Tracheal Hodgkin's disease. Report of a case.

A tracheal localization of a Hodgkin's disease is described in a 28-year-old man. The diagnosis was made on a tissue fragment spontaneously emitted and was confirmed by a tracheoscopic biopsy. Radiologic examination revealed adenopathy of the anterosuperior mediastinum, but no other localization. It was not possible to determine whether the tracheal localization was initial or consequent to the mediastinal one. However, Hodgkin's lymphomas of this site have not been previously described.

Histomorphologic study of bone marrow in acute leukemia following chemotherapy and autologous bone marrow transplantation.

Twenty-six patients with acute myeloid leukemia, acute lymphoid leukemia and chronic granulocytic leukemia in blast crisis were studied by means of multiple biopsies during a polychemotherapeutic or autologous bone marrow transplant protocol. Following chemotherapy, 3 main phases were observed: leukemic cellular depletion, stromal bone marrow reconstruction, and bone marrow hemopoietic restoration. Following intensive chemotherapy (in 2 patients after cyclophosphamide and total body irradiation) and autologous bone marrow transplantation, the 3 phases appeared to be shorter. A focal or diffuse increase in marrow fibrosis was a common finding in leukemia. An effective antileukemic therapy resulted in a decrease in fibrosis, whereas in some cases a further increase was a precocious sign of leukemia relapse.

Fetal haemoglobin synthesis following fetal liver transplantation in man.

Transplantation of haematopoietic tissue from the fetus to the adult is of considerable clinical and theoretical interest. In particular it could help to understand whether the haemoglobin switch is under the influence of environmental factors or inherently programmed in the haemopoietic cell population. In the present study we have monitored haemoglobin synthesis following fetal liver transplantation (FLT) in a patient with acute leukaemia after intensive chemoradiotherapy. 28 days following FLT, the haemoglobin synthesis showed a predominantly fetal haemoglobin pattern, as determined by globin chain synthesis of bone marrow cells and by the peroxidase anti-peroxidase (PAP) method applied to bone marrow sections treated with anti-fetal haemoglobin antibodies. These findings demonstrate that, in man, fetal haemopoietic cells, transplanted into an adult environment, do not immediately switch to the production of adult haemoglobin.

[Ultrastructural histochemical study of the mammary gland of the lactating rat].

The authors report the results of their ultrastructural study on the lactating mammary gland of the rat using Grimelius' silver stain, modified for electron microscopy. The granules of the gland proteic ésocrine secretion are positive to silver impregnation, both within the cytoplasm and in the alveolar lumen. These results are interesting since they suggest that Grimelius' stain positive structures, in the mammary gland, should not be necessarily interpreted as endocrine, at least in the rat.

'Carcinoid' tumours of the breast: the morphological spectrum of argyrophil carcinomas.

Fourteen 'carcinoid' tumours of the breast are described. They are separable into five with and nine without intracellular mucin. All the tumours are argyrophil, but none is argentaffin. Four tumours studied ultrastructurally contain dense-core granules. Argyrophil carcinomas represent the endocrine analogues of ductal carcinoma in situ, of invasive ductal carcinoma and probably of lobular carcinoma also. Current views vary between the one that the so-called carcinoid is a rare and totally distinct entity to the view, at the other extreme, that it is a very common variant of conventional breast cancer. On the basis of our findings, an intermediate view is justified: argyrophil carcinomas constitute about 5% of breast carcinomas and some varieties at least have non-argyrophil analogues. Factors influencing the prognosis in individual cases are discussed. Argyrophil carcinomas of the breast form a tumour spectrum with a wide range of morphological and histochemical appearances and a variable prognosis.

Giant lymph node hyperplasia in angiohamartomatous soft tissues.

The first case of Castleman's benign lymphoma associated with angiohamartomatous alterations in peripheral fibroadipose tissue is reported, and a pathogenetic connection is hypothesized. The lesion was encountered in a 48-year-old man; it was situated in the left and inferior retroperitoneal area with extension to the inguinal region of the same side.

Hepatocellular carcinoma with intracytoplasmic hyaline globules.

A case of hepatocellular carcinoma with intracytoplasmic hyaline globules is reported. By serological, histochemical, immunofluorescent and electron microscopic studies the authors maintain that in this case intracytoplasmic hyaline globules represent lysosomal structures. According to Von Ardenne's theory, these lysosomal globules appear related to neoplastic cells necrosis.

Cholesterol embolism of bone marrow clinically masquerading as systemic or metastatic tumor.

A case of cholesterol embolism of bone marrow, concerning the pelvis and lumbar region and clinically masquerading as systemic disease or metastatic tumor, is reported in an 82-year-old man hospitalized for acute onset of reddish purple nodules on the legs and toes, intense myalgia and dorsal vertebral bone pain. The clinical manifestations leading to consideration of a systemic disease or metastatic tumor were the abnormal bone scintigraphic findings of the pelvis and lumbar region, the elevation of serum alkaline and acid phosphatase and the increase of the bone isoenzyme fractions. The diagnosis of cholesterol embolism was explained by the histological findings of bone marrow biopsy which showed microinfarctions of bone marrow with an osteoproductive and reparative process, and presence of cholesterol clefts.

[Scintigraphy using 99m-TC HM-PAO labeled autologous granulocytes in the diagnosis of Whipple's disease].

Whipple's disease is a systemic disease, mainly localized in the small intestine, that even today shows some difficulties about its etiopathogenesis and diagnosis. The surveys usually used in the diagnosis, among which the biopsy is an indispensable test, have some limits particularly in follow-up. The 99mTc-HM-PAO labelled granulocytes scintigraphy may be a useful alternative method in the evaluation of location and extension of the Whipple's disease. Furthermore, as it is not an invasive method, 99mTc granulocyte scintigraphy may be an important means in establishing the term of the therapy mainly when other methods are not able to exactly confirm the remission of the disease.

[Human subcutaneous dirofilariasis in Italy. First case in the Marche region].

The first case of human subcutaneous dirofilariasis in the Marche region (Central Italy) is described. It was caused by Dirofilaria repens and localised in the abdominal wall of a 23 years old university student from Pesaro. The presence of many histological sections of the nematode in strong regression, suggests that the death of the parasite occurred inside the nodule many months before the surgical intervention.

Angiotropic large-cell lymphoma with predominant kidney involvement. A case report.

The authors report a case of non Hodgkin lymphoma which, given its particular course and histomorphological features, can be classified as angiotropic large-cell lymphoma (i.e. proliferating angioendotheliomatosis). The most important characteristic is that the kidney was the target organ in this case, an observation rarely reported in the literature.

Initial characterization of an anti-CD5 monoclonal antibody obtained from leukemic cells infiltrating lymphocytes.

BACKGROUND AND METHODS: CD5 is a monomeric glycoprotein expressed on normal and malignant T cells and on chronic lymphocytic leukemia cells. Murine anti-CD5 monoclonal antibodies (MAbs) were obtained using T cell lines, thymocytes, and blast cells from acute T lymphoblastic leukemia as immunogen. In the present report we describe an anti-CD5 MAb obtained from hybridization of mouse spleen lymphocytes immunized with blast cells from a patient with plasmocytoma who underwent leukemic transformation. RESULTS AND CONCLUSIONS: We demonstrate that this MAb arises from the CD5 determinant of lymphocytes disseminated among the leukemic cells. We present initial characterization of this MAb, which may represent a new CD5 epitope. Potentially, due to its efficient internalization through the cell membrane, this MAb might be used to deliver toxins to immunocompetent T lymphocytes for prevention of acute graft-versus-host disease (aGVHD) after bone marrow transplantation.
