[Ambulatory ECG monitoring in effort angina (author's transl)].

In 20 male subjects with effort angina, an ambulatory ECG monitoring (AEM) was registered and an exercise test was carried out immediately afterwards. Usual indications for termination of test have been modified to detect the temporal relationship between the appearance of ischemic ST changes and appearance of pain. During AEM 15 patients (pts) presented ischemic episodes with a total of 80 episodes, of which 46 were asymptomatic and 34 accompanied by pain. Among the ischemic episodes registered by AEM, the mean duration of asymptomatic episodes was less that of symptomatic ones. The same can be said for the mean entity of maximum ST depression. On the basis of the temporal relationship between the appearance of ischemic ECG features and pain during exercise test, we classified 4 groups of patients: 1) pts with an almost contemporary appearance of ST alterations and pain (5 pts); 2) pts where pain appeared with a delay variable between 10 and 30 sec (6 pts); 3) pts where pain appeared with a delay of more than 30 sec (4 pts); 4) pts with absence of pain (5 pts). In these pts the exercise test was stopped either because ST depression had reached 3 mm or because ST alterations persisted for 3' without pain. Comparing the responses of the exercise test with the data of AEM, a statistical relationship was found between patients from the different groups and the prevalence of asymptomatic or symptomatic episodes of ischemia. Asymptomatic ischemic episodes during AEM are more frequent in pts who during exercise test show ischemic ECG features without pain or in pts where pain appears with a noticeable delay in comparison to ECG ischemic alterations.

[Long-term prognosis of survivors from anterior myocardial infarction complicatedy by complete atrio-ventricular block (author's transl)].

Thirteen patients out of 42 affected by acute myocardial infarction complicated by atrio-ventricular block survived the illness and were discharged from hospital. Eight of these patients (60%) died, all in the first six months of follow up: 3 suddenly and 5 in consequence of progressive heart failure. Dead patients were all in III or IV class of the N.Y.H.A. Three had been previously submitted to permanent pacing, one of these died suddenly. Five patients survive at the end of follow up that varies from 2 to 56 months. Of these five survivors 1 patient belong to the I class of the N.Y.H.A., 1 to the II class and 3 to the III class. Among survivors there is only 1 patient with permanent pacing. This is also the only one who had syncopal attacks during a period of pacing failure. Prognosis of patients with myocardial infarction complicated by atrio-ventricular block who survive illness is dependent on functional class and seems to be not considerably modified by permanent pacing.

Effects of verapamil in ventricular premature beats of acute myocardial infarction.

In 8 patients affected with acute myocardial infarction verapamil was highly effective in controlling ventricular extrasystoles. Theoretically the drug ought not to be effective if the extrasystoles were related to fast cardiac fibers characterized by a rapid inward Na+ current. The result suggests that in acute myocardial ischemia ventricular extrasystoles are related to slow inward Ca++ currents.

[The antiarrhythmic effects of verapamil in acute myocardial infarction. Considerations on the possible action mechanism (author's transl)].

In 12 patients affected by acute myocardial infarction complicated only by ventricular extrasystoles, verapamil was highly effective in the control of the arrhythmias. This result is in agreement with the experimental finding that ischemia inactivates partially or totally early Na+ inward currents, so that fast fibers become slow fibers. The efficacy of verapamil stresses the importance of these fibers which have acquired a slow response in the genesis of arrhythmias due to acute coronary attacks and opens interesting therapeutical prospects.

Acute Disease↗

Prognostic value of intraventricular conduction blocks in acute myocardial infarction.

An intraventricular conduction defect complicated 61 out of 765 cases of acute myocardial infarctions. The defect, with the exception of two cases, always developed in infarctions affecting the anterior wall of the heart. Compared with the other acute anterior myocardial infarctions in which the mortality in the Coronary Care Unit was 15%, infarctions complicated by intraventricular conduction defects have on the whole, presented a worse clinical course and a higher mortality rate (54%). It must be stressed that clinical course and mortality rate have been very different in the various conduction defects. The most innocuous conduction defects were the anterior fascicular block, and, with some reserve, the right bundle branch block too, defects in which the clinical course and the mortality rate were not significantly different from those of the other anterior myocardial infarctions not complicated by intraventricular conduction troubles. On the contrary clinical course was more serious and mortality rate higher (52%) in the cases in which the two defects were associated. Myocardial infarctions complicated by left bundle branch block, right bundle branch block with posterior fascicular block, alternating bundle branch block presented an extremely serious clinical course and a very high mortality rate (over 85%). These differences in the clinical course and in the mortality rate of the various intraventricular conduction defects are explained by the fact that they are associated with myocardial damage of different degree, which is clarified by knowledge of the blood supply of the intraventricular conduction system.
