Decreased expression of HLA-DR antigens on peripheral blood B lymphocytes during glucocorticoid treatment.

In 18 patients the expression of HLA-A, B, C and -DR antigens on peripheral blood T and B lymphocytes (PBTL and PBBL) was assessed using the lymphocytotoxic microtechnique before and during glucocorticoid administration. During steroid treatment we found a significant reduction in the reactivity of PBBL to allogeneic specific anti HLA-DR antisera as well as a xenogeneic multispecific anti HLA-DR antiserum, corresponding to approx. 1-2 dilution steps. In contrast, the reactivity of both PBBL and PBTL to allogeneic anti HLA-A, B, C antisera and anti beta 2-microglobulin was unaffected during the treatment. Membrane marker studies of the isolated PBBL and PBTL showed no differences in the relative distribution of mononuclear cell subpopulations before and during treatment, which seems to exclude extravascular redistribution of particular HLA-DR positive lymphocyte or monocyte subsets during steroid administration as an explanation for the finding. Incubation of isolated normal PBBL with dexamethasone under various conditions did not affect the reactivity of these cells to anti HLA-DR antisera. Furthermore, preliminary experiments gave no indication of decreased rate of HLA-DR antigen synthesis in the presence of dexamethasone in vitro. Thus, we conclude that glucocorticoid administration in vivo decreases the expression of HLA-DR antigens on PBBL, possibly in an indirect way, the mechanism of which is still unknown.

Children of female renal transplant recipients.

Results of follow-up of 5 children, aged 4.5-9 years, who were born to kidney transplanted mothers are presented. Various immunological parameters were mainly within normal limits with the possible exception of 1 child who exhibited a granulocytopenia which normalized at about 5 years of age. Three children whose mothers were carriers of HBsAg during and after pregnancy have become antigen carriers and show slightly elevated levels of liver transferases. One child has a cardiac anomaly and signs of slight neurological dysfunction. In other respects the children are well and show normal psychomotor and somatic development. The rather sparse information from the literature regarding this group of children indicates a surprisingly low frequency of severe physical complications.

Amount of tissue evacuated by vacuum aspiration in therapeutic abortions.

Following therapeutic abortion in 280 patients prior to the 12th week of gestation, and amount of tissue aspirated was investigated. Measurement of the amount of tissue removed by vacuum suction after dilatation of the cervical canal showed a definite relationship between tissue quantity and gestational age. However, there was a wide range of tissue quantity in the various gestational age groups. The amount of blood lost during the procedure also depended upon gestational age. In 71% of the patients less than 100 ml was lost. There was no difference in the amount of blood lost between primigravidae and multigravidae when regard is paid to gestational age. No case of ectopic pregnancy was found. Re-evacuation was performed in 9 cases. The amount of tissue than recovered was small, in 7 cases 10 ml or less. Placental tissue was removed at re-evaluation in only one of these patients, decidua in all the others. It is concluded that measurement of the amount of tissue removed does not permit assessment of whether the uterine cavity has been adequately evacuated.

Some effects of bone meal supplemented diet on bone and teeth of growing rats.

Bone meal (containing 500 ppm F) was added to the diet of growing rats in an amount corresponding to that recommended ingested by children. Addition of bone meal did not influence body weight increment, or growth and ash content of bone and teeth. Fluoride in bone and teeth increased as compared to control animals, but the increase was 2-4 times less than if fluoride had been given as NaF. Analyses of feces disclosed significantly increased values of Ca and F in animals fed the bone meal supplemented diet, indicating low absorption of these minerals from bone meal.

Intracranial Saccular Aneurysms. Results of treatment in 851 patients.

The series studied comprises all 851 patients with symptom-producing intracranial saccular aneurysms admitted to the departments of neurosurgery in Denmark in the five-year period of 1970-1974. The series was divided into seven clinical stages according to Hunt's classification. More than half of the patients were grouped in stages IV and V (Hunt grade 2-3). Of the patients, 76% were found primarily suitable for operation. The mortality within the individual stages was to a great extent independent of the time of operation. A total of 94% of the aneurysms were localized within the region of the carotid artery, with a roughly equal distribution among the internal carotid, anterior communicating, and middle cerebral arteries. Multiple aneurysms were found in 19% of the patients subjected to panangiography. There was a distinct correlation between the severity of the spasms and the clinical condition. The course of operation was complicated in 49% of the patients, the complications being equally distributed within the various clinical stages. The mortality for the patients who underwent operation (total 567) was 32%, and 80% of the deaths were due to direct or indirect consequences of aneurysmal bleeding. At the follow-up performed two to seven years after operation, 52% of the survivors were fully capacitated, 20% were partly capacitated, and 28% were incapacitated. Based on a retrospective analysis, we have started a comprehensive prospective study with registration of available parameters in patients with saccular aneurysms admitted to all departments of neurosurgery in Denmark.

Some effects of bone meal and NaF tablets on the composition of saliva.

Whole saliva was collected from dental students while: 1) chewing paraffin, 2) sucking bone meal tablets, and 3) sucking NaF tablets. Initial pH, content of ionized and total calcium and content of fluoride in saliva were measured. As compared to paraffin stimulated saliva, the sucking of bone meal tablets caused a decrease in salivary pH, ionized calcium and fluoride. Sucking NaF tablets caused a fall in initial pH and an increase in total calcium, while the amount of ionized calcium remained unchanged. The content of fluoride was high in saliva during sucking of NaF tablets.

Ectopic pituitary adenomas.

We report here four cases representing different forms of ectopic pituitary adenomas localized: (1) in the nasal cavity, (2) in the sphenodial sinus, (3) on the sphenoidal wing and (4) on the petrous temporal bone. Ectopic pituitary adenomas may probably occur in extrasellar pituitary tissue deposited along the route of fetal development or as a result of intracranial dissemination of adenomatous cells during operation. It is assumed that in some cases ectopic pituitary tissue may retain endocrine function leading to a sustained pituitary function even after a radical hypophysectomy.

Hereditary mirror movements--a case report.

A five-year-old girl with abnormal mirror movements is described. The phenomenon is observed in the distal parts of the extremities and is most marked in the hands. An extensive neuropediatric examination including computerized tomography of the brain has not revealed any further abnormalities, nor has it been possible to identify the pathogenetic mechanism. The clinical picture corresponds to that previously reported in cases of hereditary mirror movements--a condition with unknown pathogenesis affecting individuals that in other respects are neurologically normal. The disorder is considered to be dominantly inherited but in this case available data suggest a recessive mode of inheritance. There is no doubt that the girl of this report is getting increasingly embarrassed by her condition. Obviously mirror movements of this degree constitute a slight to moderate handicap.

Pituitary adenoma and visual function. The prognostic value of clinical, ophthalmological and neuroradiologic findings in 51 patients subjected to operation.

The series studied included 51 patients subjected to operation for pituitary adenoma. The visual status was analysed both pre- and postoperatively. The pre- and postoperative parameters were compared with clinical information, ophthalmological and neuroradiological findings with a view to an evaluation of their prognostic value. In 36 patients with chromophobe adenoma, visual field defects were present in 92% before the operation; visual function improved in 62% and returned to normal in 24%. Among 15 patients with eosinophil or mixed adenoma, only two (13%) had visual field defects pre-operatively. In those patients where operation was followed by a distinct visual improvement, the pre-operative ophthalmological findings were as follows: visual acuity greater than 6/24, normal optic discs, visual field defects up to two quadrants, and pneumoencephalographic findings suggestive of only a small suprasellar tumour.
