[Hepatitis B screening in pregnant patients and their children--a Greifswald perinatal-pediatric study].

During a period of one year with systematic screening performed in 1,401 pregnant women 8 cases positive for HBs-antigen could be detected. For the investigation we used a counter electrophoresis method. In 500 serial specimens an additional examination was performed by aid of a RIA as well. With an incidence of 0.57% the number of HBs antigen positive pregnancies in the territory of Greifswald was found to be significantly higher than reported previously from other regions of the GDR. Our results claim for a mass screening for hepatitis B during pregnancy. Because of the possibility of virus excretion with human milk, HBs positive mothers should not be used as milk donors. Other results related to our investigations give evidence for the significance of obtaining specific hygienic regulations when medical care is given to asymptomatic HBsAg carriers. They also underline the necessity of interdisciplinary cooperation for teaching medical staff in health care centers and giving correct advice to those detected as hepatitis B carriers.

Studies on the kinetics and distribution of dihydralazine in pregnancy.

Plasma kinetics of dihydralazine (50 mg p.o.) was studied in 11 women in late pregnancy. The distribution pattern between maternal and umbilical plasma was investigated in 12 patients who received 75-100 mg dihydralazine per day. In one patient amniotic fluid concentration and in another breast milk levels could be evaluated twice. Plasma concentrations of dihydralazine were very low. Continuous plasma level curves could be estimated in only four of the examined patients. In every case dihydralazine concentrations were higher in umbilical blood than in maternal plasma. Concentrations found in breast milk were clinically negligible.

Vision and the skin camouflage reactions of Ambystoma larvae: the effects of eye transplants and brain lesions.

Salamander larvae typically adapt their dermal melanophores to achieve camouflage, and it has been known for some time that removal of the eyes abolishes the response. Here we survey the contribution of the optic system to the bright and dark camouflage reactions and report that: the stimulus depends on an interaction between the direct and reflected light; an eye mounted atop the head and oriented vertically tended not to support camouflage, even though the animal responded to visual cues and learned a vision-dependent task; deviating the transplanted eye off the vertical axis enhanced the recovery of camouflage reactions; amputating or reorienting the telencephalon, epithalamus, pretectum or tectum did not abolish either camouflage reaction whereas lesions of the ventral optic pathway blocked brightening; transection near the midbrain-hindbrain junction--well posterior to known optic terminals--retarded the dark reaction; when the latter lesion was combined with disconnection of the telencephalon and epithalamus, contrary to predictions from the lesions executed separately, the animals lost the bright reaction; the hypophysis is necessary for darkening, but the organ supported this reaction even though detached, displaced or reoriented; and the pineal body was not essential for the grosser aspects of camouflage in Ambystoma larvae but may play an adjunctive role in fine tuning.

Effect of coupling the breathing- and cycling rhythms on oxygen uptake during bicycle ergometry.

The influence of the degree of coupling between the breathing and cycling rhythms (K) on oxygen uptake (Vo2) was examined in 30 volunteers. They cycled on an ergometer with a load equal to 50% of their work capacity 170 in two experimental runs with spontaneous breathing rhythm, and in a further two runs with acoustically triggered breathing. K was continuously ascertained. Vo2 and other respiratory parameters were measured by an automatic "breath-by-breath analysis" system. In 16 subjects, Vo2-differences between runs were correlated with the differences in K. In the majority of these subjects (12), Vo2 decreased significantly with increasing K. In 14 subjects, Vo2-and K-variations within individual runs were analyzed. Phases with higher K were regularly accompanied by a decrease in Vo2. It is concluded that coupling the breathing and cycling rhythms reduces Vo2 for a given moderate work load, although the magnitude of the Vo2-reduction varies considerably between individuals.

[Falithrom and pregnancy].

A case of uncontrolled, overdosed therapy with a coumarin in unknown pregnancy is reported on. Informations are discussed from the literature about the use of coumarin-derivates during pregnancy.

[Infant hypotonia, obesity, hypogenitalism and oligophrenia--new viewpoints on the etiology and symptoms of Prader-Willi syndrome].

We present four cases of Prader-Willi syndrome. Two of them have an abnormality of a chromosome 15, the other both show different chromosomal abnormalities. Translocations or deletions were found recently in the bands 15q11/12 in about 60% of the cases of Prader-Willi syndrome. The consequences for diagnosis, symptomatology and genetic counselling of the syndrome are discussed.

Oxygen deficit and stores at onset of muscular exercise in humans.

Single-breath O2 consumption (VO2) at the mouth and heart rate were determined in five healthy male subjects at rest, during 8 min of cycloergometric exercise (50, 100, 125, and 150 W), and in the recovery period following two experimental conditions: air breathing throughout (AA); hypoxic breathing (FIO2 = 0.11) for 6 min of preexercise rest followed by air breathing from the onset of exercise (HA). The O2 deficits and debts as well as the t 1/2 values of the VO2 on- and off-responses were determined and blood lactate concentrations measured at rest and in the recovery after 4 and 8 min of exercise. At all work loads: 1) O2 deficits were on the average 0.39 liter smaller in HA than in AA; 2) VO2 on-responses were faster in HA (t 1/2 approximately equal to 7 s) than in AA (t 1/2 = 20-30 s); and 3) O2 debts and VO2 off-responses were the same in the two conditions. Since the VO2 and heart rate levels at steady state as well as the blood lactate concentrations after 4 and 8 min of exercise were the same in AA and HA, the observed differences of O2 deficit cannot be attributed to changes of energy metabolism in the two conditions; they therefore depend on the reduction of body O2 stores at rest in HA. This, independently measured, was found to be 0.46 liters, not far from the observed O2 deficit difference (0.39 liters). Thus a decrease of O2 stores before exercise is accompanied by a reduction of the O2 deficit and faster VO2 kinetics at the onset of exercise.

[Surgical aspects of the treatment of malignant thyroid gland tumors].

95 cases with primary malignant thyroid tumours underwent surgery from 1965 to 1977. In view of the interdisciplinary character of treatment and independent of histological differentiation, total thyroidectomy should be performed. Differentiated carcinomas should be treated by selective dissection of suspected cervical lymph nodes. Surgical intervention of undifferentiated and medullary carcinomas should be followed by unilateral or bilateral neck-dissection. The survival rate is shown to depend on histological stage and extension of the tumour.

[Liver tumours in pregnancy, following previous intake of hormonal contraceptives (author's transl)].

Liver tumours developed in two women during pregnancy which followed discontinuation of hormonal contraception. Liver cell adenoma developed in a secundipara, 24 years of age, and its rupture during puerperium led to death by bleeding, after surgical intervention had failed. Hepatocellular carcinoma was recorded from a secundipara, 22 years of age, and surgically treated, following premature delivery. Problems of diagnosis and the need for interdisciplinary cooperation are discussed and compared with literature data. The pathogenesis of liver tumours in the wake of exogenous hormone inflow and in the context of subsequent pregnancy is another topic of discussion.

Phleomycin and bleomycin: molecular model building studies.

Molecular model building studies were conducted to simulate phleomycin-bleomycin (PB) antibiotics and thus assess a hypothetical polyphleomycin-DNA complex proposed earlier. While the latter model was not conclusively proved it was found to be quite consistent with the structure of PB compounds.

Electron microscopic detection of phleomycin during different waves of replication in regenerating skeletal muscle of mice following staining with mercuric chloride.

Phleomycin has devastating effects on regeneration of skeletal muscle when applied during an early wave of replication in the pre-myotube (new fibre) period of the reaction. The effect cannot be explained from the contribution of dividing cells to myotubes. Phleomycin's effects on regeneration are much less severe when the challenge coincides with a later wave of pre-myotube proliferation, effects that can be explained from the contribution of such cells to new muscle. An attempt has been made, by means of electron microscopy, to explain how phleomycin distributes early versus late wave cells, using a mercury substitution stain to detect the antibiotic. Cells of the two periods showed conspicuous differences in the staining characteristics of their chromatin. Positive staining reactions outside the nucleus were confined mainly to ribosomes. Exceptions included materials in transit across nuclear and cytoplasmic membranes.

[Individual selection for operation in the surgery of ulcer].

In 918 operations of gastroduodenal ulcer the operation was individually chosen. The critically indicated selective oral vagotomy with or without drainage operation yielded comparatively good results, it is a valuable supplementation of the previously only performed resection treatment. Ulcus duodenum and ulcus pepticum jejuni post-operativum are the domain of vagotomy, whereas in ulcus ventriculi in most cases is resected according to Billroth I. In old patients or severe concomitant diseases vagotomies--even in hypochlorhydria--yield as satisfying results as excision of ulcer and segment resections. Bleeding or perforated parapyloric ulcers were in selected cases also treated by vagotomies. On account of good early and late results the 2/3 resektion after Billroth I or II is never defective and it is more favourable for the patient than a wrongly indicated and technically insufficiency performed vagotomy. Operative techniques should be used which are mastered methodically. The vagotomy demands a critical indication and cautions technique, in the hand of an experienced operator it anticipates the removal of peptic ulcer. A final judgment is allowed only after an interval of 1 to 2 decenniums.
