A longitudinal study on plasma somatomedin activity in full-term, preterm and small-for-gestational age newborns.

Plasma somatomedin (SM) activity was measured longitudinally in the first days of life by Van den Brande bioassay in 10 term, 8 premature and 5 small-for-date newborns. Other term, premature and small-for-date newborns were assayed between the 4th and 8th days by a cross-sectional-type study. A very significant increase in SM activity was observed in term newborns on the 3rd day lasting for about a week, when values usually observed in young children were reached. In premature and small-for-date newborns such an increase was not present but rather a slight decrease on the 3rd day was observed. The presence of a circulating inhibitory substance in the plasma of premature and small-for-date newborns with undetectable SM activity was shown.

Characterization of lymphoma-derived cell lines: comparison of cell lines positive and negative for Epstein-Barr virus nuclear antigen. II. Surface markers.

Fifteen of 16 lymphoma-derived cell lines and the Faji and P3HR1 cell lines were characterized with regard to certain surface markers, particularly immunoglobulins, complement receptors, Epstein-Barr virus (EBV) receptors, and Fc receptors. Ten lines positive for EBV nuclear antigen (EB-pos) were stained weakly or not at all by antihuman immunoglobulin fluorescein isothiocyanate conjugates, whereas EBV nuclear antigen negative (EB-neg) cell lines stained brightly. EB-pos lines frequently manifested Fc receptors, particularly for 7S antibody, whereas EB-neg lines did not. Receptors for the C3b component of complement and for EBV, which correlated significantly with each other, were expressed to a much lesser extent by EB-neg lines than by EB-pos lines. These findings are pertinent to an understanding of the infrequent association of this virus with American undifferentiated lymphomas of the Burkitt's and non-Burkitt's types.

Iatrogenic lactic acidosis: association with hypertonic glucose administration in a patient with cancer.

A case of lactic acidosis associated with the administration of hypertonic glucose to a patient with a bulky undifferentiated carcinoma is presented. Characteristic alterations in amino acid concentrations were observed during the period of lactic acidosis. Resolution of the metabolic abnormalities were seen with discontinuation of glucose infusion. Short-term glucose infusion in a 90 minute iv glucose tolerance test resulted in an increase in serum lactate and appropriate changes in serine, ornithine, taurine, alanine, and arginine despite normal hormonal responsiveness.

The pediatric internship as a teaching technique: a comparison of learning experiences in five hospitals.

Five interns joined in a collaborative study to assess their learning experiences during the internship year. A 3 times 5-inch data form was completed for each patient for whom the intern was responsible. Information was gathered on demongraphic characteristics of the patient, the teaching that involved this patient, and what skills were acquired. Nearly 30% of the patients were under 1 year of age; 55% were boys. More than one half of the contacts were in an outpatient or emergency department. Over 80% of the patients had not been seen before; continuity patients made up less than 9% of the contacts. Well-child care was the largest care category (19%), followed by respiratory problems (15%) and injuries (9%). About 40% of the contacts involved a teaching input, mainly from residents, attending physicians, and faculty. Care skills most frequently acquired were physical examination (49%), reading (15%), and interviewing (11%). Cross-tabulations showed that most learning was reported for inpatients, for patients with rare diseases, and when some teaching was involved.

Anorectal infections in patients with malignant diseases.

Fifty-seven episodes of anorectal infection in 44 patients with malignant diseases primarily leukemia or lymphoma, have been retrospectively reviewed. Seventeen patients died in hospital, but only in seven cases was the anorectal infection a major contributing cause of death. The most important prognostic indicator of outcome was number of days of neutropenia during the infectious episode. Cultures obtained at the time of surgical drainage or by needle aspiration of the wound revealed multiple organisms in 26 of 29 instances, and anaerobic organisms were the commonest isolates. Anorectal infection was controlled in 28 (55%) of 51 treatment courses when antibiotics were the only treatment given. However, if the antibiotic regimen included both an aminoglycoside and an antibiotic with anaerobic coverage, control of infection was observed in 15 (88%) of 17 cases. There were 26 surgical procedures performed, with acceptable morbidity. Infection was controlled in 19 (73%) of 26 cases treated with surgery and antibiotics. The results support managing most of these infections initially with medical treatment, using an antibiotic regimen that includes an aminoglycoside and a specific drug against anaerobes. Surgery is recommended if there is obvious fluctuance, a significant amount of necrotic tissue evident, or progression of the infection locally or continued sepsis after an adequate antibiotic trial.

Detection and identification of ketamine and its metabolites in horse urine.

The possibility exists that ketamine, or ketamine in combination with xylazine, is being used illicitly to affect the performance of racehorses. This study was undertaken to identify the metabolites of ketamine in the urine of adult horses and to evaluate methods for detecting and confirming ketamine administration. Detection of ketamine and two ketamine metabolites is described using thin-layer chromatography (TLC) and their identities are confirmed by comparing their mass spectra and gas chromatographic retention times with those of authentic standards.

[Activity of the hypophyso-gonadal axis in premature boys].

Postnatal evolution of testosterone, LH and FSH plasma levels was studied in 30 premature boys. Increase in LH and testosterone levels at the age of 2 months was found in these children as in full-term children; both parameters were positively correlated. Activation of hypophysogonadal function in premature seems to depend on postnatal age and not on gestational age.
