A randomized prospective study of the use-effectiveness of two methods of natural family planning.

The final results of a prospective comparative study of two methods of natural family planning indicate a significant difference in the 12 month net cumulative pregnancy rates between the ovulation and symptothermal methods. These differences are on the order of two to one in favor of the symptothermal method. Pearl pregnancy rates confirm similar differentials between the two methods. Dropout rates for both methods were high. Lack of interest or dissatisfaction with the method was the major reason for dropout training while pregnancy or desire for pregnancy were the major reasons for dropout during the formal phase of the study.

Allergy in laboratory animal workers.

179 people working with small laboratory animals were studied by questionnaire, lung-function tests, skin tests with common allergen extracts and extracts of the serum and urine of five animal species, and serology. 49 people had symptoms related to animal contact, most commonly rhinitis but also asthma and skin rashes. Skin reactivity to animal extracts was related to symptoms, particularly asthma, but many people with rhinitis only had negative skin tests. Positive skin tests without symptoms were rare. 3 people had additional late skin reactions. Atopic individuals were no more likely than non-atopics to have animal-related symptoms but were more likely to have asthma. Gel diffusion revealed no precipitating antibodies, and no specific IgG was detected with the ELISA technique. Lung-function measurements were normal. Sensitisation to laboratory animals may involve heavy antigen exposure, so that relatively unreactive individuals can be affected. Exclusion of atopic individuals from work with laboratory animals will not eliminate the problem.

Clinical, virologic, and serologic evidence of Epstein-Barr virus infection in association with childhood pneumonia.

To explore the association of Epstein-Barr virus infection with childhood pneumonia we studied two patients whose mononucleosis-like illnesses were accompanied by pneumonia; both had virologic and serologic evidence of current or recent EBV infection. We then analyzed the sera of 71 children (age range, 14 months to 9 years) with pulmonary infiltrates for the presence of four classes of antibody to EBV. Antibody responses consistent with current or recent EB virus infection were found in 15. Two children had IgM antibodies to the EBV viral antigen at titers greater than or equal to 1:160, indicating current infection, and all 15 patients had antibody to components of the early antigen complex, suggesting recent infection. A fourfold rise or drop in one or more EBV-specific antibody classes was noted in eight patients within 30 days following onset of clinical illness. Few patients had clinical features suggesting infectious mononucleosis. Eight of the 15 with serologic evidence of current or recent EBV infection also had clinical or serologic evidence of infection with another pathogen--bacterial, viral, or mycoplasmal. Thus, in childhood pneumonia, EBV may be a primary, co-primary, or secondary pathogen; it may be reactivated in the course of infection with another agent, or possibly, by suppressing immune function, it may precipitate infection with some other organism.

Pinworm granuloma of cervix uteri--incidental observation following IUD use and cone biopsy.

A pinworm granuloma of cervix uteri was found incidentally in a hysterectomy specimen following cone biopsy for severe epithelial atypia. This lesion was regarded as too recently formed to be related to prior IUD use. The granuloma was apparently asymptomatic, as were 24 of 34 such lesions cited as involving uterus, adnexa or pelvic peritoneum or some combination of these sites. The only previous instance to be found of apparently deep cervical invasion by pinworms was associated with grossly evident carcinoma; the intact cervix has not yet been reported as subject to pinworm invasion.

A randomized prospective study of the use-effectiveness of two methods of natural family planning: an interim report.

Data accumulated to date from the Los Angeles study indicate that the total termination rate for 12 months from the beginning of the training period and from formal entry into the study was high for the ovulation method (OM), and symptothermal method (STM). Voluntary withdrawal was the highest single reason for termination in both methods. The 12 month voluntary withdrawal rate, measured from formal entry into the study, was significantly higher for OM users. Pregnancy rates measured from both the beginning of training and formal entry into the study were significantly higher for OM users than for STM users. Complete analysis of the data collected during the study is currently in progress. It is anticipated that some of the causes for the differences in pregnancy rates and withdrawal rates between the two methods can be identified.

Psoriasis with oral manifestations.

A forty-seven year old woman had skin and oral lesions of three year's duration. The skin lesions were clinically and microscopically diagnosed as psoriasis. Oral examination revealed extensive involvement of the entire mucosa with white, elevated, circinate lesions. Biopsies of oral mucosa were interpreted as histologically similar to the characteristic dermal findings in psoriasis. Anesthetic mouth rinses afforded symptomatic relief; however, although there was definite improvement, the oral lesions have persisted.

Acute fluoride poisoning.

Fluoride poisoning is a potentially severe environmental hazard for children. A case of fluoride poisong is presented which was manifested by severe hypocalcemia, ventricular arrhythmias, and respiratory failure. Treatment of this poisoning, including peritoneal dialysis, is discussed. The kinetics of fluoride distribution as measured in this patient suggest a rapid binding of ingested fluoride to bone, followed by gradual release and excretion. Peritoneal dialysis resulted in no significant fluoride removal.

Anti-Dob implicated as the cause of a delayed hemolytic transfusion reaction.

A 27-year-old, gravida 3, para 2 woman experienced a delayed hemolytic transfusion reaction. She had anti-Dob in both serum and eluate 8 days after infusion of 6 units of Do(b+) red cells. No antibody had been detected prior to transfusion. By the 15th day after transfusion, there was no evidence of survival of red cells from any of the 6 units. Anti-C and anti-M were demonstrated later, but 29 months after transfusion, no atypical antibodies were detectable. The evidence suggests anti-Dob should be considered an antibody of potential clinical significance until contrary evidence becomes available.

The management of acutely bleeding varices by injection sclerotherapy.

Acutely bleeding varices present a therapeutic dilemma because of the limited efficacy and high complication rates of conventional treatments. Over a period of 26 months, 56 episodes of acutely bleeding varices were treated with injection sclerotherapy as the initial management. Immediate control of haemorrhage was achieved in 91% of patients and control of bleeding during the hospital admission in 84%. Balloon tamponade and surgery were reserved for those patients in whom bleeding was not controlled by sclerotherapy; in 66% of patients the bleeding was controlled by sclerotherapy alone. Of the 56 episodes, 75% survived their admission to hospital. Acute injection sclerotherapy is as least as effective as conventional therapies in the control of bleeding with a low incidence of complications. The technique can be used in all patients irrespective of the severity of their liver disease and, when bleeding is controlled, allows the physician the choice of all the available prophylactic therapies.

Evaluation of the BAX system for detection of Listeria monocytogenes in foods: collaborative study.

A multilaboratory study was conducted to compare the automated BAX system and the standard cultural methods for detection of Listeria monocytogenes in foods. Six food types (frankfurters, soft cheese, smoked salmon, raw, ground beef, fresh radishes, and frozen peas) were analyzed by each method. For each food type, 3 inoculation levels were tested: high (average of 2 CFU/g), low (average of 0.2 CFU/g) and uninoculated controls. A total of 25 laboratories representing government and industry participated. Of the 2335 samples analyzed, 1109 were positive by the BAX system and 1115 were positive by the standard method. A Chi square analysis of each of the 6 food types, at the 3 inoculation levels tested, was performed. For all foods, except radishes, the BAX system performed as well as or better than the standard reference methods based on the Chi square results.

Sensitivity and specificity of diastolic indexes for rejection surveillance: temporal correlation with endomyocardial biopsy.

Although acute diastolic dysfunction is an early sequela of the rejecting heart, reported sensitivities and specificities have varied widely when Doppler echocardiography is used for rejection surveillance. This study examines the temporal relationship between changes in Doppler echocardiographic indexes of diastolic function and sequential endomyocardial biopsies to identify possible factors accounting for false-positive and false-negative results. A total of 114 Doppler echocardiographic studies and biopsies were performed weekly in 39 patients aged 14 to 59 years during the initial 3 months after heart transplantation. All Doppler examinations were within 24 hours of biopsy and were analyzed in a blinded fashion. Onset of restrictive physiology, defined as a 15% decrease in either isovolumic relaxation time or pressure half-time, was determined by analysis of the Doppler mitral flow velocity curve.
