Appendix mass in the very young child.

During the first 3 yr of life 54 (34%) of the 158 patients presenting with appendicitis had an appendix mass at the time the diagnosis was made. In 19 the mass was discovered on clinical examination and in 35 on examination under anesthesia. Complication rate was markedly reduced and hospitals stay shortened if the appendix mass was managed non operatively.

Appendectomy↗

Delayed primary anastomosis following spontaneous growth of esophageal segments in esophageal atresia.

Between 1977 and 1979, five consecutive cases of esophageal atresia had delayed primary esophageal anastomosis at ages ranging from 5 1/2 to 18 wk. Serial growth studies of the two esophageal segments were performed under fluoroscopic screening. It was observed that spontaneous growth of esophageal segments occurs in the absence of any stretching or bougienage of the esophageal ends.

IgA-antibody responses in the neonate following gastrointestinal surgery.

IgA-antibody responses relative to other immunoglobulin classes were serially studied in 14 neonates undergoing gastrointestinal surgery and in a comparative group of 10 neonates undergoing surgery for neural tube defects. While IgM levels were raised postoperatively in gastrointestinal surgery patients as well in patients with neural tube defects the level of serum IgA following surgery was significantly higher in gastrointestinal patients (p less than 0.001) compared to neonates with neural tube defects. A close correlation between time of operation and an increase in IgA levels was observed. Changes in IgA antibody levels were independent of alteration in IgM and IgG levels.

Perforated appendicitis in children: use of metronidazole for the reduction of septic complications.

Fifty-four consecutive children with performed appendicitis were treated with intravenous and rectal metronidazole combined with another antimicrobial agent. Positive cultures were obtained from peritoneal swabs from all children except from four patients who produced no growth on culture. Mixed growth of aerobic and anaerobic organisms were isolated from 44 patients. Four patients had pure growth of aerobes and two had pure growth of anaerobes. Results were compared with 49 cases of performed appendicitis treated with a combination of Gentamicin and Cephradine and who did not receive metronidazole. The overall incidence of complications was reduced from 44.9% to 14.8%. A significant reduction in the incidence of wound infection (p less 0.01) and pelvic abscess (p less than 0.025) was observed in the metronidazole treated group. The period of hospitalization averaged 13.8 days in patients treated with metronidazole as compare to 18.2 days in those treated without metronidazole.

Immune status of the neonate maintained on total parenteral nutrition.

Immunological studies were carried out on 7 neonates maintained on total parenteral nutrition for periods ranging from 14 to 31 days after surgery for correction of gastrointestinal anomalies. IgM and IgA levels were high in all patients but IgG levels were low, compared with normal values. Total leucocyte counts, absolute lymphocyte counts, and T- and B-lymphocytes in all patients were similar to those of healthy controls. Neonates maintained on adequate total parenteral nutrition showed no evidence of impairment of immune function.

B-Lymphocytes↗

Lymphocyte transformation after surgery in the neonate.

Nine neonates born with congenital malformations were investigated for lymphocyte response to phytohemagglutinin (PHA). No significant change was found in the transformation response of lymphocytes to PHA after surgery, which is in contrast to studies reported in adults and older children.

Anesthesia, General↗

Relationship between functional and histologic appearances of developing ganglion cells in the guinea pig rectum.

Time dated fetal guinea-pigs between 47 and 67 days gestation were studied for segmental propulsive activity in the rectosigmoid under fluoroscopy. Large bowel from the sacrificed fetus was studied histologically to determine distribution and morphology of ganglion cells. Contractions were first observed on day 54 and these became more frequent and vigorous after day 63. Between 47-53 days gestation circular muscle is thin and neurons consists of cluster of neuroblastic cells. Near the end of gestation the neurons arrange themselves into discrete groups and have an appearance like mature ganglion cells.

Lymphocyte response after surgery in the neonate.

Fourteen neonates born with congenital malformations were investigated for lymphocyte function before and after surgery. Total leucocyte and absolute lymphocyte counts were unaltered after surgery. The mean percentage of T-lymphocytes observed either pre- or postoperatively was considerably lower than that reported in older children and adults. While there was an increase in the percentage of B-lymphocytes after operation in the infants, the absolute number of B-cells remained unchanges. The preoperative transformation response of lymphocytes to PHA (mean 12.9 +/- 5.4 X 10(3) counts/min) was little different from the postoperative values (mean 12.4 +/- 4.4 X 10(3) counts/min). These results suggest that the neonate is immunologically different from older children and adults in its response to anaesthesia and surgery.

Immunological studies in neonates with spina bifida.

Fifteen neonates with spina bifida were investigated prior to surgery for T and B lymphocytes subpopulations and mitogen responsiveness to PHA. Results were compared to similar studies in 30 normal healthy neonates. The mean percentage of T-lymphocyes in spina bifida patients was 44.6% compared to 48.9% in normal neonates. A normal dose response to PHA was obtained in all patients. These results suggest no impairment of lymphocyte function in neonates born with spina bifida.

Appendicitis in infancy.

One percent (40 infants) of all the children treated for acute appendicitis at Our Lady's Hospital for Sick Children presented during the first 2 yr of life. Nineteen of the 40 children had an appendix mass at the time of the diagnosis. Conservative management of the appendix mass followed by delayed elective appendectomy is a safe and effective method of treatment.

Acute Disease↗

Duodenal ulcer in childhood: a continuing disease in adult life.

Thirty-one of the 37 children with duodenal ulcers were available for follow-up. Of the 28 patients treated medically, 22 had persistence of symptoms over periods up to 18 yr after the time of the initial diagnosis. Five patients required surgery elsewhere for complications, thirteen showed ulcer crater or duodenal deformity on a follow-up barium meal studies. Earlier surgery is recommended in chronic ulcer disease.

Hirschsprung's disease: an appraisal of histochemically demonstrated acetylcholinesterase activity in suction rectal biopsy specimens as an aid to diagnosis.

Suction rectal biopsy specimens, from a series of 168 infants and children with constipation and other gastrointestinal problems, were stained with a sensitive acetylcholinesterase method, and the results were compared with routine histology, radiology, anorectal manometry, and the final diagnosis. In all cases of Hirschsprung's disease, there was an increase in numbers and sizes of cholinergic nerves in the lamina propria and muscularis mucosae. No false-positive or false-negative results were found. The test was found to be more reliable and consistent than other methods available.

The results of treatment of anorectal anomalies: a thirteen to twenty year follow-up.

The long-term results of low anomalies are excellent, as expected, whether primarily treated by cutback or transplantation. Those of the high anomalies are perhaps better than expected but only achieved after an unacceptably long period of incontinence. Better techniques are still needed for these supralevator deformities. Permanent colostomy also had its problems.
