The article reports on the case of a 10-week-old male infant with a perforated appendix localised in the hernial sac of a right-sided inguinal hernia. The simultaneous occurrence of an acute appendicitis in infancy with the atypical localisation in the hernial sac presenting as an incarcerated hernia is a rare combination.

The influence of selective calcitonin blood level measurements and the radical surgical approach in the treatment of C-cell carcinoma of the thyroid is demonstrated using the case of an 11-year-old girl.
Open lung biopsy by a limited thoracotomy has an important place in the diagnosis of localized or diffuse pulmonary disease. Its application provides adequate pulmonary or mediastinal tissue biopsy, and permits a reliable histologic evaluation. The postoperative risks are small if certain pre-requisites are observed. We recommend atraumatic removing of the tissue, excluding the middle lobe and lingula. To prevent complications careful suturing of the biopsy site with absorbable suture material seems essential.

A gastroenterostomy without entero-anastomosis in rats favours the development of adenomatous epithelial lesions at the gastroenteral borderline in dependence of exposition to MNNG. The extent of such changes in the mucosa could be modified by vagotomy, pyloroplasty, or the prevention of duodenogastric reflux (Roux-en-Y method), whereby vagotomy has an enhancing effect on proliferation. A comparison of the mucosal changes at the gastroenteral anastomosis indicates that a multitude of factors causes environmental changes at the gastroenteral borderline, stimulating epithelial proliferation to the point of cancer formation. Thus, it is not possible to accuse any single factor such as intestinal reflux, carcinogens or different surgical techniques as the sole culprit in carcinogenesis.

The diagnosis of early gastric cancer can be preoperatively suspected, but can be made only by careful histological study of the resection material. In suspicion of early gastric cancer by X-ray or endoscopy the operative procedure cannot be simplified systematically, but the rules of cancer surgery require full observance. The extent of stomach resection will depend on tumor localization, macroscopic tumor appearance and the knowledge of the histological tumor type. Multicentricity and the continuous tumor growth, especially in diffuse type gastric cancer, and possible lymph node metastases must be taken into consideration in the surgical management. Despite differences in survival rates between various histological types of early gastric cancer the prognosis of the patients is highly favourable, given to adequate therapy.
Sclerosing mesenteritis is a rare, non-systemic disease confined to the mesenteric adipose tissue and typified by a variety of inflammatory pseudo-tumours. Histological investigation uncovers a wide spectrum of pathological changes, with collections of foam cells, fibrosis and for the most part sparse mononuclear inflammatory infiltrates in the mesenteric fat tissue. In the course of several weeks to months the pseudotumours generally resolve without therapeutic measures. The disease entity is discussed on the basis of three clinical cases.
The duodenum or the stomach were involved in 20 (11%) of 177 patients with Crohn's disease. Four patients had no symptoms in the upper gastrointestinal tract, in ten patients pains and gastric disorders recurred. Six patients had severe symptoms caused by stenosis of the stomach or duodenum. The endoscopical and histological findings correlated to the severity of the symptoms. Early pathological changes were identified only by gastroscopy. We have little experience of the medical treatment of Crohn's disease of the duodenum or the stomach. A bypass procedure (gastrojejunostomy combined wih proximal-gastric vagotomy (PGV)) is indicated in all cases of severe gastric or duodenal stenosis.
The concentration of nitrite and N-nitroso compounds was examined in the fasting gastric juice of 44 patients operated on for ulcer disease an average of 2.5 years previously and in 26 age-matched patients with healthy stomachs (controls). The concentration of nitrite and N-nitroso compounds in the gastric juice of the vagotomised patients did not differ from that of the controls. On the other hand, significant increases in nitrite concentrations were found in the gastric juice of patients managed by Billroth I or II procedures. The fraction of N-nitroso compounds was significantly increased only in the Billroth II resected stomach. These findings are relevant to the known cancer risk inherent in the gastric stump.
The concentration of nitrite and N-nitroso compounds was examined in the fasting gastric juice of 44 patients with regard to the known cancer risk to the operated stomach. All patients had surgery for therapy of an ulcer disease. The operation had taken place an average of 2.5 years previously. Enlisted as a control group were 26 age-matched individuals with healthy stomachs. The concentration of nitrite and N-nitroso compounds in the gastric juice of the vagotomized patiens (PGV) did not differ from that of the controls. On the other hand, a significant increase of the nitrite concentration was found in the gastric juice of those patients resected according to the Billroth I or II procedures. The fraction of N-nitroso compounds, however, was significantly elevated in relation to the other gastric juice samples only in the Billroth II resected stomach. The importance of the increase of nitrite and N-nitroso compounds to the cancer risk inherent in the gastric stump is the subject of discussion.
