Neurologic applications of positron emission tomography.

The impact of computerized neuroimaging in the neurologic sciences has been so dramatic that it has completely changed our approach to the individual patient. Further changes may be expected from the newborn positron emission tomography (PET) and nuclear magnetic resonance (NMR) in order to help the reader digest a large bulk of data and fully realize the present state of the art of PET, the authors have shaped this review mainly on results rather than on methods and on published reports rather than on future potential.

Tracking of indium-111-oxine labelled lymphocytes in autoimmune thyroid disease.

Peripheral lymphocytes (approximately 10(8)) from 4 subjects affected by autoimmune thyroid disease (2 Hashimoto's thyroiditis, 1 primary myxoedema, 1 Graves' disease) and 4 normal subjects were labelled in vitro with 40 microCi of indium-111-oxine and, following autologous injection, the distribution of the cells in the body was investigated by gamma camera imaging. Lymphocytes in the thyroid were observed 24 h after injection in both patients with Hashimoto's thyroiditis and in the patient with primary myxoedema, but not in the patient with Graves' disease or in any of the controls. To our knowledge, this is the first report using this method to try and demonstrate lymphocytes in the thyroid gland, and supports the concept that cell-mediated immunity may be playing an important role in the pathogenesis of Hashimoto's thyroiditis and primary myxoedema.

Regional cerebral blood flow and oxygen consumption in human aging.

The oxygen-15 continuous inhalation technique and PET were used to study the age-related changes in regional CBF and CMRO2. Twenty-seven patients, aged 19 to 76 years, free of any history of cerebral disease and vascular risk factors were examined in "resting state." CBF, CMRO2 and oxygen extraction fraction (OEF) values were calculated in seven different brain structures as well as in mean gray matter. Left-right ratios were also computed for all symmetrical structures analyzed. Mean gray CBF, but not mean gray CMRO2, decreased linearly with age (p less than 0.02). However, when younger subjects (less than or equal to 50 yrs) were compared to older subjects (greater than 50 yrs), an age-related matched decrease in CBF and CMRO2 was observed in mean gray matter (18% and 17%, p less than 0.05) and in all gray matter regions analyzed, particularly in frontal, temporo-sylvian and parieto-occipital cortex. White matter CBF and CMRO2 remained remarkably stable with advancing age. Although the possibility of methodological artifacts was considered, we favor progressive loss of cortical neurones and/or diminished activity of those remaining to explain our findings. In addition, age-related changes in cognitive activities might also be involved.

The combined use of radionuclide angiography and Doppler-sonography in the screening of carotid lesions.

The aim of our research is to check if the combined use of radionuclide angiography and Doppler-sonography leads to correct screening of carotid lesions in the neck, independently from the indexes of reliability of the individual examinations. 125 cerebrovascular patients (for a total of 250 carotid studies) were investigated with radionuclide angiography and Doppler-sonography. 100 of these carotids were also angiographically investigated. On the basis combined angioscintigraphic and Doppler results, these patients have been classified in four groups. When both non-invasive examinations result negative, the angiography shows pathological arteries only in 22% of cases. When only radionuclide angiography is positive, the angiography shows pathological findings in 63% of cases: when only Doppler is positive, the angiography confirmation occurs in 72% of cases. Finally, when both non-invasive tests results positive, there is always arterial pathology in the angiography.
