ECG gated thallium 201 myocardial images: value in detecting multivessel disease in patients on anti anginal therapy 1-3 months after myocardial infarction.

ECG gated 201Tl scintigraphy was compared to non gated images for detecting 2-3 vessel disease 1-3 months after a first uncomplicated myocardial infarction. In all, 111 patients on anti anginal treatment underwent coronary arteriography and stress thallium imaging; 39 showed single vessel disease (SVD), and 72 multivessel disease (MVD). Sensitivity of black and white analog images was 82% for SVD and 8% for MVD. Sensitivity of computerized colored static images was 87% for SVD and 33% for MVD. For gated images, sensitivity was 100% and 92% in patients with SVD and MVD respectively. Specificity for detecting MVD was 95% for black and white images, 77% for computerized colored static images, and 69% for ECG gated images. Thus, ECG gated exercise 201TL scintigraphy is useful for predicting the extent of coronary artery disease 1-3 months after myocardial infarction in patients on anti anginal therapy.

Non-invasive measurement of pulmonary arterial pressure: I. A haemodynamic modelling approach.

In order to monitor pulmonary arterial pressure (P) by any non-invasive imaging technique, a haemodynamic model of blood flow kinetics and wall mechanics has been developed. It is a one-dimensional model of pulsatile flow in an elastic pulmonary arterial trunk, assuming that blood is an incompressible fluid and viscous effects are negligible. The equations are P(t)-Pd = rho c2lnS(t)/Sd-1/2pw-2(t) Pd = (Sd/Ss)1/2Pp where, at any time of the ejection phase of systole, P(t), S(t) and w(t) are the pulmonary arterial pressure, cross-sectional area of the pulmonary artery and blood velocity averaged on the cross section S, respectively, PP is the pulse pressure, the difference between the peak systolic pressure and the diastolic pressure Pd; rho is blood density, c pulse wave velocity, and Ss and Sd are maximum (systolic) and minimum (diastolic) values of the cross-sectional area S. Using these equations, P(t) can be calculated if the three parameters, i.e. c, S(t) and w(t) are measured. So far, it has been impossible to measure the pulse wave velocity c non-invasively. We have investigated the calculation of c from S(t) and w(t) using the equation of continuity in the absence and presence of reflected pressure waves. The hypotheses of the haemodynamic model are discussed.

[Dose-response relation of intravenous enoximone in congestive cardiac insufficiency].

Enoximone (MDL 17043) is a new generation inotropic drug which acts by inhibiting phosphodiesterase and is endowed with both inotropic and vasodilator properties. The purpose of this study, which involved 23 patients aged from 18 to 75 years in NYHA class III or IV and with evidence of severe haemodynamic disturbances (cardiac index below 2.5 1/mn/m2, pulmonary wedge pressure above 15 mmHg), was to evaluate the acute haemodynamic responses to doses of enoximone that ranged from 0.25 to 2.50 mg/kg administered by bolus intravenous injection. Heart failure was either of ischaemic origin (6 cases) or idiopathic (10 cases) or due to various causes (7 cases). Group A patients (n = 11) received the drug in low doses (less than or equal to 1 mg/kg) as opposed to group B patients (n = 12) who were given high doses (greater than 1 mg/kg). Results were evaluated from the amplitude and duration of the haemodynamic response at maximum effect time (30 min). The following parameters were measured: cardiac index, pulmonary wedge pressure, systemic vascular resistance, mean arterial pressure and heart rate. Cardiac index and pulmonary wedge pressure were significantly improved in both groups (P less than 0.005): cardiac index +39 p. 100 in group A, +55 p. 100 in group B; pulmonary wedge pressure -36 p. 100 in group A, -48 p. 100 in group B; systemic vascular resistance -46 p. 100 in group B. Heart rate and arterial pressure were not significantly altered. The duration of response was 1 to 3 hours in group A patients and 4 to 8 hours in group B patients.(ABSTRACT TRUNCATED AT 250 WORDS)

Evaluation of 3 dimensional space and time filtering on ECG gated 201Tl myocardial images.

Stress 201Tl myocardial images were obtained using a standard ECG gated acquisition protocol for 10 min per view. These images were filtered using a three dimensional space and time filter. The effects of filtering were evaluated by sectorial analysis. Comparing raw and filtered dynamic images, quantification demonstrated that filtering did not produce artefacts. A conventional static image was also obtained as the sum of the series of unfiltered dynamic images. Comparison of raw static and filtered diastolic images indicated that the latter demonstrated myocardial abnormalities more clearly (P less than 0.01) in 33 patients. Improvement in the signal to noise ratio produced by filtering made cinematic display and visual analysis possible which was not feasible with raw gated images. Clinical evaluation is in progress.

[Primary pulmonary lymphoma].

The authors report 2 cases of primary pulmonary lymphoma. The first case was a 65 year old woman with a diagnosis of lymphocytic lymphoma confirmed by surgical biopsy, after being suspected on the basis of bronchoscopic biopsies and bronchiolo-alveolar aspiration. Twenty one months after the beginning of treatment with chemotherapy and six months after this treatment was completed, the patient was in complete remission. The second patient was a 52 year old man who underwent left inferior lobectomy for a lympho-plasmacytic lymphoma, which was only diagnosed definitively on the operative specimen. The authors discuss the clinical and radiological features and the histological and immunological criteria of primary pulmonary lymphomas (PPL). There are a number of related diseases with varying degrees of malignancy: interstitial lymphocytic pneumonitis (ILP), pseudo-lymphoma, which corresponds to a localised form of ILP, lymphomatoid granulomatosis (LYG) and pulmonary lymphoma, which usually has a lymphocytic or lympho-plasmocytic cell type.

Iodine 123-labeled metaiodobenzylguanidine imaging in heart disease.

Scintigraphic images of myocardial iodine 123-labeled metaiodobenzylguanidine (MIBG) reflect the relative distribution of adrenergic neurodensity and function in the myocardium. In patients with hypertrophic cardiomyopathy or after infarction, MIBG uptake in hypertrophied myocardium and the infarct-related myocardium was found to be decreased in comparison to blood flow distribution, delineated with thallium 201. Most intriguingly, semiquantitative measurements in patients with congestive heart failure demonstrated reduced myocardial MIBG uptake. This reduction correlated directly with indexes of left ventricular function. Decreases in neuronal density, dysfunction of adrenergic neurons, or chronically elevated circulating norepinephrine levels may account for this diminished myocardial uptake, which, as demonstrated in a pilot study of 90 patients with congestive heart failure, was found to be of predictive value for survival.

[Enoximone and the therapeutic strategy in patients awaiting emergency cardiac graft].

Enoximone is a positive inotropic agent belonging to the group of phosphodiesterase F-III inhibitors. The drug was tested in 34 patients uncontrolled by sympathomimetic drugs and referred to our department for urgent heart transplantation or circulatory assistance. After insertion of a Swan-Ganzgatheter and a radial artery catheter for haemodynamic monitoring, enoximone was administered as a 15-minute intravenous bolus injection of 1 to 2.5 mg/kf every 8 hours, in addition to sympathomimetic agents. Clinical and haemodynamic improvement was observed after thirty minutes in 30 patients. The cardiac index rose from 1.82 to 2.67 l/min/m2 and the pulmonary wedge pressure fell from 30.8 to 18.9 mmHg. Systemic arterial resistance decreased from 2170 to 1520 dyn. s. cm-5, and pulmonary resistance from 5.5 to 4.6 Wood units (p less than 0.01 for all values). Four patients had no haemodynamic improvement and were put on circulatory assistance, using a Jarvik 7 total artificial heart in 3 of them and heterotopic circulatory assistance in one. After clinical investigation for contra-indication to heart transplantation, and as their improved haemodynamic status permitted, 12 of the 30 patients were considered suitable (group B) for heart transplantation. Transplantation was performed within a week of admission in 11 patients without any need for mechanical assistance. One of the group B patients who required implantation of a Jarvik 7 artificial heart died after 12 hours of assistance. Eighteen patients were considered unsuitable for transplantation (group A) and treated medically.(ABSTRACT TRUNCATED AT 250 WORDS)
