The advantage of using ceramic inserts is to prevent major drawbacks of composite resins such as polymerization shrinkage, wear and microleakage. This in vitro study evaluated the marginal adaptation of two approximal ceramic insert systems after cementation to the cavities opened with ultrasonic tips. Proximal cavities with margins in enamel were prepared in 20 intact molars using ultrasonic tips (SONICSYS approx tips [microtorpedo size #2 and #3]; Siplus Instrument approximal [U-shaped]). Inserts of similar sizes (n=10) from two systems corresponding to the ultrasonic tips were placed in the cavities (SONICSYS Inlay; SDS-Inlay system), one on the mesial side and the other on the distal side of the same molar. Following cementation and thermocycling (5000 cycles, between 5-55 degrees C), cement thickness was measured at the buccal, lingual walls and pulpal floors of the proximal boxes under light microscope (x150). The mean cement thickness values recorded for SONICSYS inserts #2 (25 microm) was not significantly different (p>0.05) from that of SDS inserts of similar size (26 microm). There was a significant difference (p<0.05) in cement thickness values between SONICSYS #3 inserts (34 microm) and SDS inserts of similar size (23 microm). Comparison of mean values between the ceramic insert systems examined revealed that marginal adaptation was better at the buccal and lingual proximal walls than those at the pulpal floor in the SDS system, however, there was no difference for SONICSYS at both sizes. Ceramic inserts placed in cavities prepared with ultrasonic tips provide clinically acceptable marginal quality.

Lipoxygenase factors of arachidonic acid (mono- and di-Hetes) are not only mediators but also modulators of inflammatory reactions. They have chemotactic and spasmogenic properties; the latter are similar to those induced by the slow reacting-substance of anaphylaxis (SRS-A). The eosinophil chemotactic factor has been identified as a lipoxygenase product. Among the chemically mono- and di-Hetes the 5-S-12-R di-Hete (LTB4) and the 5-S 12S di-Hete demonstrated the most pronounced eosinophil chemotactic activity. The phospholipase-arachidonic acid sequence is involved in the IgE induced activation and secretion of human basophils. With arachidonic acid analogs the lipoxygenase transformation pathway is inhibited.
It is well established that IgE-antibodies against allergens are involved in allergic diseases. Thus, the regulation of IgE antibody synthesis appears to be a fundamental approach to its treatment. The mechanisms of the IgE-antibody response are somehow different from IgG production. It appears that the immunocompetent cells involved are distinct and exert a different susceptibility with regard to IgE- and IgG antibody production. A more precise under-standing of the cellular processes involved in IgE-antibody production will lead to novel immunotherapeutic approaches of allergic disease processes.
Large veins LMS is a rare slow growing malignant tumor originating from smooth muscle cells of the media. The authors report a case of LMS of the left common iliac vein propagating to the Inferior Vena Cava that presented with a left femoral-iliac deep thrombophlebitis. CT scan showed an uneven solid mass approximately 5 cm large within the left side of the pelvis. The mass displaced the left iliac artery and compressed the left iliac vein without a significant cleavage surface between the mass itself and the vascular structures. Location was next to the spine, medially and anteriorily to the psoas muscle. A thrombosis could be noticed within the distal segment of the inferior Vena Cava and within the proximal segment of the left iliac vein. US scan with fine needle biopsy of the mass didn't yield significant information. At surgical exploration a neoplastic mass involving and blocking the left iliac vein was found. Veinotomy performed on the iliac vein and on the distal segment of the Inferior Vena Cava but without infiltration of the vein walls. Surgical treatment consisted of asportation of the neoplastic mass, resection of the left iliac vein and thrombectomy of the Inferior Vena Cava. Histologic examination of the operated specimen revealed a mixoid LMS with vascular origin without involvement of the surrounding lymph nodes. Absence of clinical and radiological signs of relapse eight months after surgery makes further surgical and complementary (drug- and radiotherapy) treatments currently unnecessary.
Several methods have been developed for the measurement of gene amplification and expression. This study compared different molecular genetic analyses (Southern blot analysis (SBA) and polymerase chain reaction (PCR)) with immunohistochemical (IHC) evaluation of the corresponding protein content. PCR may be used as a semi-quantitative analysis of gene amplification and allows DNA extraction from paraffin-embedded blocks. SBA is more accurate than PCR to measure the exact degree of amplification, but only DNA extracted from frozen or fresh tissue can be used. We examined 23 breast tumors and 16 lung tumors, where the genes HER-1 coding for the epidermal growth factor receptor (EGFR) and HER-2 coding for p185HER-2 were analysed. Furthermore, PCR performed on DNA from frozen tissue was compared to PCR on DNA extracted from paraffin-embedded blocks. The results showed correlation between the different analyses, especially when the gene copy number was highly amplified. Some breast tumors showed moderately increased gene copy number of HER-1 by SBA, but no increased protein content by IHC evaluation. This probably reflects that minor degrees of genetic aberrations are not sufficient to cause major biological disturbances, because regulatory cellular pathways are still operating.
