and incorporation of the bone segments through "graft manipulation" with and without immunosuppression. Through clinical and radiological study during the 20-week experimental period, we observed transplant healing with differing structural phenomena. After autologous knee joint replantation in 4 dogs and allogeneic transplantation in 2 dogs (with and without revascularization and immunomodulation with Cyclosporin A) we present a canine model to investigate all open questions about the transplantation of allogeneic, fresh joints (i.e. transplant biology, immunology, and vitality).

Segmental bone grafting. Comparison of different types of graft in dogs.

We have compared the biological behaviour of eight types of segmental tibial grafts in dogs under different immunological conditions: autografts, allografts, allografts with short-term immunosuppression (4 weeks), and allografts with longterm suppression (20 weeks). Some grafts were revascularised by microsurgical anastomosis, others were not. The long-term immunosuppressed allografts are most similar to autografts in terms of perfusion pattern and graft-host interface healing. The difference between long-term immunosuppressed allografts and allografts which were not immunosuppressed was highly significant (P less than 0.01). The viability of long-term immunosuppressed allografts, as measured by fluorochrome-labelled osteons, did not reach the level of the autografts, but still differed significantly from the allografts which were not immunosuppressed. Immunosuppression, therefore, improves the survival and the quality of the graft, more so for long-term immunosuppressed grafts, and also in revascularised grafts.

Treatment of subtrochanteric femur fractures using the dynamic condylar screw.

Thirty-two consecutive subtrochanteric femur fractures were treated using the 95 degree dynamic condylar screw between 1982 and 1985. All fractures were classified according to the radiographic criteria of Seinsheimer. Twenty-two patients were available for follow-up study, with an average time of 23.8 months. Functional results were rated using a new traumatic hip score rating system. The overall union rate was 77.3% (17/22), with functional results rated as excellent or good in 68% (15/22). All five technical failures were associated with extensive bony comminution. Four of these failures were not bone grafted. There were no cases of screw penetration of the femoral head or neck. The indications for the use of this device include transverse, short, and long oblique subtrochanteric fractures, with or without lesser trochanteric avulsion. When excessive comminution exists, this device should not be employed.

Treatment of supracondylar-intracondylar fractures of the femur using the dynamic condylar screw.

Forty-nine distal femur fractures were treated with the Dynamic Condylar Screw (DCS) between 1982 and 1985. The results in 35 fractures were reviewed in 32 patients with an average follow-up of 27.5 months. Functional results were graded using a stringent traumatic knee rating scale and Schatzker's criteria. The overall results were seen to be excellent to good in 71%, and excellent to good to fair in 83% of the cases. Using Müller's fracture classification, the less comminuted fractures were found to have better results than those with more comminution. Ease of insertion, the ability to obtain good fixation even in osteoporotic bone, and the capability of revising non-unions with a simple plate exchange are distinct advantages of the DCS over the 95 degree condylar blade plate. Medially placed iliac crest bone grafting should be performed if there is proximal extension of the fracture.

[4 fragment fractures of the proximal femur: is the dynamic hip screw a suitable implant?].

The DHS-implant system is widely used in the operative treatment of pertrochanteric fractures of the femur. The telescoping effect of the implant in the unstable 4-fragment fracture leads to lateralisation of the greater trochanter and shortening of the leg. The quality of reposition influences this secondary dislocation. In our series of 65 patients treated with this device 17 repositions were in a varus position, followed by leg shortening of more than 10 mm in 9 patients. Independent of the quality of reposition in the long-term follow-up only one patient of 25 complained of persistent pain.

[Clinical significance and treatment concept of Lisfranc dislocation and dislocation fracture].

Dislocations and fracture dislocations of the tarsometotarsal joint are uncommon (only 30 cases have been treated in our hospital in a 20-year period). The late results of tarsometotarsal injuries in 20 patients have been reviewed. The average follow-up was 3.8 years (range 8 months to 20 years). Methods of treatment were either open (n = 15) or closed (n = 5) reduction with (n = 18) or without (n = 2) internal fixation and cast immobilization (n = 10). Late results clearly correlate with the quality of reduction. An open procedure is usually necessary to achieve anatomical reduction. Diagnostic and operative problems are discussed.

[Therapy and prognosis in 102 liver injuries].

Case histories of 102 patients with liver trauma were analyzed. 88% sustained blunt and 12% penetrating liver injury. 71% of all patients had a polytrauma. The liver injuries were classified in 4 types. With 25 patients primary resuscitation was unsuccessful, in 12 of these cases even though immediate laparotomy and aortic clamping was performed in the emergency room. In 62 of the remaining 77 patients haemostasis was accomplished with minor surgical procedures (lethality 16%), in 13 cases major surgery had to be performed (lethality 62%). Packing was used in one patient only. Prognosis of liver trauma depends mainly on associated injuries and on severity of liver trauma.

Unicondylar fractures of the distal humerus: an operative approach.

This paper retrospectively reviews a series of 22 unicondylar fractures of the distal end of the humerus that were treated by open reduction and internal fixation over a 10 year period. The fracture patterns were classified according to the system of Müller et al. A strict rating scale was developed that incorporated subjective data, objective elbow motion, and the functional status of the involved elbow. At an average follow-up of 5.9 years (range 2.3 to 12.3 years), 12 elbows were rated as excellent, 6 as good, and 4 as fair. Complications included extensive posttraumatic arthritis in four patients, a nonunion in one, and a transient radial nerve palsy in one.

[Intra-articular unicondylar humerus fractures. Late results following stable osteosynthesis].

The authors report the late results in a series of 45 unicondylar intra-articular fractures of the distal humerus treated by internal fixation. The fractures were classified according the AO system. The average follow-up time was 4.25 years. A rating scale was used that was based on patient interviews. Based on the late results, internal fixation is concluded to be the treatment of choice for all unicondylar intra-articular fractures of the humerus.

[Femoral neck fractures: conservative or operative therapy in view of therapeutic results and rehabilitation].

The analysis of late results of 75 consecutive cases of abduction fractures of the femoral neck gave the following results: One third (24/75) of the patients must be operated because of a secondary dislocation. This secondary dislocation cannot be predicted. All the patients successfully treated conservatively can walk like before the accident. This confirms our therapeutic concept for abduction fractures of the femoral neck: Primary conservative treatment with immediate mobilization and weight bearing. Secondary operation for the cases with varus dislocation (one third).
