[Critical follow-up study of subjects with senile plaques: neuropathological, clinical, and electroencephalographic data (author's transl)].

Frontal isocortex biopsy or operative specimens exhibited senile plaques in 25 patients aged 40 to 73 years, association with Alzheimer's "neurofibrillary degeneration" in 20 of them. These patients were followed up for up to 14 years. 3 patients died 1 to 3 months after operation. 16 patients evolved into confirmed dementia, 12 of them most probably of the Alzheimer type. 6 patients recovered fully, and their subsequent evolution appears incompatible with a diagnosis of Alzheimer's dementia (follow-up period of 3 to 14 years). Retrospective evaluation of EEG data showed a good predictive value of previously described EEG changes, as indicative of later confirmation of alzheimer's dementia.

Contributions of intermolecular interactions between constitutive arabinoxylans to the flaxseeds mucilage properties.

The main fraction (about 75%) of the mucilage extracted from seeds of Linum usitatissimum which consists of arabino-xylans (AX) has been studied in dilute and semidilute regimes by SEC/MALLS analysis and rheology, respectively. It has been found that AX contains 3 populations of about 5 000 000 g mol(-1) (less than 10%), 1 000 000 g mol(-1) (about 40%), and 200 000 g mol(-1) (about 50%). We have also observed a great retention of polymer during the filtration procedure, which is much pronounced as the AX concentration increases. This evidences the presence of large aggregates in the solution. The retention can be greatly diminished if the filtration is conducted under higher temperature. Aggregation could result from the establishment of intermolecular associations via hydrogen bonds. This hypothesis seems to be confirmed by the two higher populations in molar masses which present a random coil conformation consistent with a low degree of branching. Rheological measurements, conducted at 20 g L(-1), have confirmed the association tendency leading to pseudo gels behavior. Viscoelastic properties have been evidenced by time-temperature master curves of dynamic spectra. Such master curves have also been established with addition of chaotropic (i.e., KSCN) and lyotropic (i.e., NaCl) salts. It has been shown that intermolecular associations are greatly diminished under chaotropic salts influence. This has been also confirmed by SEC/MALLS analysis. These results point out the role of hydrogen bonds in the organization of the AX system.

Computer-assisted patient-care management.

A computer-assisted patient-care management system is currently operating in two patient-care units as part of a pilot project at the Hôpital Cardiologique, a member institution of the Hospices Civils de Lyon. The system includes the management of administrative records (notice of admission set to the patient-care unit, appointment for admission sent to the patient, notice of admission set to the physician requesting hospitalization, admissions and discharges, patient census) and of patient-care records (orders, examinations, laboratory tests, medications, injections and treatments, a summary list or orders to be carried out during the day, labels for laboratory specimens and requistions for the tests and examinations to be performed); and the medical management of the patient (clinical summary of patient status at admission, results of tests and examinations, summary of hospitalization, data for research and statistics). Computerization was begun in 1977, following the implementation of individualized patient-care records in 1976. The system will be evaluated in 1978 and extended to the remainder of the hospital (18 patient-care units) by the end of 1979.

[Impregnation and liberation of nicotine with hydrogen copolymer discs].

Reticulated copolymers with products N-vinyl-2 pyrrolidone (NVP), hydroxy-2 ethyl (HEMA) and methyl methacrylate (MMA) were realized in disc form. Nicotine was used as active principle. On these discs the following study was carried out: permeation, percentage of liquid absorbed, texture, as well as nicotine release kinetics. A 2-variable (NVP and HEMA/MMA), 5-level experimental plan has enabled the outlining of the significant differences on the action of these parameters. The release kinetics show particularly that these discs behave as forms of prolonged released and that the released quantities are sufficient to envisage the use of these reticulated copolymers within the framework of a transdermal system.

[Acute poisoning with thyroxine in children].

Two girls aged 5 and 14 years were admitted to hospital after ingestion of L-thyroxine in estimated doses of 72 micrograms.kg and 118 micrograms.kg in the preceding 4 and 8 hours, respectively. Both had high serum T4 (19 and 20 micrograms/dl) and were asymptomatic at the time of admission and both were treated with Iopanoic acid 3 g orally every 72 hours, in order to block the conversion of T4 into T3. Serum levels of T4 were still elevated for up to ten days (8 and 16 micrograms/dl, respectively at day 10), but serum levels of T3 came down to 60 micrograms/dl within the first 48 hours of treatment in both cases. It should be taken into account that these patients may be initially asymptomatic and that symptoms may appear even later than 24 hours after the ingestion, so they should be admitted and treated at hospital. Iopanoic acid has been proved to be a treatment of choice in order to block the conversion of T4 into T3.

[The technique of ureteric advancement in vesico-ureteric reflux (author's transl)].

The authors, anxious about the crossed position of the ureteric meatuses after transverse submucosal advancement of the ureters, prefer descending submucosal advancement. They emphasise the widely open nature of the hiatus of penetration of the ureter into the bladder. Tightening of this orifice, distal to the orifice left free for the penetration of the ureter, participates in elongation backwards of the muscular layer upon which the submucosal advancement will take place. If necessary, if the trigone is too short, the vesical muscularis layer is slit in a proximal direction, uretero-vesical penetration withdrawn by an equal amount and suture of the muscular breach behand the ureter elongates a submucosal course which, without the use of this technique, would be too short. The authors have collected a series of 63 ureters. No stenosis, 3 residual reflux and at the time of follow up cystoscopy where applicable, the new meatus is easy to locate and to catheterise.
