Prodrugs of anthracyclines for chemotherapy via enzyme-monoclonal antibody conjugates.

New prodrugs of daunorubicin, 1c, 1e and 2c, including a galactopyranosyl residue linked to the N-3' of the daunosaminyl moiety through substituted o- or p-benzyloxycarbonyl groups were synthesized. Their low cytotoxicity and high stability in plasma fulfil the conditions for antibody-directed enzyme prodrug therapy (ADEPT). Enzymatic hydrolysis using alpha-D-galactosidase gives rise to daunorubicin by subsequent self-elimination of the spacers. However, elimination clearly depends on the aromatic substitution pattern, as demonstrated especially by comparison with non-substituted analogues.

Physeal and epiphyseal extent of primary malignant bone tumors in childhood. Correlation of preoperative MRI and the pathologic examination.

Twenty-two patients with metaphyseal primary malignant bone tumors (17 osteosarcomas, 5 Ewing's tumors) occurring before closure of the growth plate were examined with plain radiographs and MRI in order to determine the physeal or epiphyseal extent of the tumor. Results were correlated with the pathologic examination. Transphyseal spread was pathologically proven in 13 cases (59%): 12 cases of osteosarcoma and 1 case of Ewing's tumor (70% and 20%, respectively). There was no significant relation between epiphyseal invasion, age of patient, length of tumor or, in the cases of osteosarcoma, response to chemotherapy. Plain radiographs showed epiphyseal involvement in 4 cases and there were 10 false negatives. MRI revealed epiphyseal involvement in all cases; there were no false positives or false negatives. T1-weighted images in coronal or sagittal planes appeared to be sufficient. These findings are very useful in planning surgical limb salvage procedures and stress the ineffectiveness of the "barrier effect" of the growth plate against tumor spread.

Effects of a fenugreek seed extract on feeding behaviour in the rat: metabolic-endocrine correlates.

Fenugreek seeds (Trigonella foenum graecum L.) are assumed to have restorative and nutritive properties. The present work was designed to investigate the effects of a fenugreek seed extract on feeding behaviour. Experiments were performed to determine food consumption and motivation to eat as well as metabolic-endocrine changes in chronically treated animals. Male Wistar rats were given the seed extract orally (10 and 100 mg/day per 300 g body weight), mixed together with food, and control animals were monitored in parallel. The results show that chronic oral administration of the fenugreek extract significantly increases food intake and the motivation to eat. The treatment, however, does not prevent the anorexia nor the decreased motivation to eat induced by d-fenfluramine (2 mg/kg, IP). An increase in plasma insulin and a decrease in total cholesterol and very low-density lipoprotein (VLDL)-low-density lipoprotein (LDL) total cholesterol were also observed. In conclusion, chronic administration of a fenugreek seed extract enhances food consumption and motivation to eat in rats and also induces hyperinsulinemia as well as hypocholesterolemia.

[Research of the mechanism of the inhibition of insulin secretion induced by a peripheral benzodiazepine: 4'-chlordiazepam].

We have previously shown that the selective peripheral-type benzodiazepine agonist 4'-chlordiazepam inhibited glucose-induced insulin secretion from rat pancreatic islets. The present work was designed to further investigate the mechanism of this effect. Since interactions have been described between benzodiazepines and purinergic modulators such as adenosine, we studied the effect of the adenosine receptor antagonist 8-phenyltheophylline. This substance, which on its own had no effect on insulin release, prevented the inhibiting effect of 4'-chlordiazepam. We then performed experiments in the presence of forskolin, which directly stimulates adenylyl-cyclase, and dibutyryl-cyclic AMP, which enhances the intracellular cAMP content. The inhibiting effect of 4'-chlordiazepam was prevented neither by forskolin nor dibutyryl-cAMP, in contrast to the inhibiting effect of adenosine, which has been previously shown to be prevented by dibutyryl-cAMP. In conclusion, the inhibiting effect of 4'-chlordiazepam on glucose-induced insulin secretion is antagonized by 8-phenyltheophylline; the signalling mechanism of this effect appears to be independent of the adenylyl cyclase/cAMP pathway.

Nager acrofacial dysostosis and preaxial polydactyly: a further example with lethal outcome.

A sporadic malformed premature stillborn showing the typical craniofacial features of mandibulofacial dysostosis combined with unilateral extra thumb and visceral anomalies is described and classified as a preaxial acrofacial dysostosis of the Nager type (NAFD). Comparison with other rare similar cases reported in the literature is made and genetic heterogeneity discussed.

Effects of a peripheral-type benzodiazepine on glucose-induced insulin secretion.

Benzodiazepines, besides interacting with central-type receptors which mediate their well-known pharmacological actions, bind to peripheral-type receptors that are distributed in a variety of peripheral tissues including numerous endocrine organs. The present work was designed to investigate the effects of a selective peripheral-type benzodiazepine, 4'-chlordiazepam (Ro 5-4864), on glucose-induced insulin secretion in vitro. In the rat isolated pancreas perfused with a Krebs-bicarbonate buffer containing 8.3 mM glucose, the drug (10(-6) and 10(-5) M) induced a progressive and significant decrease in insulin release. Concomitantly, it induced a vasodilator response of the pancreatic vascular bed. In rat isolated islets incubated for 1 h in the presence of 15 mM glucose, 4'-chlordiazepam (10(-5) and 10(-4) M) induced a significant and dose-dependent inhibition of insulin release. In contrast, the selective central-type benzodiazepine, clonazepam (10(-6) - 10(-4) M), did not significantly modify glucose-induced insulin secretion. In addition, experiments were performed to test the effect of 1-(2-chlorophenyl)-N-methyl-N-(1-methylpropyl)-3-isoquinoline-carboxamid e (PK 11195), a peripheral non-benzodiazepine ligand proposed as a putative antagonist. This substance did not counteract the inhibitory effect of 4'-chlordiazepam but itself (10(-6) and 10(-5) M) elicited a potent inhibitory effect on insulin secretion. These results show that drugs such as 4'-chlordiazepam and PK 11195 which have a high affinity for peripheral-type benzodiazepine receptors, in contrast to a central-type benzodiazepine agonist, inhibit glucose-induced insulin secretion in vitro.

Evidence for a direct stimulatory effect of cibenzoline on insulin secretion in rats.

The effect of cibenzoline succinate, a new antiarrhythmic agent, was studied on insulin secretion in rats. Experiments were performed both in vivo and in vitro using two preparations: the isolated perfused pancreas and isolated islets. In anaesthetized rats, cibenzoline was able to increase plasma insulin levels and to reduce glycaemia. These effects were observed at 1 mg/kg i.v. in fed rats and at 3 mg/kg i.v. in fasted rats. In the isolated pancreas perfused in the presence of a slightly stimulating glucose concentration (8.3 mM), cibenzoline (2 and 6 microM) elicited a progressive and sustained insulin response in a concentration-dependent manner. In the presence of a non-stimulating glucose concentration (4.2 mM), cibenzoline was ineffective at 2 microM and slightly increased basal insulin release at 6 microM. In isolated islets incubated with 8.3 mM glucose, cibenzoline (6 and 20 microM) caused a concentration-dependent stimulation of insulin release. It is concluded that cibenzoline stimulates insulin secretion by a direct action on pancreatic B cells in rats.

Acrofacial dysostosis syndrome type Rodriguez: a new lethal MCA syndrome.

We present a female fetus with the lethal acrofacial dysostosis syndrome recently delineated by Rodriguez et al. [1990]. The present findings confirm that this syndrome constitutes a true MCA syndrome in which mandibulofacial dysostosis and severe limb reduction defects are associated with complex malformations of different organs and systems especially the CNS, the urogenital tract, heart, and lungs.
