Extraadrenal paragangliomas. An immunocytochemical and ultrastructural report.

Although the majority of extraadrenal paragangliomas are nonfunctional, some of these tumors are associated with hormone production and clinical symptoms, notably hypertension. The authors have investigated 22 paragangliomas, five of which were diagnosed as clinically functional in a light microscopic immunocytochemical and electron microscopic study (nine cases). Histologically, all the paragangliomas exhibited similar features, with a "Zellballen" pattern of polygonal cells. All 22 cases were strongly immunoreactive to protein gene product 9.5 (PGP 9.5) antisera and moderately reactive to antineuron-specific enolase (NSE) sera. Ten cases (five functional) were focally immunoreactive to antichromogranin sera. Seven cases (four functional) were immunoreactive to neuropeptide Y and enkephalin antisera, and six (five functional) to tyrosine hydroxylase antisera. The clinically functional tumors expressed at least two of the antigens, enkephalin, neuropeptide Y, or tyrosine hydroxylase, whereas none of the 17 nonfunctional possessed more than one of these. Electron microscopic study revealed cells from all the nine cases studied to contain secretory granules. Granule sizes ranged from 100 to 280 nm and the morphologic examination of the secretory granules generally showed a dense core with a membrane-bound halo of variable size. Secretory granules were observed in the five functional cases and these were larger (220-280 nm) than those seen in the nonfunctional tumor cells (100-180 nm). Also, tumor cells from the functional cases contained numerous dilated mitochondrial profiles.

Visualization of the secretory canaliculi of human parietal cells with a peroxidase-labelled peanut lectin. Light- and electron-microscopic observations.

Peanut lectin reactivity was examined in normal fundic glands from human gastric samples, both at light- and electron-microscopic levels, using a peroxidase conjugate. Positive reaction was observed in the glycocalyx of parietal cell secretory canaliculi as well as in the mucous globules of mucous cells and in the luminal cell coat of chief cells. The presence of terminal galactose in the canalicular glycocalyx may be connected with the peculiar function of hydrochloric acid secretion. Peroxidase-labelled peanut lectin is proposed as a marker for visualizing the secretory canaliculus of parietal cells.

[Selective hypogastric arteriography in the diagnosis of vascular impotence].

Clinical employment of selective hypogastric arteriography has been instrumental in the detection of stenosing and obliterating lesions of the pudendal arteries and their penile branches capable of giving rise to impotentia erigendi as the sole symptom. Differentiation of psychogenic and vascular impotence is now possible through non-invasive instrumental examinations such as the nocturnal penile tumescence test, and Doppler ultrasonography of the penile arteries. Arteriography to clinch the diagnosis was only employed when a suspicion was aroused by these two examinations. An account is given of technique employed for selective hypogastric arteriography and the angiographic results obtained.

Endocrine cells in intestinal metaplasia of the stomach.

In this study we have investigated the mucin profile and the endocrine cell population in gastric endoscopic biopsies from 22 patients affected by chronic gastritis and intestinal metaplasia and in five surgical specimens of stomachs removed because of intestinal-type carcinoma (4) or peptic ulcer (1). High iron diamine-Alcian blue (HID-Ab) staining and peptide immunocytochemistry (peroxidase anti-peroxidase technique) were used. Forty-one foci of intestinal metaplasia were detected, 15 produced sulphomucins and 26 sialomucins. Of the endocrine cells investigated, gastrin and somatostatin cells were the most frequently observed, while cholecystokinin, glucose-dependent insulinotropic peptide-, secretin- and enteroglucagon-containing cells were also found in the metaplastic areas, but less frequently. No significant correlation was found between the type of mucin and the types of endocrine cells present, the latter usually resembling those normally found in the small intestine. On the basis of these results we conclude that intestinal metaplasia involves mucin- and peptide-producing cells of the stomach in a variable manner, with no correlation between the two.

Giant coeloadenoma of the kidney.

A case of giant adenoma of the kidney is described with special regard to the histological and ultrastructural features of the neoplastic cells. The tumoral proliferation consisted mainly of clear and dark epithelial cells among which cells of a third type are occasionally interspersed. A possible origin of this last population from the coelomatic lining is suggested and the hypothesis of a dysontogenetic nature of the neoplasia is purposed.

[Histopathology of the renal artery. Stenosing lesions causing hypertension].

The Authors describe the anatomopathological background of different types of renal artery lesion, causing renovascular hypertension. Various pathological aspects of dysplastic lesions are examined and compared with the aspects of arteriosclerotic disease. Finally correlations between angiographic and histological reports are considered, discussing when and how a surgical procedure should be advisable.

[Selective arterial perfusion with antienzymes in experimental acute pancreatitis. Preliminary experience].

Authors analyze some results obtained treating, with selective arterial perfusion (using antienzymes), experimental acute pancreatitis in dogs (fifteen). The first dogs' group was used to perform several experimental acute pancreatitis; the second group was treated by selective arterial perfusion and was related to group of dogs treated by i.v. antienzymes drugs. Histological and biochemical data are discussed. The results obtained with arterial perfusion are preliminary referred as better than classic i.v. antienzymes drugs therapy.

[Histological and clinical correlations in thyroid pathology].

The cases of 138 patients who underwent, during the last six years, an operation for thyroid affections, have been reviewed in order to compare the definitive histological response with the clinical diagnosis. On the basis of this analysis it was pointed out that, even with the aid of the commonly used diagnostic investigations, it is difficult to get a certain and sure knowledge of the anatomo-pathological picture, before beginning the operation. Furthermore, the definitive histological diagnosis itself may be wrong if it is not based upon the examination of the whole removed tissue, using as many sections as possible.
