Serum oestrone, oestradiol and oestriol levels in ovariectomized women receiving a high dose of oestradiol valerate.

Fourteen recently ovariectomized women received 4 mg of oestradiol valerate therapy daily for 6 mth. Serum oestrogens were measured by radioimmunoassay at monthly intervals. The results of therapy produced a 25-fold increase in serum oestrone (E1) concentration. Circulating oestradiol (E2) increased 5-to 6-fold. No accumulation of oestrogens was observed as a consequence of treatment. Serum concentration of oestriol (E3) showed no elevation. One month after the completion of therapy the E1 and E2 concentrations had returned to pre-treatment levels. Although therapy of high-dose oestradiol valerate was well-tolerated, it is not recommended because of the non-physiological high serum concentrations of oestrone and high serum level of oestradiol.

Peripheral venous concentrations of oestrogens in postmenopausal women with ovarian cancer.

Serum concentrations of oestrone, oestradiol and oestriol were measured in 23 postmenopausal women, 12 with ovarian cancer and 11 with genital prolapse. Oestrone (387.6 pmol/l) and oestradiol (72.7 pmol/l) levels were higher in the cancer group than those in the control group (159.8 and 27.5 pmol/l respectively), while oestriol levels did not differ (434.5-270.8 pmol/l). The results indicate abnormal ovarian function in postmenopausal patients with ovarian cancer.

Steroid receptors in normal and neoplastic female reproductive tissues.

The presence of cytosol estrogen (ER) and progestin (PR) receptors in specimens of normal uterine cervix, endometrium, myometrium, Fallopian tubes and corpora lutei or in samples of neoplastic female reproductive tissues was investigated. The material consisted of PR and ER measurements of tissue samples obtained from fertile and postmenopausal women, receptor assays were performed by a dextran-coated charcoal technique. The radio PR/ER was highest in specimens from the Fallopian tubes (44) and endometrium (4-10),2-3 in specimens of uterine ecto- or endocervical epithelium and about 4 in the myometrium. No ER or PR were found in the four corpora lutei examined. PR or PR/ER ratio in specimens of myoma tissues did not differ from that found in the myometrium specimens. The highest ER and PR values in the endometrium were measured in specimens taken during the late proliferative or intermediate phase of the menstrual cycle. The PR values in specimens from postmenopausal myometrium were lower in comparison with the samples taken from the myometrium of fertile women in contrast to ER values. In the specimens of carcinoma of the vulva and the uterine cervix ER levels were very low, with no measureable values of PR. In the endometrial carcinoma samples the PR/ER ratio was lower than in the normal endometrium. 4 out of 7 specimens taken from ovarian adenocarcinoma had a measureable amount of ER and in 2 out of 7 cases PR.

Reproductive performance of women with uterine anomalies. An evaluation of 182 cases.

A total of 182 women with uterine anomalies were observed over a period of 18 years. In all, 126 women had 265 pregnancies, fetal survival rate was 66%, perinatal mortality 8% and premature labors occurred in 23%. When the uterine anomalies were grouped according to degree of failure of normal and uterine development, the complete septate uteri had the best fetal survival rate (86%), complete bicornuate uteri (50%) and unicornuate (40%) poorest. Cervical cerclage was applied most in the partial bicornuate uteri (17%), where fetal survival in operated cases was: before operation, 53% and after, 100%. The frequency of breech presentation was 40-50% in all groups except complete septate uteri (25%) and complete bicornuate (0%). The highest cesarean section rate (82%) was in deliveries of patients with uterus didelphys. The frequency of cesarean section among total deliveries was 53%. Metroplasty was carried out on 19 patients. The obstetric outcome improved from 10% successful pregnancies before surgery to 88% postoperatively.

Results of treatment of primary ovarian carcinoma.

A retrospective evaluation of 146 patients with primary ovarian carcinoma treated in the University Central Hospital of Tampere over the period 1962-1971 is presented. Diagnosis and clinical stage of the carcinoma were always confirmed operatively. 54.1% of the cases were diagnosed at clinical stage I-II. The mode of treatment was surgery without any adjuvant therapy in only 15 cases (10.3%), surgery and chemotherapy in 55 cases (37.7%), surgery combined with chemotherapy and radiotherapy in 29 cases (19.9%) and surgery combined with radiotherapy in 13 cases (8.9%). In 34 cases of advanced ovarian cancer the operation remained explorative. Chemotherapy comprised high-dose cyclosphosphamide therapy and radiotherapy was given as external irradiation. The 5-year survival rate in the entire material was 47.9%; in stage I 82.3%, stage II 58.8%, stage III 23.1% and stage IV 0%. The prognosis was best for patients with mucinous cystadenocarcinoma and poorest for patients with anaplastic and endometrioid adenocarcinoma.

Effect of oestriol succinate on serum lipids.

The series comprises 70 women with climacteric symptoms; 14 of them were oophorectomized. The first group of patients received 4 mg of oestriol succinate per day or a placebo in a double-blind experiment. In the second part of the study oophorectomized women were given 8 mg of oestriol succinate per day or 16 mg/day divided into two doses. Serum total cholesterol, HDL-cholesterol and serum triglycerides were estimated before drug treatment and after drug administration for three and six months. Administration of 4-16 mg/day of oestriol succinate was without effect on serum total cholesterol. A significant increase of 25-30% in HDL-cholesterol was observed following administration of oestriol succinate (8 mg divided into two doses) with a rising tendency in total triglyceride value.

LH(hCG) receptor in benign and malignant tumors of human ovary.

LH(hCG) receptors were identified with [125I]hCG in 38 benign and 26 malignant ovarian tumors of the human ovary. Eighteen per cent of all the benign and 27% of all the malignant tumors were LH(hCG) receptor-positive. Four of 12 benign serous tumors and 3 of 17 benign mucinous tumors displayed definitive binding of [125I]hCG. Two Brenner tumors failed to bind [125I]hCG. Six of 21 malignant epithelial tumors displayed definitive binding of [125I]hCG. Only one out of 4 malignant granulosa cell tumors bound [125I]hCG, while the other sex cord stromal tumors as well as one dysgerminoma failed to bind [125I]hCG. LH(hCG) receptor content in the benign and malignant receptor-positive tumors was low compared with the normal ovarian tissue. The presence of LH(hCG) receptors in certain benign and malignant ovarian tumors may be a sign of the gonadotropic control of these tumors. The possible applications of these findings for the diagnosis and treatment of human ovarian tumors is discussed.

Radical surgery in the treatment of vulvar squamous cell carcinoma.

A total of 58 cases of vulvar squamous cell carcinoma treated between 1962 and 1980 were analyzed. Of these, 32 were radically, 21 non-radically operated and 5 were treated with radiation only. The study showed that radical surgery is to be recommended in vulvar carcinoma. In addition to evacuation of the inguinal region the method should also include removal of iliac and obturator lymph nodes.

Results of treatment for uterine cervical carcinoma.

A total of 229 patients with invasive cervical carcinoma were treated over a period of ten years 1962--1971. 152 (66%) of the patients were operated on. Radical surgery was performed in 107 cases. Radiotherapy was combined with the operation in 84 cases (55.2%) and 77 patients (33.6%) were treated with radiotherapy without operation. The overall 5-year survival rate in the material was 66.4%; in stage I 91.8%, stage II 50.0%, stage III 33.3% and stage IV 12.5%. Early complications were found in 11.3% and late complications in 14.8% of the patients.

Results of treatment of endometrial cancer in Tampere University Central Hospital 1962--1971.

The aim of the present study was to ascertain the outcome of treatment of 212 patients with carcinoma of the body of the uterus in the University Central Hospital in Tampere over the period 1962--1971. Of the total number of patients involved, 182 were treated operatively. In 97 cases, additional radiation treatment was administered, 25 of these receiving preoperative radium therapy only. Progestin treatment was prescribed for 85 of the operated patients. Cytostatic (cyclophosphamide) treatment alone was applied in 8 cases. Results of treatment were assessed on the basis of five-year survival. The best results were obtained with radical surgery undertaken in the early stages of the disease, the five-year survival rate being 88.2%. The five-year survival rate in the entire material was 76.9%. In the present study combination of radiation or progestin treatment with surgery did not improve the results of treatment.

Induction of labour with sulprostone after foetal death and in hydatidiform mole.

Induction of uterine contractions was carried out with an intravenous infusion of sulprostone, a 16-phenoxy derivate of methylsulphonylamid prostaglandin E2 in 21 patients after intrauterine foetal death and in seven patients having hydatidiform mole. The mean total dose of sulprotone was estimated as 1100-1300 microgram in different groups. The mean induction-delivery time was 7-13 hours. Expellation of the foetus occurred in 20 out of 21 cases during 24 hours after commencement of sulprostone infusion. In all patients having molar pregnancy uterine contractions induced with sulprostone opened the uterine cervix for evacuation. The drug was clinically well tolerated without any serious side-effects.

Catecholamines and dopamine-beta-hydroxylase activity during therapeutic abortion induced by sulprostone.

To evaluate the changes in circulating norepinephrine (NE), epinephrine (E) and dopamine-beta-hydroxylase (DBH) caused by an intravenous infusion of a derivate of PGE2, sulprostone, in connection with legal termination of pregnancy, serial plasma samples were analyzed for six gravidae. Plasma catecholamines were measured by a sensitive radioenzymatic method (9,10) and DBH activities by a photometric assay (11). Intravenous infusion of sulprostone, in abortifacient doses as an intravenous infusion of 3-4 microgram per minute for six to eight hours produced a decrease in circulating norepinephrine. No significant alteration was found in plasma epinephrine or dopamine-beta-hydroxylase activity. The finding suggests an inhibitory effect of sulprostone on the release of norepinephrine from the adrenergic terminals without inhibition of the adrenal medulla.

Effects of 4 mg estradiol valerianate on serum lipids in newly oophorectomized women.

The material consisted of 32 patients, 12 of whom were selected for control. All were cases approaching the menopause and had been subject to oophorectomia bilateralis and hysterectomia for myomas. All 20 investigation patients received estradiol valerianate orally 4 mg/day of 6 mth. During the course the serum estradiol level increased 5--6-fold from the postmenopausal levels. No significant changes occurred in the serum triglycerides and total cholesterol, whereas in the control group the triglycerides decreased. The proportional concentration of beta-lipoproteins decreased. There was also a slight decrease in the pre-beta-lipoproteins. The proportional concentration of beta-lipoproteins increased, as also serum high density lipoprotein (HDL) cholesterol. In the liver enzymes no changes occurred. Blood pressure increased in one patient. There were no other complications.

Effect of an oral contraceptive on the lysine-8-vasopressin test in the diagnostic evaluation of pituitary function.

A comparative study was performed on the plasma 11-hydroxycorticosteroids (11-OHCS) responses to lysine-8-vasopressin (LVP) before and during ingestion of an oral contraceptive wieh oestrogenic activity. A total of 19 healthy women with a normal menstrual cycle and normal plasma 11-OHCS content in blood samples at 8 a.m. and 4 p.m. were included in the study. The results show that the mean response of plasma 11-OHCS to the administration of LVP was equal in magnitude before and during ingestion of the contraceptive, although the baseline value of 11-OHCS doubled as a result of the treatment. On the other hand, during contraceptive treatment the increase of plasma 11-OHCS concentration after the administration of LVP was somewhat slower, reaching its peak later than before treatment. The noticeable variation in individual responses of plasma 11-OHCS to LVP increased during the treatment. Three subjects showed weak responses to LVP, although the adrenocortical responses to exogenous ACTH were quite normal. The inidvidual responses to LVP before and during ingestion of an oral contraceptive with oestrogenic activity are discussed.

Urography and renography in the follow-up of patients with toxemia of late pregnancy.

We have studied 129 patients with toxemia of pregnancy two months post partum by urography and renography. The same investigations were performed on 17 healthy parturients post partum. 49 patients (38.8%) had an abnormality either in urography or renography. 20 patients (15.5%) had an abnormal urogram and 42 (32.6%) an abnormal renogram. The findings correlated so that 7 patients had an abnormal urogram with a normal renogram and 13 patients had an abnormality in both investigations. 29 patients had only an abnormal renogram. In the control series 3 patients had an abnormal urogram and the same patients also had an abnormal renogram. We divided the studied patients into groups according to the clinical severity of toxemia. Urographic and renographic findings were divided evenly in the series so that abnormalities were no more common in the severe cases. Renography cannot be considered merely as a screening method when compared with urography in the follow-up of toxemic patients, but these investigations are complementary.

Activities of aspartate and alanine aminotransferases and alkaline phosphatase in sera of healthy subjects.

The activities of serum aspartate aminotransferase (EC 2.6.1.1, L-aspartate: 2-oxoglutartate aminotransferase, ASAT) and alanine aminotransferase (EC 2.6.1.2, L-alanine: 2-oxoglutarate aminotransferase, ALAT) were determined in the sera of 1484 apparently healthy subjects using kinetic methods according to the Scandinavian recommendation (33). In the adult sera the mean activity of ASAT was 21.4
