Computed tomography in the preoperative planning of oral endo-osseous implant surgery.

Computed tomography (CT) is a well-established aid in the preoperative assessment of the dimension of mandible and maxilla for endo-osseous implant installation. CT is a valuable tool for the measurement of the alveolar ridge and recognition of the course of the inferior alveolar nerve canal. In the present study, three different techniques were examined: normal CT images with coronal and sagittal slices, standard reconstructions based on axial slices, and multiplanar reconstruction and display (MPR/MPD), also based on axial slices. Six dissected human jaw bones were examined with these three techniques. Afterwards, these jaws were sawn, and the real values were measured. Comparing these scores with the radiological measurements, the standard reconstruction technique seemed the most reliable method in the preoperative examination of the jaw bone quality and bone proportion.

CT scan standard reconstruction technique for reliable jaw bone volume determination.

Computed tomography can assist the surgeon in planning the exact three-dimensional positioning of endosseous implants in the jaw bone. This study examined the reliability of three CT techniques: direct imaging (normal coronal and sagittal slices), standard reconstruction (based on axial slices), and multiplanar reconstruction and display (also based on axial slices). The three imaging techniques were used to estimate the bone height, bone width, and maximal implant length in six dissected human jaw bones. These jaws were then sawed perpendicular to their axis to enable measurement of real values for comparison. The sagittal and coronal slices frequently were overestimated, especially in the canine and premolar regions. The mean absolute deviation was 1.4 mm. Standard reconstruction offered the most reliable cross-sectional images, with a mean absolute deviation of 0.5 mm. The multiplanar reconstruction and display technique frequently demonstrated underestimations (mean absolute deviation was 2.3 mm). The standard reconstruction technique seems to be the method of choice in preoperative radiographic examination of patients before implant placement.

[Angioplasty using a Nd:YAG laser with a sapphire probe].

Laser angioplasty with Sapphire Contact Probes coupled to a Nd:YAG laser followed by balloon angioplasty was performed in 30 patients with 26 femoropopliteal and 4 iliac artery occlusions. Mean length of the occlusions was 4.5 +/- 1.6 cm with a mean duration of occlusion of 7.06 +/- 6.18 months. Initial angiographic success was obtained in 25/30 (83%) procedures, including 3/4 successes in the iliac artery (75%) and 22/26 successes in the femoropopliteal artery (84%). Reason for failure was perforation in 2 patients, and subintimal passage of the probe in 2, and heavy calcification of the lesion in 1 patient. Early rethrombosis in 3 patients reduced the primary success rate to 73%. Peripheral emboli necessitating surgical intervention occurred in 2 patients. Of the 22 recanalized arteries 21 remained patent with a mean follow-up of 8.4 +/- 3.3 months. It remains to be determined whether the angiographic and clinical short and long term success rate of "laser assisted balloon angioplasty" will be greater than after thrombolysis or conventional balloon angioplasty.

Transcatheter arterial embolization in the management of gynaecological bleeding.

Transcatheter arterial embolization was attempted in ten patients with gynaecological bleeding. It concerned eight malignant lesions (five cases of spinocellular carcinoma of the uterine cervix, one case of endometrium carcinoma, one case of choriocarcinoma and one case of vulvovaginal metastasis of hypernephroma), and two benign lesions (one uterine arteriovenous malformation and one bleeding after curettage). Bleeding could be stopped in six patients and considerably reduced in two. In two patients selective catheterization of the uterine artery was impossible due to atherosclerotic tortuosity in one and tumoral invasion in the other.

Preliminary results of percutaneous transluminal atherectomy.

Percutaneous transluminal atherectomy with the Simpson atherectomy catheter was performed in 5 patients with severe atheromatous stenoses of the femoropopliteal arteries. In all patients plaque material was removed with successful restoration of vessel patency. Further clinical follow-up will have to determine whether recurrence rate of stenosis will be lower with this method than with conventional balloon angioplasty.

Outpatient angiography.

Outpatient angiography of the abdominal aorta and the peripheral arteries of the under limbs was performed in 100 patients with complaints of peripheral vascular disease. In all patients small 5F catheters, a low-osmolar contrast medium and intraarterial digital subtraction angiography could reduce the local and general invasiveness of the angiographic procedure. Two complications occurred. In one patient thrombosis at the puncture site was noted. In another patient, an onset of angina pectoris was a minor and transitory complication. In 42 out of the 100 patients, treatment was conservative so that hospitalization could be avoided. With newer technical equipment angiography can be performed safely on an outpatient basis with important costsaving for the community. The late occurrence of some complications though, requires good information of the patient.

Diagnosis and differential diagnosis of branchial cleft cysts by CT scan.

The author stresses the importance of CT scanning in diagnosing branchial cleft cysts (BCCs) and in differentiating them from other cystic masses of the neck and parotid region. The CT examinations of 40 patients with neck or parotid cystic lesions suspected of being BCCs have been reviewed and correlated with the histological diagnosis. Sixteen BCCs (12 of the second and 4 of the first cleft) and 24 BCC mimics were evaluated. A definitive CT diagnosis of second BCC was possible in 83% of the cases. Exclusion of this diagnosis was possible in 94% of BCC mimics. The total accuracy of CT was 90%. Differentiation of first BCCs from their mimics is more difficult. Most first BCCs are located juxta-parotidal and at the lower pole of the parotid gland.

Angiographic diagnosis and treatment of posttraumatic carotid-cavernous fistulas.

Balloon embolization was attempted in 7 patients with posttraumatic carotid-cavernous fistulas. The procedure was successful in 4 patients and failed in 3 other. Two of these fistulas showed spontaneous regression after failed embolization. In one patient, premature detachment of the balloon led to cerebral infarction, but with total clinical recovery. One patient showed temporary palsy of the VIth cranial nerve after embolization.

Pre-operative embolization of juvenile nasopharyngeal angiofibromas.

Pre-operative embolization of juvenile nasopharyngeal angiofibroma was performed in 15 patients. The lesion was supplied by the internal carotid arteries (8 cases), by the internal maxillary artery (15 cases), the accessory meningeal artery (10 cases) and the ascending pharyngeal artery (10 cases). Superselective embolization of the external carotid artery feeders was performed with Ivalon particles, without neurological complications. Good control of per-operative blood loss was noted in 13 out of 15 cases, 2 patients presenting severe per-operative venous bleeding. Recurrence was noted in only one patient, which could be controlled by reembolization.

Solitary or predominantly right-sided varicocele: a possible sign of situs inversus.

Among more than 850 patients undergoing outpatient sclerotherapy of the internal spermatic vein, only 1 unilateral and 1 predominantly right-sided varicocele were encountered. In both patients with this finding a total situs inversus was present, with inversion of normal anatomy, enlarged "right" internal spermatic vein draining into the renal vein, and the "left" internal spermatic vein entering the inferior vena cava directly. Total situs inversus should be considered whenever right-sided varicocele is solitary or predominant.

Percutaneous transluminal atherectomy: preliminary results.

Percutaneous transluminal atherectomy with the Simpson atherectomy catheter was performed in 10 patients with 14 severe atheromatous stenoses of the femoropopliteal arteries. Removal of plaque material with restoration of vessel patency was successful in all patients without complication. Further clinical follow-up will have to determine whether the recurrence rate of stenoses will be lower with this method than with conventional balloon angioplasty.

The angiographic incidence of renal artery stenosis in the arteriosclerotic population.

The incidence of renal artery stenosis was evaluated with intra-arterial digital subtraction angiography in 100 consecutive patients referred for peripheral arteriopathy. Fifty-seven patients were normotensive, 43 were hypertensive. In the normotensive group, renal artery stenosis was found in ten patients (17.5%). In the hypertensive group renal artery stenosis was found in twelve patients (28%). It is concluded that the incidence of renal artery stenosis is high in an arteriosclerotic population both in normotensive and hypertensive patients.

Intravascular ultrasonography, a new alternative for the assessment of vascular lesions.

Intravascular ultrasonography (IVUS) was performed in ten patients undergoing a diagnostic or therapeutic angiography. The ultrasound probe was inserted in a 6 French catheter and introduced through a 7 or 8 French sheath. Exact information was yielded about the extent of arteriosclerosis, the stenotic process and the composition of the atheromatous plaque (soft plaque or presence of calcifications). In the patients undergoing a percutaneous transluminal angioplasty, the status after dilation was also assessed, and intimal tears and residual atheromatous plaques could well be evaluated.
