Use of the free jejunal loop to replace cervical oesophagus in an experiment.

The paper presents the results of experimental studies exploring the possibility to replace the cervical oesophagus with a free intestinal loop. The experiments were carried out in dogs and cervical oesophagus replacement was performed using a jejunal loop. Nutrition was provided by mesenteric vessels anastomosed by means of a microsurgical operative technique. The intestinal segment was subsequently used to replace the resected segment of the cervical oesophagus. The results of our experiments indicate this new operative technique can be used in clinical practice.

[Surgical aspects of treatment of esophageal achalasia].

Based on experience assembled with surgical treatment of 88 patients with achalasia of the oesophagus in 1980-1988 the authors assume that patients in the second stage of the disease where dilatation treatment produced only temporary relief lasting several weeks and all patients in the third stage of the disease are indicated for treatment. The authors prefer the abdominal approach to the cardia and distal oesophagus as the cardia has an intraabdominal localization. It is also possible to treat other intraabdominal diseases and an antireflux operation can be easily performed. Patients tolerate laparotomy better than thoracotomy. In the author's opinion myotomy extending over 8 cm is sufficient Its protraction to the dilated portion of the oesophagus is not necessary. The antireflux operation is made only when before operation gastrooesophageal reflux was present or where during operation the oesophageal mucosa is open or where mobilization in the area of the oesophageal hiatus damaged several anatomical antireflux structures. Resection of the oesophagus in the IIIrd stage of the disease is exceptional in the authors' opinion.

[Subtotal colectomy in the treatment of obstructive tumors in the left half of the colon].

The authors give an account of 14 patients during a two-year period where on account of an occluding tumour of the left half of the colon they performed subtotal colectomy with ileodescendent--in 2, ileosigmoideal--in 10 and ileorectal anastomosis in two patients. After operation and by the time when the material was processed, none of the patients died. Based on data in the literature and the authors' own material, the authors discuss the advantages of subtotal colectomy, as compared with hitherto used multistage procedures. The advantages include in particular a lower operative mortality (0-14%), a higher five-year survival (46%) a markedly lower morbidity, time economy, a shorter period of hospitalization and in particular, the patients are spared colostomy, even temporary one. The authors recommend subtotal colectomy as the method of choice on account of ileus caused by tumourous occlusion of the left half of the colon.

Acute cricopharyngeal obstruction in dermatomyositis.

We report a case of a 44-year-old woman with dermatomyositis complicated by acute total cricopharyngeal obstruction resistant to corticosteroid therapy. This condition was successfully resolved by cricopharyngeal myotomy.
