Facilities for the management of patients with lipid disorders in the United Kingdom: results of the British Hyperlipidaemia Association Survey.

A national survey was undertaken by the British Hyperlipidaemia Association to obtain data on the current clinical practice for the investigation and management of patients with hyperlipidaemia. An additional objective of the survey was to establish a United Kingdom register of lipid clinics. The results show a considerable variation in the availability of lipid clinics, and medical practitioners involved in these clinics came from a variety of specialty backgrounds. Some clinics had clearly defined policies with regard to acceptance of referrals. However, deficiencies in clinical services were identified including the lack of dietetic and laboratory support.

New survey shows rift between health executives and radiologists.

This report marks International Radiology Group's first annual survey of hospital administrators and diagnostic imaging department heads. The survey was conducted to determine how the managers of diagnostic imaging departments at a broad range of hospitals evaluate the quality and cost effectiveness of their departments. In particular, the survey addresses how administrators and department heads evaluate the working relationships they have developed with radiologists affiliated with their facilities.

Recent legal developments affecting the medical staff.

In the past two years there has been a significant increase in the number and variety of hospital medical staff cases decided by the courts. The issues range from whether medical staff bylaws constitute a contract with individual medical staff members, the legal opportunity to assert employment discrimination claims in medical staff disciplinary proceedings, and ongoing antitrust concerns. This article presents an overview of the recent litigation related to these subjects.

Capillary electrophoresis determinative and LC-MS confirmatory method for screening selected imidazolinone herbicides from soil.

Residues of imazapyr, imazamox, imazapic, imazethapyr, imazaquin, and imazamethabenz (meta and para) are extracted from soil with 0.5 N sodium hydroxide. The pH is adjusted to 2.0-2.2, and the resulting precipitate is filtered. Compounds are trapped onto a tC18 solid-phase extraction (SPE) cartridge, then eluted from the cartridge and passed through a strong anion exchange (SAX) SPE cartridge onto a benzenesulfonic acid strong cation exchange (SCX) cartridge using ethyl acetate. After eluting the analytes from the SCX cartridge using saturated potassium chloride in methanol, the solution is evaporated and redissolved in 1% formic acid in water. The sample is then desalted using a tC18 SPE cartridge and eluted with methanol. After evaporating the methanol to dryness, the compounds are partitioned from acidic solution (pH 3.5) into methylene chloride. The methylene chloride is evaporated to dryness and the residues are then dissolved in Milli-Q water (Millipore, Bedford, MA, U.S.A.) in preparation for analysis by capillary electrophoresis. Results are calculated by direct comparison of the sample peak heights to the peak heights of bracketing standards. The validated sensitivity of the method (LOQ, limit of quantitation) is 2.0 ppb for each compound. Confirmation for individual residues greater than 2.0 ppb is provided by liquid chromatography-electrospray ionization mass spectrometry (LC-ESMS) of the final extract.

Enduring negative psychological characteristics of male and female step-children toward the step-parent.

Many divorced people remarry, and if either one or both of the remarried partners has a child or children from a former marriage, then the youth need adjust to not only the new marital structure, but to the new step-parent as well. The present research was conducted to determine: (1) enduring negative psychological characteristics of male and female step-children toward the step-parent, and (2) a comparison of the similarities and/or differences of each. Results suggested ten of the most frequently stated variables of both subject groups, similarities or significant differences between them by subject status, and their relevancy to the step-child-step-parent relationship.

Fishbein's model of reasoned action and compliance behavior of hypertensive patients.

A causal model based on Fishbein's Model of Reasoned Action (Ajzen & Fishbein, 1980) was tested in a hypertensive population. Intention was hypothesized to affect compliance behavior of hypertensive patients directly and to mediate the effect of the other variables. Attitude, perceived beliefs of others, and motivation to comply were hypothesized to directly affect intentions and indirectly affect compliance behavior as mediated by intention. Fifty-six newly diagnosed hypertensive patients completed the variable measures 6 months after receiving outpatient instructions. Path analysis demonstrated the Fishbein Model was found sufficient for the prescriptions of diet, smoking, activity and stress, but not for medication. Findings indicated that compliance behavior was directly influenced by intention which, in turn, was influenced directly by attitude and motivation to comply and indirectly by the perceived beliefs of others mediated by motivation to comply for the prescriptions of diet, activity, smoking, and stress prescriptions. For the medication prescription, attitude and motivation to comply directly influenced regimen compliance.

Regimen compliance two years after myocardial infarction.

Two-years postinfarction, the effect of a nursing intervention at 30 days postinfarction, and intentions, attitudes, and perceived beliefs of others on regimen compliance of myocardial infarction patients was investigated. The sample was comprised of 51 patients (E = 29, C = 22) who participated in a five-phase study over 2 years. No differences were found between experimental and control groups for regimen compliance to activity, stress, and medication prescriptions. The experimental group was significantly more compliant to the diet prescription than the control group. The control group was significantly more compliant than the experimental group with cessation from smoking. Perceived beliefs of others were predictive of compliance for all regimen prescriptions at 2 years. Attitude was also predictive of compliance with the diet, smoking, and stress regimens.

The management of open wounds: acute and chronic.

The history of wound healing dates back thousands of years before Christ to the Ancient Egyptians. Detailed tomb and temple hieroglyphics depict wound treatments of that era. Through the historic milestones set forth by Hippocrates (400 BC) to Lister (1800s), current theory of wound management and trends have evolved. During the last several decades, thanks to breakthroughs in research and technological advances, wound care has taken a giant leap forward.

Preoperative information and recovery of open-heart surgery patients.

Postoperatively, 19 adult open-heart surgery patients were interviewed as to information, included or not included in preoperative instruction, they identified as important to recovery. It was found that subjects desired details of all aspects of their conditions and treatment--preoperatively, operatively, and postoperatively. They had the greatest difficulty with pulmonary aspects of their care postoperatively. Subjects indicated a specific need for more information about the endotracheal tube, the ventilator, mucus, suctioning, deep breathing and coughing, and chest tube removal. Subjects experienced transient, primarily depressive, nonpsychotic postoperative mood changes unrelated to preoperative information, physical manifestations, and/or behavioral manifestations. Subjects regarded the nurse as an important provider of information and found the content of preoperative information satisfactory but wanted more individualization of instruction. They also indicated a need for more family involvement in preoperative instruction to provide them with information of the patient's progress throughout hospitalization. Questions and hypotheses for future research were generated.

Analysis of the Jalowiec Scale with cardiac patients' spouses.

The Jalowiec Coping Scale was factor analyzed using scores of spouses of 210 cardiac surgery patients. Three factors were found which agreed with the factors reported by Jalowiec. Factor I was related to Confrontive coping strategies, Factor II to Palliative strategies, and Factor III to Emotive strategies. A little over 31% of the variance was accounted for by the three factors. Alpha reliabilities were .84, .70, and .74, respectively, for the three factors.

Intracerebral malignant fibrous histiocytoma: a light and electron microscopic study with immunohistochemistry.

A 65-year-old man presented with a tumor within the right cerebral hemisphere. Biopsy showed a pleomorphic neoplasm which was diagnosed as a malignant fibrous histiocytoma only after immunohistochemical and electron microscopic examination. Full clinical and radiological investigation, including total body CAT scanning, failed to reveal any other deposits, and thus it is believed to have arisen in the brain. It is emphasised that this diagnosis could not have been reached had only conventional histological techniques been available.
