[Inferior vena cava thrombosis responsible for chronic Budd-Chiari syndrome during hepatic and digestive amyloidosis].

The case of a 39 year old woman with amyloidosis of the liver and the digestive tract presenting with obstruction of the inferior vena cava is reported. Computed tomography after bolus injection of contrast material revealed a typical patchy fan-shaped image, of hepatic vein obstruction. The association of Budd-Chiari's syndrome with amyloidosis may be related to the increased risk of thrombosis observed in the latter disease.

[Treatment of cholesterol gallstones with ursodesoxycholic acid (author's transl)].

A double-blind clinical trial comparing ursodesoxycholic acid and chenodesoxycholic acid in patients with cholesterol stones in the gall-bladder showed that ursodesoxycholic acid was superior to the older drug not so much in percentage of biliary calculi dissolved but in dosage reduction (50%) and improved clinical and biological tolerance.

[Irreducible ascites in cirrhotic patients. Results of peritoneo-jugular shunt in 16 patients (author's transl)].

Peritoneo-jugular shunt with Le Veen's valve was performed in 16 cirrhotic patients with intractable ascites. The immediate effects were remarkable, with only one early death. In the long term, however, the results progressively deteriorated. Relapses occurred frequently (60%) and sometimes repeatedly, and were due to obstruction of the shunt, which had to be replaced, or in some cases to insufficient flow rate. Numerous complications were observed including, in particular, infections and digestive haemorrhages. Consumption coagulopathy was detected by laboratory tests in 5 cases, but did not produce bleeding. After 8 to 25 months, 31% of the patients are still alive.

[Hepatitis: the problem of terminology (author's transl)].

Every case of hepatitis must be defined as far as possible on the basis of four reference criteria: aetiological, clinical presentation, histological and course. Each criterion offers information concerning the classification, severity and course of the disease. The use of these criteria makes it possible to avoid all errors of interpretation and to avoid imprecise terminology, a typical example of which is "subacute hepatitis" which for some implies severe acute hepatitis running a prolonged course and for others the existence of confluent necrosis without it being possible to say whether this should be classified amongst cases of acute hepatitis or chronic hepatitis. The terms acute and chronic should retain their clinical significance, the term aggressive should retain a histological significance and, insofar as the piecemeal necrosis which characterises it is seen in both types of hepatitis, it should be dissociated, in terms of classification, from chronic hepatitis.

Aspirin and gastrointestional bleeding. Interest of plasma salicylate determination.

From history-taking and from analysis of plasma salicylate levels it is shown that a link exists between aspirin and gastrointestinal bleeding in 68% of cases. Salicylate levels alone indicate that aspirin has been taken in 22% of cases. Plasma salicylate measurement and endoscopy allow a better understanding of haemorrhagic lesions due to aspirin. Aspirin is responsible especially for haemorrhage from ulcers and acute gastritis or duodenitis. Aspirin is seen to be dangerous in a moderate number of susceptible individuals: those with peptic ulcer constitution or cirrhosis.

[Chronic active hepatitis with a favorable course, after fulminant hepatitis].

The case reported here is that of a girl with presumably viral non A, non B, acute hepatitis with a very unusual course. Fulminant hepatitis with submassive and bridging hepatic necrosis and a 17-day coma began during the 7th week of evolution. Prolonged chronic active hepatitis followed. Treatment was initiated 6 months after the beginning of the affection and was maintained for more than 2 years, with an apparent cure persisting after follow-up period of 6 months; fibrous scars were the only abnormalities demonstrable on histologic examination of liver biopsy. It is possible that such type of fulminant hepatitis with unusual course will become more frequent, as survival of the initial acute episode increase.

Perhexiline maleate-induced hepatitis.

5 patients treated with perhexiline maleate, 200-400 mg/day for at least 6 months, exhibited evidence of hepatitis. The picture was very similar to acute alcoholic hepatitis, clinically, biologically and histologically with presence of necrosis, Mallory's hyaline, polynuclear infiltration and to a lesser degree, steatosis. Association with peripheral neuropathy, hypoglycemia, and renal failure appears strikingly frequently. The evolution was severe since 3 patients died within 6 months, even after treatment withdrawal. Further studies are to be done to understand the mechanisms of hepatic and neurologic toxicity, and to measure the hazards of this drug. These studies could bring a new insight to alcohol toxicity.

Tissular immunoenzymatic detection of hepatic alphafetoprotein in human hepatomas.

5 human cases of hepatoma have been chosen with respect to their different seric alpha-fetoprotein (alpha-FP) level and histological characters. Cells producing alpha-FP have been studied with specific horseradish-peroxidase-labelled immunoglobulins. Ultrastructural examination shows that alpha-FP is present in the cytoplasm of some tumoral hepatocytes. alpha-FP is also present in the cytoplasm of some rare nontumoral hepatocytes of a nonsecreting hepatoma. Ultrastructural differences are described in tumoral hepatocytes according to the grade of differentiation of the tumoral cell population. alpha-FP production appears to be restricted to moderately differentiated tumoral hepatocytes. These observations led to the hypothesis that production of alpha-FP may transiently develop either during the differentiation of tumoral hepatocytes, or during the new differentiation of nontumoral hepatocytes involved in a proliferative process.
