Clinical and CSF findings in multiple sclerosis patients with or without IgG oligoclonal bands at isoelectric focusing examination of CSF and serum proteins.

The IgG oligoclonal bands in CSF can be found in high percentage of MS patients but the existence of a limited number of cases with CSF normal IgG profile is known as well. In the present study 63 out of 70 patients with definite MS and 24 out of 35 with probable MS had oligoclonal bands in the CSF at the isoelectric focusing examination. The 18 patients with normal CSF IgG pattern did not show any statistically significant difference as concerns the age at onset and the duration of the disease, the functional disability, the course of the disease and the quantitative CSF parameters (IgG index, IgG synthesis and serum/CSF albumin quotient). The oligoclonal pattern does not seem of value to discriminate different groups of MS patients but it remains essentially of great diagnostic importance in this disease.

Isoprinosine in multiple sclerosis treatment: a preliminary study.

Isoprinosine, a drug enhancing the cellular immunity, was administered to 11 patients with multiple sclerosis (MS) for a period of 60 days. Kurtzke's Disability Status Scale was evaluated in all the patients before the starting of the therapy and after 60 days and after 14 months of follow-up. 11 MS patients were taken as a control group and were treated with prednisone for the same period of time. No significant difference in the clinical course of the disease between the two groups of patients after the therapeutical trial was observed. CSF examination showed IgG oligoclonal bands in all the cases; no modification of the CSF IgG profile was found after the therapy in those patients in whom the lumbar puncture was performed after the follow-up period.

Identification of the kappa and lambda light chains within the CSF immunoglobulin region in multiple sclerosis and subacute sclerosing panencephalitis by immunofixation after isoelectric focusing.

The distribution of the kappa and lambda light chains within the cerebrospinal fluid (CSF) IgG oligoclonal bands of 15 multiple sclerosis (MS) and 3 subacute sclerosing panencephalitis (SSPE) patients has been studied by isoelectric focusing (IF) followed by direct immunofixation. The oligoclonal bands demonstrated by IF were both homogeneous and heterogeneous with regard to the light chain type; in the MS patients there was a prevalence of heterogeneous bands and of homogeneous bands of lambda type, while in SSPE a slight prevalence of kappa homogeneous bands was found. The kappa homogeneous bands were preferentially disposed in a more cathodal position in respect to lambda IgG bands; in some cases bands probably constituted only of free light chains of lambda type were found. Finally, in some cases, the oligoclonal IgG pattern was more distinct after immunofixation than after basal IF.

[Comparative quantitative and qualitative analysis of intrathecal IgG synthesis during multiple sclerosis and other neurologic diseases].

Intrathecal IgG synthesis has been studied in neurological patients by both quantitative and qualitative methods. An elevated correlation was found between IgG/Albumin quotient, IgG-Index and IgG SYN: moreover the appearance of oligoclonal IgG bands in CSF seems clearly related to high IgG Index values, as observed by statistical analysis. Nevertheless oligoclonal pattern in CSF is always present in diseases associated with immunological abnormalities, while quantitative parameters may be found elevated also in the diseases without a clear immunological disturbance. The authors stress the great importance of CSF examination by Isoelectric Focusing in the evaluation of intrathecal IgG synthesis in multiple sclerosis.

DNA synthesis enzymes and proliferating cell nuclear antigen in normal and neoplastic nerve cells.

The activity of nuclear DNA polymerases alpha, beta and delta/epsilon, uracil-DNA glycosylase, thymidine kinase and the presence of Proliferating Cell Nuclear Antigen (PCNA) have been examined in developing rat glial cells, in rat and human glioma, in human neuroblastoma and in differentiated neuroblastoma cell lines in vitro. During glial development the activity of all enzymes tested, except DNA polymerase beta, markedly decreased, suggesting their coordinate regulation in respect to the proliferative state of the cells. Glioma and neuroblastoma cell lines restore the enzymatic activities that were no longer expressed in normal adult cells. Neuroblastoma cell lines induced to differentiate in vitro by retinoic acid showed a decline of the activities of DNA polymerase alpha, DNA polymerase delta/epsilon, uracil-DNA glycosylase and thymidine kinase similar to that observed during in vivo differentiation. We also demonstrate that PCNA is not detectable in glial and neuronal cells at all developmental stages, but can be found in tumor nerve cells. A possible use of enzymatic assays or anti-PCNA antibodies to detect brain tumors is discussed.

Microheterogeneity of CSF alpha 2-macroglobulin in multiple sclerosis and other neurological diseases. A study performed by direct immunofixation after isoelectric focusing.

Cerebrospinal Fluid (CSF) and serum alpha 2-Macroglobulin (alpha 2M) was studied in patients affected by Multiple Sclerosis (MS) and other neurological diseases (OND) using Isoelectric Focusing followed by direct immunofixation. In the serum a simple broad band has always been observed; inside it several more stained subfractions were sometimes detected. In 74.3% of MS patients and in 65.7% of OND patients the CSF alpha 2-M was divided in two major bands. One corresponded to that observed in serum and the other was more anodal. The significance of this finding is discussed.

The quantitative parameters of intrathecal IgG synthesis. A comparison.

Cerebrospinal fluid IgG/Albumin, IgG Index, Tourtellotte's formula and Schuller's formula have been calculated in 258 patients with definite Multiple Sclerosis and oligoclonal bands at isoelectrofocusing examination of cerebrospinal fluid. A step wise discriminant analysis was performed on single parameters and on a combination of IgG Index, Tourtellotte's and Schuller's formulas. The latter showed the highest percentage of right classification.

Intrathecal methylprednisolone acetate in multiple sclerosis treatment. Clinical evaluation.

Twenty-eight patients with clinically definite Multiple Sclerosis were treated with intrathecal injections of Methylprednisolone acetate, 40 mg at four days intervals. Kurtzke's Disability Status Scale was evaluated in all the patients together with sum of scores for the neurological functions and the subjective assessment, before and after the treatment. Ten Multiple Sclerosis patients, treated with prednisone (100 mg at alternate days) were taken as a control group. No real advantages were found with the intrathecal treatment as compared to the orally administered steroids.

[Relation between the clinical and cerebrospinal fluid parameters in multiple sclerosis, with special reference to subjects without oligoclonal IgG in the cerebrospinal fluid].

120 patients with clinically definite or probable Multiple Sclerosis (MS) were studied by Isoelectric focusing of cerebrospinal fluid and serum proteins. Of the 62 patients with definite MS, 56 (90%) had IgG oligoclonal bands only in CSF, while in the group with probable MS 44 patients over 58 (75,8%) showed the same finding. The group of 20 patients with normal IgG profile has been compared with a group of 22 patients selected by random out of the 100 with IgG oligoclonal bands. No statistically significant difference was discovered between these groups but a tendency to present an higher age at onset and a longer duration of the disease in the group with a normal IgG pattern than in the patients with oligoclonal bands was pointed out.
