[New drugs for substitution treatment of opioid dependence. Buprenorphine and 1-alpha-acetylmethadol (LAAM) ].

Two new long-acting opioid agonists have recently been introduced for the substitutional treatment of opioid-dependent patients: Orlaam (LAAM) and Subutex (buprenorphine). Both have a dose-related duration of action two to three times that of methadone, and can be given only three times a week instead of daily, thus reducing the need for take-home doses. Further, the longer duration of action provides a smoother blood level with fewer fluctuations between doses. With the availability of buprenorphine and LAAM as alternatives to methadone, agonist treatment can be more differentiated, and one can hope that this will increase the quality of the treatment.

[Prevalence of HIV and hepatitis B and C among drug addicts in the city of Copenhagen].

Blood samples were drawn from 291 drug users attending methadone clinics throughout 1997 and interviews performed about associated risk factors for seroconversion. The overall prevalence of HIV was 3.4%. Sixty-four percent tested positive for HBV and 75% for HCV. Almost all with a ten year history of drug use acquired one or more of the infections, including those who reported not to have injected. Among the sexually active 54% never used a condom and 14% of the injectors had shared needles within the last six months. Prevention measures contributing to the low prevalence of HIV had apparently no effect on transmission of hepatitis B and C. The findings call for more focus on hepatitis prevention including information, counselling, testing and vaccination.

[Meningococcal disease 1987-1989. Outbreak of serogroup B:15:P1.16 in the county of Frederiksborg].

Epidemiological features of an outbreak of group B:15:P1.16 meningococcal disease in Frederiksborg County 1987-1989 were investigated. The study comprised 149 cases notified during the outbreak and the two preceding years. One hundred and fifteen were confirmed by the isolation of Neisseria meningitidis. In 1989 the incidence had increased to 14.1 per 100,000 population. Among group B strains, B:15:P1.16. accounted for 80%. The overall mortality rate was 10%. Regarding cases due to group B:15:P1.16 strains, a significant time-space clustering was demonstrated, occurring exclusively within the 10-19 year-old group. The prophylactic measures used included administration of rifampicin to household contacts. During the outbreak the proportion of secondary cases was high (six to fifteen percent). All secondary cases occurred outside the household, indicating that the household had been protected.

[Salmonella infections in the county of Frederiksborg during the period 1983-1988].

During the years 1983 to 1988, a marked increase in the number of Salmonella infections notified to the Medical Officer of health in Frederiksborg was observed. The epidemiology of Salmonella infections has altered decisively during the past 5-10 years. The number has increased drastically and this increase is due particularly to increase in the number of isolated sporadic cases. The primary cause of the increasing number of infections is increasing contamination of foodstuffs with Salmonella. Improved hygiene in the food industry is essential if the number of infections is to be reduced. Children are frequently infected by direct or indirect contact. Salmonella infections are, therefore, a problem in day institutions and in day care homes. Contact infection may be prevented by improved hygiene and quarantine for children with infectious diarrhoea. Symptom-free excretors, on the other hand, do not present any particular risk for infection and should not be excluded from institutions. Children may excrete bacteria for several months and quarantine based on positive faeces cultures may have considerable social consequences.

[HIV transmission and risk behavior among drug addicts at a treatment institution in Copenhagen (Klub 47)].

From autumn 1985 until 31.12.1988, 127 drug addicts were examined for HIV-antibodies. Twenty-five (19.7%) were positive. No connection was found between age, sex and HIV-status. The incidence of positive test results fell from 32.8% in 1985/1986 to 0% in 1988. From autumn 1987, 49 drug addicts completed a questionnaire about risk behaviour as regards sharing syringes or needles during the month prior to commencement of treatment. Thirty-nine had no risk behaviour and the remainder very limited risk behaviour. The majority had reduced their risk behaviour on account of AIDS. Drug addicts have shown their will and abilities to reduce their risk behaviour. It is thus important to continue the informative efforts and prescription of syringes and needles. The therapeutic efforts should be extended with the object of preventing further infection in drug addicts and heterosexual and perinatal infection from the infected drug addicts.

[Examination of drug addicts on methadone treatment for HIV antibodies in the county of Frederiksborg].

In November 1988, all of the general practitioners in the County of Frederiksborg were requested to examine drug addicts on methadone therapy for HIV antibodies in December 1988 and January 1989. 81.3% of the general practitioners returned the completed form of results which they had received. Twenty-eight of the drug addicts were examined for HIV antibodies. One was found to be positive, corresponding to 3.6% (95% confidence limits 0.09-18.35%). According to the official lists of patients from whom dangerous drugs were prescribed in the Danish Board of Health, 128 persons had received methadone during the two months involved. Information was thus obtained concerning 22% of the group concerned by means of this questionnaire method of investigation. This method is thus of relatively limited value for monitoring of the spread of HIV-infection among drug addicts receiving methadone therapy.
