Ureteral metastases--computed tomographic findings.

CT findings in three cases of ureteral metastases are reported. Metastases to the ureter is an uncommon cause of the upper urinary tract obstruction. The most common primary sites include breast, stomach and neoplasms from pelvic organs. Radiographic studies revealed an ureteral stenosis in all the presenting cases. CT scans demonstrated a thickened regular ureteral wall at the level of the narrowing and no other abnormalities of the retroperitoneum. Such CT features suggest the possibility of ureteral metastases in a patient with the appropriate clinical setting. Histological studies, whenever possible, must confirm these radiographic findings. Early recognition of ureteral metastases and appropriate treatment for relief of urinary tract obstruction may prolong survival time.

[Epididymitis. Apropos of 82 cases].

The authors report 82 cases of epididymitis, studied from 1976 to 1985: 66 acute epididymitis and 16 chronicle epididymitis. It is the affection of young patients: middle age 39 years. The frequency of epididymitis seems to increase the last years. The coliform organisms were the predominant pathogens isolated from the patients more than 35 years old; for patients less than 35 years old, in most cases, the Chlamydia trachomatis was the cause of infection. Among 12 patients with Chlamydia trachomatis infection, 11 were less than 35 years old. The treatment before the results of the sample has to be the Doxycycline; if Chlamydia trachomatis is not the cause of infection, the treatment will be changed. The corticotherapy is always associated with antibiotics, and his action is discussed.

[Urinary stress incontinence in women: contribution of electromyography of the striated urethral sphincter].

Fourteen women within their menopausal period and suffering from stress urinary incontinence were studied. Electromyographic studies show that sphincter weakness is almost constant (9/14), usually associated with a bladder instability and/or a lack in abdominal urethral transmission, both conditions being known as possible causes of urinary stress incontinence. However, neurological causes at the origin of urinary stress incontinence, such as neurogenous sphincter, may be found (3/14). Electromyography, coupled with urodynamic evaluation, therefore presents itself as the most accurate method for a good assessment of correct pathophysiology in urinary stress incontinence and thereby for good therapeutic prescription.

[Are retinoids of value in bladder tumors? Pharmacological study and literature review].

Retinoids are vitamin A derivatives with a modified final radical, lateral chain or initial cycle. Their action on malignant cells is not fully known, but probably consists of the control of cell growth and differentiation. Experiments suggest that retinoids are non-toxic, and that they are far more effective against severe lesions than against dysplasia. Their effect is dose-related and dependent on the type of carcinogen.

[Bladder contraction. Physiopathology, therapeutic effects. I. The bladder muscle. Structure. Electrical properties. Intrinsic regulation].

The author undertakes an overall study of the phenomenon of bladder contraction, attempting to place it in the context of normal micturition. It starts with a study of the structure of the vesical muscle (muscle cells, intercellular links, contractiel structure, intramural nerve structure), followed by study of the electrical properties of the vesical muscle and finally extrinsic regulation of bladder contraction. The only possible way to analyse such a long text is to give its plan.

[The place of urinary cytology in the diagnosis of urothelial tumors].

A cytologic study was done in three groups of patients, either for screening purposes or for the follow up of vesical tumors already treated by transurethral resection or cystectomy. The reliability of urinary cytology is closely related to the degree of tumor anaplasia (81,8% positive cytology in grade III tumors). When a cytologic and cystoscopic follow-up is done, the results of these two methods are in agreement in 60% of the cases. In the follow up of patients already treated for vesical tumors, the association of cystoscopy and cytology increases the discovery of recurrences by 40%. The persistence of isolated positive cytologies is indicative of highly malignant lesions (grade III). In cystectomized patients, a close correlation exists between the histologic grade and the positivity of the cytologic survey. A cytologic follow up after cystectomy helps to discover a greater number of urethral recurrences.

[Cryptorchism and sterility].

The authors report their observations on the association of sterility with antecedent cryptorchidism. In 1 190 sterile men, 88 had history of cryptorchidism (bilateral in 55 cases and unilateral in 33 cases). All had testicles in normal position in the scrotum at the time of their clinical examination, whether or not they had had an operation. Of 73 cases operated, 46 had been followed, and amongst these, 12 had had an operation for varicocele. The majority of these patients had had treatment with clomiphene or androgens. In these patients the earlier the operation had been done, the better the chances for testicles of normal volume. Of 20 patients with azoospermia, 16 had had bilateral cryptorchidism. Only 9 of the whole group had a normal sperm analysis, while all the other had oligospermia, teratospermia and altered motility. One third the subjects had elevation of FSH and hypotestosteronemia, and in 13 with hypotestosteronemia, 10 had had bilateral cryptorchidism. Treatment resulted in 10 pregnancies. Since the majority of the patients had been treated late for their cryptorchidism, the authors advise bringing the testicles down before the age of 4. Hormonal treatment and varicocele ligature can improve fertility in sterile men having cryptorchidism in their past.

[Varicocele and hypofertility. Results of 220 cases. Postoperative results].

Having studied 220 varicocelectomies which were performed by a method already described, we have been able to correlate the degree of varicocele present with testicular atrophy, with a rise in the plasma LH and with the severity of the histological lesions found. In 52.3% of cases there was a higher concentration of sperms present. Above all if the motility was less than 30% beforehand it improved most. 120 patients were reviewed. 43 (35%) became fertile. These was no difference in the improvement in the spermograms between those who were operated on who stayed infertile and those that become fertile. Thermography is not a reliable test: there were 35 poor correlations out of 107 patients. These was no correlation between the size of the varicocele, the gradient of the temperature and the biological abnormalities as against the concentration, the mobility and teratospermia.

[Parenteral hyperalimentation and cystoprostatectomy for carcinoma of the bladder (author's transl)].

The authors report a series of 16 total cystoprostatectomies with only one postoperative death. Eleven of these operations, performed with the intention of achieving a complete cure, were carried out after flash radiotherapy of 1.000 rads on the day before surgery and were completed by uretero-sigmoidostomy. The five palliative operations were completed by direct bilateral cutaneous ureterostomy, the patients having received preoperative radiotherapy of 5 to 6,000 rads. The authors attribute the low postoperative mortality to the value of the energy and protein provided by parenteral hyperalimentation which made it possible to administer to the patient from the day after surgery onwards 1,400 to 2,600 calories per day (60% as carbohydrate and 40% as lipids) and 1 gram/kilo of protein per day. Infusion catheters were inserted under strictly aseptic conditions on the day prior to surgery into the deep main veins by percutaneous puncture of a subclavian vein or cutdown of an external jugular vein, and were kept in place until such time as oral alimentation could ensure an equivalent intake. For the 11 patients undergoing curative surgery, weight loss at the time of discharge was no more than 1 to 2 kilos, a figure to be contrasted with the 7 to 8 kilos before the use of parenteral hyperalimentation. The only death occurred amongst the 5 patients undergoing palliative surgery. In the latter group, the authors were stuck by the uncomplicated postoperative course, and the control of complications such as intestinal obstruction or bacteraemia which was obviously better in veiw of the maintenance of the calorie reserves of the body. All parenteral hyperalimentation was administered in the form of commercial solutions and emulsions.

Studies of female urethral pressure profile. Part I. The normal urethral pressure profile.

The urethral pressure profile is influenced by the degree of patient relaxation, which explains marked variations in measured pressure values among normal subjects, as well as the frequent lack of reproducibility in a given subject. The quality of relaxation can be estimated by observing the difference between maximum resting and maximum holding pressures. The most reproducible parameters are 1) the total urethral length, 2) the length to the point of maximum pressure and 3) the ratio of area to the point of maximum pressure over total area. Changes in intravesical volume do not produce significant differences in peak pressure, total area or area to maximum pressure. When subjects were asked to strain the pressure in the proximal urethra increased by 60% of the intravesical pressure observed, even in the absence of a vesical neck opening.

Studies of female urethral pressure profile. Part II. Urethral pressure profile in female incontinence.

Most pressure values, except U1 and areas under the curve, were significantly lower in the 456 curves obtained in 38 incontinent women than in normal subjects. Standard deviations of all parameters are important because of variations in the degree of patient relaxation, explaining a large overlapping zone in which curves from the incontinent and the normal groups are superimposed. There are 3 factors that cause a reduction in urethral pressure profile: 1) aging and menopause (which are attributed to reduction in urethral compliance), 2) multiparity and 3) previous significant urogynecological operations. Besides urethral compliance urethral pressure profile studies allow measurement of 5 other components of the urethral resistance: 1) the degree of patency of the vesical neck, 2) the maximum voluntary sphincter contraction, 3) the functional urethral length, 4) the facilitatory urethral relaxation reflex and 5) the degree of urethral displacement to some extent. Separate measurements of each urethral factor should allow a more accurate analysis of urethral resistance and, consequently, enhance the quality of therapeutic indications in the management of female incontinence.

[A summary of the results of microsurgical exploration and treatment. 40 cases of male sterility (author's transl)].

The authors report their statistical results of 40 subjects that were explored and where double epididymo-deferens anastomoses were achieved. They found that there were associated conditions which worsened the prognosis, namely: stenosis of the genital tracts and broncho-pulmonary infections. On the other hand stenosis of the genital tracts associated with varicocoele gave a better prognosis. Permeability was achieved in 40 p. 100 of cases. Six pregnancies occured from 33 patients who were followed up with post-operative spermograms. The following treatments are useful if added to the surgical treatment: gonadotrophins, corticoids and antibiotics. They point out that surgical cure is much more likely when varicocoele is associated.
