[Experiences with a subcutaneous, fully resorbable bridge in construction a double loop ileo- and colostomy].

Our experience with the subcutaneous absorbable bridge for constructing a temporary loop ileostomy and loop colostomy is described. The use of this subcutaneous absorbable bridge in 15 patients - 6 with loop ileostomy and 9 with loop colostomy - was almost without complications. The absorbable bridge is a progress for maturation of the stoma and for immediate postoperative as prospective fitting of a watertight appliance. The actual trend substituting the temporary loop colostomy by the loop ileostomy may be advanced by the unlimited use of the subcutaneous absorbable bridge for constructing a temporary loop ileostomy.

[Results of 10 years' experience with peranal Parks and Stuart extirpation of villous and tubulo-villous adenomas of the rectum].

From 1973 to 1983 68 patients with villous and tubulovillous adenomas were operated by the peranal technique of Parks and Stuart. Seven patients had a circumferential adenoma and in ten patients (14.7%) an invasive cancer was found, four patients with Dukes A and six patients with Dukes B. The lethality of the 68 patients was 1.5% and the morbidity 4.5%. By a follow-up between 5 and 15 years the recurrence rate was 20.9%. There was no recurrence of cancer, especially not in the two patients with primary excision of an invasive carcinoma.

Sliding flap advancement for the treatment of high level fistulae.

The technique of sliding flap advancement for the treatment of high anal fistulae is described. The technique is not suitable for cases with an acute abscess and is reserved for patients with a well-established chronic fistula track. Thirty patients with anal fistulae were treated by sliding flap advancement from 1980 to 1984. Twenty-nine patients had satisfactory results. With a follow-up ranging from 18 months to 4 years no recurrence of fistulae or abscess were observed. The advantages of the advancement flap technique over the staging division technique with application of a seton and over the rerouting technique are discussed.

[Problems of pelvic infection following rectopexy using synthetics and its treatment].

The complication of the infected Ivalon-sponge after rectopexy is described in five patients. In two patients the pelvic sepsis perforated spontaneously into the vagina and in another two patients the pelvic abscess perforated through the levator muscles into the ischio-rectal fossa formating a typical horse-shoe abscess in one case. The management of choice in cases of pelvic sepsis is the complete removal of the infected Ivalon-sponge. We personally prefer the laparotomy for the complete removal of the prosthesis and not the removal through the vagina or through the rectum.

[Current aspects of the surgical treatment of colonic ileus caused by stenosing colonic carcinoma].

Formerly it was considered dangerous to undertake resection in the presence of colonic obstruction. This idea has been abandoned. Immediate right hemicolectomy is the accepted treatment for obstructed lesions of the right colon. For obstruction of the left colon proximal colostomy or caecostomy are being challenged by immediate resection as a Hartmann operation with iliac colostomy or finished with a colo-colonic or colo-rectal anastomosis. A subtotal colectomy with ileo-sigmoid or ileo-rectal anastomosis is a further method with encouraging experiences. From 1979-1984 a series of 33 patients with obstruction from a carcinoma of the left colon underwent emergency abdominal colectomy with primary ileosigmoidostomy or ileorectostomy without diversion. The mortality and morbidity was favourable compared with those reported in series of similar cases treated by stages procedures or primary resection. Several advantages of this method are explained and further encouraging experiences from other centers are reported.

[Emergency colectomy using primary anastomosis in obstructive cancer of the left colon].

From 1979-1982 a small series of 12 patients with obstruction from a carcinoma of the left colon underwent emergency abdominal colectomy with primary ileosigmoidostomy or ileorectostomy without diversion. The mortality and morbidity was favourable compared with those reported in series of similar cases treated by staged procedures or primary resection. This study could not document the more advanced state of cancers with acute obstruction. Further follow-up of this series is needed. This procedure is conceptually attractive in cases with obstructing left side carcinomas.

[Morbidity and mortality following intraperitoneal closure of transverse loop colostomy (author's transl)].

The postoperative course of 104 patients, who underwent closure of a transverse loop colostomy at the Surgical Department of the Teaching Hospital Herford between 1974 and 1980 after distal resection and anastomosis of the large bowel for neoplastic or diverticular disease has been reviewed in detail. The mortality was 0.9% and the morbidity rate was 25%, including 25% wound infections and 4.8% fecal fistulas. The highest complication rate was noted, when colostomies were closed during the first 6 weeks. Wound infections and fecal fistulae did not occur more frequently than in patients with diverticulitis. The intraperitoneal procedure of transverse loop colostomy closure can be recommended as to be straightforward and safe.

[Local recurrence following anterior resection of rectal cancer. CEA-titer (author's transl)].

The records of 70 patients undergoing a curative anterior resection at the Surgical Department of the Academic Teaching Hospital in Herford for carcinoma of the rectum between January 1, 1974 and December 31, 1978 were reviewed. There were 6 cases (8,5%) developing suture line and local recurrence between 9 and 16.5 months after anterior resection. In this study a rising CEA-titer was the first evidence of recurrence in only 29% of our patients. Among 9 patients with confirmed local recurrence - 6 from our own and 3 from outside - only 2 (22%) showed an increase in CEA levels above normal. In the face of the poor 5-year survival with secondary abdomino-perineal resection following anterior resection one must ask if the significance of those operative procedures with their technical difficulty is justified. Our own experience of 9 patients with secondary abdominoperineal resection for local recurrence has changed our mind for a more palliative operative procedure in such cases.

[Problems of sigmoid diverticulitis in the female and its differential diagnostic delineation from the left-sided adnexal tumor].

In the last three years 42 patients with complicated diverticulitis were treated surgically. There were 18 women in all. Nine of these women were believed to have gynecologic disease because of the palbable pelvic tumor and were hospitalized at the Gynecologic Department. 2 patients underwent an exploratory operation by gynecologic surgeons based upon preoperative diagnosis of ovarian mass. The diagnosis at operation in all two cases was perforated sigmoid diverticulitis. Another three of the nine patients had initally emergency exploratory operations by gynecologic surgeons based upon diagnosis of pelvic mass. Also here the diagnosis at operations were perforated sigmoid diverticulitis and they underwent emergency primary resection of the perforated sigmoid by surgeons. In all the cases of complicated diverticulitis the surgical proceature was the primary resection; the anterior resection was combined with a temporare transverse colostomy.--Diverticulitis is an important differential diagnosis of a left pelvic tumor in women with or without clinical and laboratory indications of infections and history of diverticulitis.

[Value and reason for routine rectosigmoidoscopy (author's transl)].

Routine rectosigmoidoscopy was undertaken in 700 patients under surgical treatment for disease in other parts of the body. Pathological changes were found in 39 patients (5.6%): 24 with adenomatous polyps, 5 with adenopapillary polyps, 7 with infiltrating growing "carcinomatous" polyps without involvement of the muscularis mucosae, and three with carcinoma. The polyps were removed with an electric loop or haemoclip in 28, by transanal submucous excision in eight. One of the carcinoma cases was treated by resection, the other two by abdominoperineal rectal resection. The results indicate that routine rectosigmoidoscopy is of value.
