Regional chemotherapy by a totally infusion pump preceding radiotherapy in advanced cervical carcinoma: a preliminary report.

The authors report the preliminary results of a prospective study on intra-arterial chemotherapy and radiotherapy of advanced cervix carcinoma. The drugs were delivered by a totally implanted Infusaid Model 400 pump and patients received cyclic therapy consisting of Cis-Platinum, Methotrexate and Bleomycin. The catheter tips were positioned during a laparotomy in the hypogastric arteries with sutures and the pump was implanted in a subcutaneous pouch in the abdomen. After chemotherapy all patients had radiotherapy. Five patients entered in the study. Preliminary results suggest that long-term intra-arterial infusion chemotherapy for the treatment of advanced cervix carcinoma is practical for many patients. Two of five patients, after intra-arterial chemotherapy and radiotherapy, had radical hysterectomy.

[A new medico-nuclear outlook for the diagnosis and treatment of pheochromocytomas: 131I-meta-iodobenzylguanidine (131I-MIBG). Comments on results obtained].

The diagnostic and therapeutic potential of 131I-MIBG, the new imaging agent with structural similarity to norepinephrine, was evaluated in 23 patients with a suspect pheochromocytoma. Seven out of 8 benign pheochromocytomas (2 ectopic) were correctly localized with 131I-MIBG scintigraphy (1 false negative). In 2 catecholamine-secreting malignant pheochromocytomas both the primary tumor and the metastases (1 case) were demonstrated. One uncommon case of dopamine-secreting malignant pheochromocytoma, as expected, was not shown. In the remaining 12 cases, found to be "normal" after completing the work-up, distribution of the radiopharmaceutical was evaluated using a semiquantitative grading system. 131I-MIBG scintigraphy proved to be a non invasive and accurate (95%) diagnostic modality, specific for neuroadrenergic tumors. The treatment with 131I-MIBG was also carried out in 2 malignant pheochromocytomas with encouraging results.

Uterine cervical tumor metastases and their treatment.

Uterine cervical carcinoma is a locoregional disease. However, particularly in advanced stages, in the presence of N+ and pelvic relapse, it may produce secondary lesions located far from it. In our case series, consisting of 473 portio carcinomas treated at the Institute of Radiology of the U.C.S.C., we found distant metastases in 6.76% of the cases. This study examines the cases of metastasis in relation to stage, histology, risk-factors, treatment of the primary tumour and survival after the treatment of the secondary lesions.

Interstitial radiotherapy with Ir192 in vulvar cancer.

Radical surgical intervention is not always possible in vulvar tumours, particularly in infiltrating forms of paraurethral locations. In our case-series, the supplementary performance of Curietherapy, particularly with Radium substitutes (Iridium 192) and following the afterloading method, has enabled us not only to obtain long disease-free periods but also, coupled with the "large volume" of External Radiotherapy, to master forms exceeding the surgical action scope.

New perspectives in brachycurietherapy of malignant brain tumors. Preliminary report.

Between 1980 and 1988, 16 patients (9 females, 7 males) with malignant neuroepithelial tumors of the brain were treated by brachycurietherapy (BCT) and external radiation therapy (ERT) in a phase II nonrandomized study. There were 13 grade III and 3 grade IV tumors according to the WHO classification. Five patients in the grade III group who showed anaplastic foci in a generally grade II background were separately examined. Six patients received only BCT whereas 10 patients were treated by a combination of BCT and ERT, according to the tumor volume. We utilized as radioactive sources 192Ir in 14 cases and 125I in 2. Thirteen patients underwent permanent implant, 3 others received temporary irradiation with removable afterloaded catheters. The target volume was less than 50 cc in 8 cases, between 50 and 100 cc in 7, and larger than 100 cc in 1 patient. The total dose at the periphery of these volume ranged between 70 and 144 Gy. The result were analyzed referring to the following aspects: performance status, survival, side effects, grading. The impact of BCT was analyzed especially in a grade III with anaplastic foci group which showed a median survival of 6 years compared with only 2 years median survival of the pure grade III group.

[Coexistence of hyperthyroidism and thyroid carcinoma. Description of a clinical case].

The association of hyperthyroidism and thyroid carcinoma, especially multicentric forms, is an unusual event. We present the case of a 49-year-old woman with clinical and radiological findings of hyperfunctioning thyroid nodule that showed, after a subtotal thyroidectomy, histologic evidence of multicentric carcinoma in one lobe and adenoma in the other lobe. The interest of this clinical case is in the coexistence of an independent adenoma with a multicentric carcinoma, revealed by radical surgery that was necessary for the occurrence of a multinodular goiter. The incidence of this situation could be underestimated in consequence of the current surgical approach to the single "hot" nodule and suggests we pay attention to these patients for the possible presence of malignancy in functionally-inhibited thyroid tissue.

Possibilities and limits of brachycurietherapy in brain tumors.

Brachicurietherapy represents a low dose-rate irradiation able to deliver high focal doses within relatively small tumor volumes. Its application to brain tumors utilizing the stereotactic techniques is nowadays considered with growing interest. The Authors analyze, on the basis of their personal experience, the possibilities and the limits offered by this therapeutic procedure in neuro-oncology. The choice of the sources and their modality of application, the definition of the optimal dose/time and dose/volume ratio are the main problems taken into consideration and discussed.
