Ventilatory function in pre-school children snorers and nonsnorers.

BACKGROUND: The value of flow-volume curves alterations for screening for sleep apnoea is controversial in adults. OBJECTIVE: The purpose of the present study was to assess the possible value of flow-volume curves, in snoring pre-school children to detect upper airway abnormal dynamics. METHODS: We analysed ventilatory function of 190 children aged 5 to 6 years from nine kindergartens in Nancy, France, according to the presence or absence of snoring as declared by their parents. RESULTS: More than half (103 = 54%) of the children never snored; 26 (= 13.7%) snored only with colds, 42 (22.1%) snored occasionally, and 19 (= 10%) snored habitually. The "saw tooth" sign described in adults by Sanders et al in 1981 was absent in all the children in this study, and the height-adjusted forced expiratory volume in one second (FEV1/m2) was similar across the four groups. The height-adjusted peak expiratory flow (PEF/m2) decreased uniformly (but insignificantly) from the group of non-snorer (2.11 +/- 0.381/s/m2) through the group of children snoring with colds. (2.05 +/- 0.37) to the group of children snoring occasionally (1.99 +/- 0.33); no significant decrease was found in the group of habitual snorers (2.06 +/- 0.36). CONCLUSION: In this group of young children, we were unable to find the alteration of the expiratory flow-volume loop described in part of the studies in adults. We cannot, however, exclude an alteration of the inspiratory arm of the loop, as this was not recorded by us. Our results, suggesting a reduction in peak expiratory flow with increase in frequency of snoring need to be validated in a larger population of children.

Is uranyl scavenger hexakis(3,6-anhydro) tetrakis(2A,B,D,E-O-octyl) cyclomaltohexaose (OCT) relevant with biosystems?

Hexakis(3,6-anhydro)tetrakis(2A,B,D,E-O-octyl) cyclomatohexaose (OCT) has been recently shown as a powerful cryptant for lead, mercury, and especially for uranyl. As previous results have been obtained in an organic solvent (methanol), a similar evaluation of OCT complex formation was achieved in aqueous medium and in the presence of membrane-mimicking systems such as phospholipid vesicles, liposomes and micelles. It was found that OCT, while completely insoluble in water, forms solid gel structures when in equimolar mixtures of water and methanol. Moreover, OCT exhibits detergent properties. Finally, OCT was successfully introduced in detergent solutions while keeping. Uranyl complexing properties. Possible applications of such models were also discussed.

[Diagnostic strategy of beta-thalassemic mutation in a Tunisian family, application in prenatal diagnosis].

At present, the application of combined methods in molecular biology allows us to carry out the prenatal diagnosis in a more rapid and less onerous manner especially when the family presents an index case. In this study, we have analyzed a family with one case of intermediate beta-thalassemia. First, we have used the denaturing gradient gel electrophoresis (DGGE). Then, we have identified the mutations by the refractory mutation system technique (ARMS PCR) using specific primers for the most frequent mutations in the Tunisian population (codon 39 (C --> T) and IVS-I-2 (T--> G) for beta0 thalassemias and IVS-I-110 (G --> A) for beta+ thalassemias). The analyzed family has shown the IVS-I-110 (G --> A) mutation in the heterozygous state in the mother and the index case. Subsequently, sequencing in the gene revealed a frameshift 8 (-AA) mutation in the father and his daughter. This patient is thus a compound heterozygote Codon 8 (-AA)/IVS-I-110. DGGE and ARMS PCR analysis of foetal DNA extracted from trophoblast culture didn't show any of the two mutations found in the family.

[Urinary infection in urology. A rapid technique for confirmation of the diagnosis and the orientation of medical treatment (author's transl)].

A rapid technique for confirmation of the diagnosis and the orientation of medical treatment of urinary infection. Performance of direct antibiotic sensitivities on urine is a rapid (14 to 18 hours) procedure for determining the presence of organisms in the urine and seeking the antibiotic most active against them. In order to assess the value of the procedure, it was compared with classical bacteriological techniques. The correlation was found to be satisfactory. This procedure of direct sensitivity studies on urine is not in to way in competition with classical bacteriological techniques but is of great value in rapidly confirming the diagnosis of urinary infection and in the orientation of therapy. It eliminates a number of the justifications of blind treatment.

[Micromethod for rabies antibody detection by immunoenzymatic assay with Staphylococcus protein A (author's transl)].

The rabies virus and its glycoprotein are used as antigens in a microimmunoenzymatic assay (IEA). Human rabies antibodies are detected and titrated using anti-human antibodies and staphylococcus protein A conjugated to peroxidase. Several mammal antirabies sera are tested by this method with protein A. The results are compared to those obtained by seroneutralization in mice, which is the standard method for rabies antibody detection.

[Comparison of inactivated rabies vaccines obtained from diploid and polyploid heterologous cells (Hak, BHK, and Vero)].

Homologous or heterologous diploid or polyploid cells have been established as good producers of virus. In this study we present three types of experimental inactivated antirabies vaccines obtained under identical conditions from three types of cells. The study concerns the following characteristics of the vaccines: protecting powers, rabies antigens and major polypeptidic components; this study was performed on supernatants of tissue cultures and concentrated and purified vaccines. Moreover, some cellular and serum contaminants as well as minor rabies proteins were analysed in the viral solutions. It, thus, appears that all these types of vaccines have a similar protecting power (Number of 50% protective doses/micrograms viral protein).

[Rapid detection of rabies antibodies by immunoenzymatic assay].

The antigen is a semipurified tissue culture virus, and peroxidase conjugated anti-human immunoglobulines are used. When sera from immunized humans are evaluated by this procedure, the values obtained show a good correlation with seroneutralization titres. The detection of antibodies by this method is earlier than by seroneutralization.

[Phototherapy: clinical and therapeutic evaluation of a 2-year experience].

The concept of seasonal periodicity in psychiatry has been studied for thirty years in different domains, particularly in that affective disorders. It seems to be established that winter seasonal depressions are correlated to a particular vulnerability to the decrease in ambient light; this led directly to the development of phototherapy as primary treatment of SAD, simultaneously to the development of the clinical concept. Since the first demonstration by Rosenthal of the efficacy of phototherapy in SAD, numerous studies have confirmed that bright light provides rapid and effective treatment for this type of disorder. Phototherapy has been used at the Centre Psychothérapique des Hôpitaux Civils de Colmar for five years in three ways: as an alternative treatment to medication, in addition to antidepressive medication, and as primary treatment of seasonal depression. We describe the evaluation protocol of this treatment, which was established in collaboration with the Psychiatric Clinic of the University of Basel. The principal inclusion criterion for this study was the presence or regular phases of depression or loss of energy in autumn and winter; the regularity of the appearance of symptoms in winter was predominant over the severity of the symptoms, their seasonal occurrence being the most important predictive factor in favour of phototherapy. Our prospective study enabled us to include, over a period of two years, 18 subjects presenting very different symptomatology. Though the sample was heterogeneous, the results of treatment efficacy are satisfactory: we note effectively a decrease of 67.5% in the modified Hamilton scale even though strict criteria for seasonality (Seasonal Screening Questionary, Rosenthal's Criteria) were not always respected. These data should lead us to widen the indication for phototherapy, particularly in patients presenting mood disorders which are not exclusively seasonal, and support the accumulating experimental data for the selective antidepressive effect of light.

[Immunogenicity and protective activity of glycoprotein extracted from rabies virus: results from comparison of preparations obtained by different methods of purification (author's transl)].

A rabies vaccine was prepared from virus grown on primary cultured bovine foetal kidney cells. After beta-propiolactone inactivation of the virus, the vaccine was partially purified by zinc acetate precipitation, G-75 Sephadex chromatography, and ultracentrifugation prior to a non ionic detergent solubilisation. A glycoprotein fraction with protective capacity was extracted by Triton X100 from the spikes of the inactivated rabies virus. An electrophoretically homogenous glycoprotein was found to have higher values of protective activity compared to the initial tissue culture vaccine. T-e yield from this extraction method was from 10 to 20 per cent, based upon an immunological assay.
