Palliative and adjuvant chemotherapy of metastatic renal cancer.

Despite the prevalent scepticism regarding its value, chemotherapy for metastatic renal cell cancer was used in 45 patients, of whom 44 were nephrectomized, aged between 34 and 75 yr with a mean age of 57.3 yr. In 32 patients chemotherapy was used as palliative treatment because of the presence of their often multiple metastases or in 13 patients following surgical removal of metastases and/or histologically high-grade malignancy, as adjuvant treatment. Our treatment schedule consisted on day 1 of vinblastine 5 mg/m2 to a maximum of 10 mg provided intravenously in 1 or 2 to 3 week intervals X 6 and CCNU 130 mg/m2 orally each 6th week. This treatment schedule was supported by an intensive antiemetic regimen. Gastrointestinal side effects were well tolerated while 30% hematological depression necessitated extension of treatment intervals. The mean number of cycles was 2.5-3.7, respectively, with a maximum of 6. Palliative treatment resulted in 19% complete plus partial remission, 62% stabilization, and 19% progression, while adjuvant therapy proved to be superior with 10 of 13 patients in remission at 3-21 months, and 1 of 3 patients with metastatic recurrence in further remission for 20 months; two of three patients died. Palliative chemotherapy with CCNU-vinblastine acted successfully by blocking cancer progression, while adjuvant treatment together with surgical extirpation of metastases may have prolonged tumor-free remission.

Impaired primary, but not secondary, immune response in breast cancer patients under adjuvant chemotherapy.

The antibody response after vaccination against tick-borne encephalitis (TBE) was studied in patients with breast cancer. Although sex- and age-matched control persons produced high titers of anti-TBE antibodies 2 to 4 weeks after the second of two consecutive vaccinations, patients with breast cancer who were first vaccinated after the start of adjuvant chemotherapy consisting of cyclophosphamide, methotrexate and 5-fluorouracil (CMF) failed to do so. The lack of anti-TBE antibody production was found not only in patients under CMF chemotherapy, but also in those primarily vaccinated 6 to 12 months after the termination of CMF treatment. Patients with breast cancer who had been vaccinated either before or after the onset of disease, but before the initiation of chemotherapy, were shown to have developed significant anti-TBE antibody titers which persisted throughout the course of adjuvant treatment and could be boostered by revaccination during the course of CMF administration. The authors conclude that patients with breast cancer undergoing adjuvant chemotherapy experience a serious and prolonged defect in primary antibody production, whereas secondary immune responses remain unimpaired.

[Aminoglutethimide therapy in advanced breast cancer].

45 women (postmenopausal or after former oophorectomy) with metastatic breast cancer resistant to tamoxifen as well as to chemo- and radiotherapy were treated with aminoglutethimide (750 mg or 1000 mg resp. daily) and hydrocortisone (40 mg daily). Treatment resulted in 33% remissions (2 CR + 13 PR) and 53% stabilizations (24 patients) besides a non-responder rate of 13% (6 patients). Side effects (mainly central sedation, possibly exanthema, seldom anorexia, but no hematotoxicity) were observed in 13 patients (29%). Duration of remission lasted for 1 to 24 months (mean duration 10,5 months) while stabilization extended for 2 to 17 months (mean duration 7,7 months). Mechanism of action by enzymatic inhibition of the adrenal steroid hormone synthesis ("medical adrenalectomy") as well as by extra-adrenal diminution of estrogens is considered in detail. Aminoglutethimide provides effective treatment in patients with generalized metastatic breast cancer followed by best outcome in cases with osseous metastases. Clinical results are presented as an approach to successful therapy of advanced breast cancer and an endocrine alternative in tamoxifen resistance as well as a therapeutic possibility in cytotoxic induced pancytopenia.

[Treatment of metastasizing breast cancer with aminoglutethimide].

Aminoglutethimide was applied for treatment of 10 patients (8 postmenopausal, 2 ovariectomized) with metastasizing breast cancer. The patients were extensively pretreated with cytostatic and hormonal measurements and already completely resistant to these therapies. Using aminoglutethimide (500-1000 mg daily) plus additional substitution with 20-40 mg hydrocortisone one complete and three partial remissions could be achieved. Stabilization was observed in 3 cases. The duration of remission varied between 4-8 months in 3 cases and lasted one month in a further patient.

Increased monocyte-mediated antibody-dependent cellular cytotoxicity (ADCC) in Hodgkin's disease.

Monocyte-mediated antibody-dependent cellular cytotoxicity (ADCC) was tested in 23 patients with histologically proven Hodgkin's disease and 29 healthy normal controls. Seven patients presented with active and 16 with inactive disease. The lytic capacity of the individual monocytes was significantly (P < 0.02) higher in patients with Hodgkin's disease than in normals. However, no significant difference was found between the numbers of monocytes in both groups of individuals, as determined by non-specific esterase staining. No correlation was found between the lytic capacity of monocytes and the activity of the disease.

[Mesothelioma of the peritoneum with pulmonary metastases (author's transl)].

Report of the case history of a female patient suffering from malignant mesothelioma of the peritoneum. Exceptional findings were a dense nodular hematogenic dissemination over both lungs and the duration of disease, which till now extends over 35 months. Pathogenesis, course of disease, diagnosis therapy and epicrisis are discussed.

Concept of cytostatic therapy in advanced tumours of the head and neck.

In cases with inoperable tumours of the head and neck some benefit may still be derived from cytostatics and radiotherapy. One treatment concept is based on intraarterial infusion therapy (bleomycin, methotrexate), while the systemic intravenous application of cytostatic substances (bleomycin, cyclophosphamide) is considered as an alternative of the intra-arterial approach is ruled out. Of the 31 intra-arterially and the 30 intravenously treated patients (all except 2 had squamous cell carcinomas), 64,5% and 20%, respectively, went into remission. This documents the validity of our treatment concept.

[Roentgenological differention of intrapulmonary findings in patients with hodgkin's disease (author's transl)].

The spectrum of intrapulmonary findings of Hodgkin's disease is illustrated by 75 pulmonary episodes of 22 patients with Hodgkin's disease (duration of illness up to 14 years). Both the typical and frequent, as well as the rare pulmonary manifestations are discussed. We analysed the time correlation between pulmonary changes and clinical manifestations, and the effect of therapy. This retrospective investigation showed that more than 55% of the observed 75 pulmonary episodes have been diagnosed correctly by the initial radiological examination. By simultaneous appearance of different forms of disease propagation, sideeffect of therapy and superimposed diseases, diagnosis may be impaired.

[Oral 5-fluorouracil therapy in liver metastasis].

The therapeutic results of 5-FU given by oral application (15 mg/kg body weight x 10 days) in 11 patients with hepatic metastases (after colorectal and mammary carcinoma) are reported: objective response in 2 patients and subjective improvement in 4 patients contrasts to 3 patients without response and 2 patients with indefinite response to adjuvant chemotherapy. However, a long lasting complete remission in a female patient with important metastatic hepatic involvement following breast cancer must be emphasized.

[ECG changes in adriamycin therapy].

185 electrocardiograms, of a total of 39 patients were analyzed during and after Adriamycin treatment. Total Adriamycin doses did not exceed 70--120 mg in each course, respecting the 550 mg/m2 limit. ECG changes were found in 80% of such therapeutic regimen, changes of heart-rate, PQ, QT-intervals and ST/T predominating, Arrhythmias (supraventricular and ventricular ectopic beats as well as AV-blocks grade I--II) were observed in several patients.

[Ixoten therapy in malignant lymphomas].

Clinical experience with Ixoten (trofosfamide), a derivative of cyclophosphamide, is reported in 29 patients with malignant lymphomas. Daily doses of 150-300 mg p.o. and 6.000-200.000 mg total dose respectively, resulted in an 80% complete or partial remission. In addition to a dose dependent haematotoxicity, other side effects (gastrointestinal and/or hair loss) were almost negligible.

[Hemolytic anemia in Hodgkin's disease].

In 12 of 128 patients with Hodgkin's disease (10%) haemolytic anaemia could be observed. In 3 of 12 patients an (auto-)immunological etiology of haemolysis was proven by the positive antiglobulin-test. In the rest of patients (9 of 12) the haemolytic syndrome could be explained by the concomitant enlargement of the spleen. In addition to high-dose prednisolone, splenectomy is the method of choice for the treatment of the haemolytic syndrome (4 of 6 patients). If adequate treatment of Hodgkin's disease is provided, the disease (2--37 months) will not be influenced by haemolytic anaemia.

[Monocyte cytotoxicity in Hodgkin's disease].

Monocyte mediated antibody dependent cellular cytotoxicity (ADCC) was determined in 23 patients with Hodgkin's disease and 29 normal healthy controls. ADCC activity of monocytes was found to be significantly increased (p less than 0.01) in patients when compared to the controls. No significant difference was observed in the number of non specific esterase positive-monocytes in both groups of individuals, investigated. Thus increased ADCC activity of monocytes in Hodgkin's disease in this particular ADCC system is due to an increased lytic capacity of monocytes.
