Chemical studies on the structure of human serum high-density lipoprotein (HDL). Photochemical crosslinking of azido-labelled lipids in HDL.

The lipid classes of native human serum high-density lipoprotein (HDL) were exchanged against phosphatidylcholine, sphingomyelin and cholesteryl ester species substituted with photosensitive fatty acyl residues and against 25-azido-27-norcholesterol. The photosensitive fatty acyl residues were 5-and 16-azidopalmitic, 12-azidooleic and 18-azidolinoleic acid, all labelled with high tritium radioactivity. The lipid exchange method previously described was used. After UV irradiation and delipidation the apoproteins AI and AII, photocrosslinked with the radioactive lipids, were separated. The yield of covalently crosslinked lipid molecules amounted to 30% of the photosensitive lipid molecules incorporated into the HDL particle. ApoAI and apoAII were labelled by each of the photosensitive lipid classes, intergrated in the HDL particle although in very different stoichiometry. The regiospecific photochemical labelling of the lipopolypeptides AI and AII was established by cyanogen bromide cleavage and the separation of the four apoAI and two apoAII CNBr fragments. The analytical data prove the close steric relation and interaction of defined protein regions with hydrophobic regions of the lipid molecules by chemical means. These results are discussed with respect of the HDL structure.

Penile circumference and erection.

Simultaneous registration of penile circumference and intracavernous pressure during artificial erection was performed in 6 fresh cadavers. Maximal penile circumference was obtained at an intracavernous pressure below the pressure required for full erection. This reduces the clinical value of penile tumescence measurement in evaluation of erectile function.

Penile blood pressure.

Measurement of penile blood pressure has been advocated for investigation of erectile impotence due to arterial insufficiency in the penis. Using both strain gauge plethysmography and ultrasonic technique, and expressing the penile blood pressure as an index of the brachial blood pressure, the coefficient of variation in double determinations in non-arteriosclerotic, potent men was 6%. The normal range of the index was 0.7 to 1.02. More than 90% of men with arteriosclerotic disease of the legs and penile blood pressure index below 0.60 were impotent. The penile blood pressure thus can demonstrate, though it does not exclude, arterial insufficiency in the penis.

Gracilis muscle repair of perineal hernia following pelvic exenteration.

This report describes the use of a rotated gracilis muscle to repair a perineal hernia that occurred 18 months following pelvic exenteration for recurrence of cervical carcinoma. The anatomy of the gracilis muscle and the operative technique of the hernia repair are described. The gracilis muscle procedure offers a satisfactory correction for this complication of pelvic exenteration. The ability to mobilize the gracilis muscle for reconstructive procedures in the pelvis is a valuable procedure for the gynecologist.

Covalent cross-linking of photosensitive phospholipids to human serum high density apolipoproteins (apoHDL).

Human serum high density apolipoproteins were reassociated with three different lecithin species substituted with radioactively labelled photosensitive azido fatty acids, bis([3H]-16-azidopalmitoyl)-, bis([3H]12-azidooleoyl)- and bis([3H]18-azidolinoleoyl)glycerophosphocholine. The lipoprotein particles were reconstituted from a mixture of azido-labelled phosphatidylcholine and non-labelled dioleoylglycerophosphocholine (1:9). Excess lipid was separated from the homogeneous particles by Bio-Gel A-5m. The molecular weight, stoichiometry, fluorescence and circular dichroism of the reconstituted particles were determined before and after photoactivation with covalent cross-linking of the phospholipids with the apoproteins. The physical parameters of the reconstituted lipoproteins remained unperturbed by the cross-linking reaction between the generated nitrenes and apolipoprotein A-I and A-II. Thus the hydrophobic interactions of the phospholipid molecules with the apoproteins have been proved for the first time by a chemical method.

Covalent binding of photosensitive 1-(12-azido-[9,10(-3)H2]oleoyl)glycero-3-phosphocholine (lysolecithin) to human serum high density apolipoproteins.

Human serum high density apoproteins were complexed with increasing concentrations of 1-(12-azido-[9,10(-3)H2]oleoyl)glycero-3-phosphocholine up to the saturation concentration (72 mol lysolecithin per mol apo HDL). Ultraviolet irradiation generated the nitrene which led to crosslinking with the two main apolipoproteins AI and AII. Methods are described for the removal of excess, unbound lipid and the column chromatographic separation of the lipopolypeptides AI and AII.

Retroperitoneal approach for implantation of aorto-iliac and aorto-femoral vascular prosthesis.

During the period 1963--76, sixty-nine patients underwent aorto-iliac of aorto-femoral by-pass operation at the department. Fifty-two of the operations were performed through a retroperitoneal approach. The operative procedure is described. The approach is found gratifying and the statement of earlier authors concerning fewer cases of paralytic ileus and smoother convalescence is confirmed.

Occult traumatic dislocation of the patella.

In series of patients with clinically established traumatic patellar dislocation other authors have found surgical and radiological lesions pathognomonic for this condition. In a series of patients admitted with hemarthrosis, and with no knowledge of having had dislocation of their patella, we found the same lesions thus illustrating that a syndrome of occult traumatic patellar dislocation exists. The vertical location of the patients' patellae was found normal measured by the index of Insall and Salvati (9). The intercondylar groove measured by the method of Brattström (4) was found in almost all cases to be under the upper normal limit. Their knee caps were normal following the descriptions of Knutsson. None had valgus knees beyond normal upper limits and none had general laxity of joints, thus indicating that this syndrome may be found in normal knees.
