Uric acid, uric acid dihydrate, and urates in urinary calculi, ancient and modern.
Uric acid, uric acid dihydrate, and ammnoniumn acid urate occur in bladder stones both ancient and modern. They are seldomn abuindant in stones fromn technically developed areas. Urates are usually confined to children's endemic bladder stones; uric acid dihydrate is rare, but uric acid used to be commzinon in bladder stones from elderly men.

Management of gonococcal ophthalmia neonatorum with single-dose kanamycin and ocular irrigation with saline.
Two hundred nineteen neonates with gonococcal ophthalmia neonatorum, including 40 infected with penicillinase-producing strains, were treated as outpatients with a single intramuscular injection of 100 mg of kanamycin and hourly ocular irrigation with saline. Neisseria gonorrhoeae was isolated from three (1.4%) of the 212 babies attending for follow-up, and post-gonococcal conjunctivitis developed in 22 (10.4%) of those who returned for follow-up.
Effective adjuvant chemotherapy for advanced nasopharyngeal carcinoma in children: a final update of a long-term prospective study in a single institution.
PURPOSE: The purpose of this study was to determine the efficacy and toxicity of a doxorubicin/cyclophosphamide-based chemotherapy and local radiation therapy in children with locally advanced or metastatic nasopharyngeal carcinoma (NPC). PATIENTS AND METHODS: Twelve patients aged 6 to 20 years old were treated with a chemotherapy regimen comprised of vincristine (1.5 mg/m2) and doxorubicin (45 mg/m2) on day 1 and cyclophosphamide (210 mg/m2) and 5-fluorouracil (240 mg/m2) on days 1 to 5. Chemotherapy was administered every 3 weeks for 1 to 2 years. Radiotherapy to the primary site (59 to 68 Gy) and to the neck (59 to 66 Gy) was given before or after 2 to 4 courses of chemotherapy. RESULTS: All patients achieved a complete response 4 to 16 months from the start of therapy (median 7 months). Nine patients have remained tumor free from 2 to 21 years (median 11 years) from diagnosis. One child was lost to follow-up and one died of tuberculosis; both were disease-free. One child developed a secondary osteosarcoma in the left mandible. Chemotherapy caused grade 4 neutropenia and thrombocytopenia in four patients. There were no therapy-related deaths and the most common late effect of therapy was neck fibrosis, which was observed in all patients. We conclude that the chemotherapy and radiotherapy regimen used in this study is highly effective for children and adolescents with locally advanced NPC and is associated with tolerable toxicity.
Radioimmunotherapy after chemotherapy compared to chemotherapy alone in the treatment of advanced ovarian cancer: a matched analysis.
Ovarian cancer has an overall five-year survival of around 30% in spite of complete remissions being obtained after optimal surgery and platinum-based chemotherapy. Previous studies have indicated a survival advantage for patients treated with radiolabelled monoclonal antibodies (radioimmunotherapy). We report here on the survival of patients who received single-dose intraperitoneal radioimmunotherapy after having achieved complete remission with standard management. Twenty-five patients with epithelial ovarian cancer, stages Ic-IV, received adjuvant intraperitoneal radioimmunotherapy following completion of conventional chemotherapy. On achieving complete remission they receive once 25 mg of HMFG1 labelled with 18 mCi/m2. Controls for cases were sought from the database of the North Thames ovary group (NTOG). Controls were selected on the basis of stage, histological grade and type, and age of patient at diagnosis. Kaplan-Meier survival plots were constructed for cases and controls and subjected to statistical analysis with the log-rank test. Additionally, using a database of 84 NTOG patients known to be disease-free at the end of chemotherapy, estimated survival curves were constructed using Cox's proportional hazards regression model. Close matches were found for 20 of the 25 patients. Median survival has not been reached at a median follow-up of 59 months for cases and 27 months for controls. Survival at five years is 80% for cases and 55% for controls (p=0.0035). The Cox model estimates long-term (10-year) survival of 70% for patients who received radioimmunotherapy, compared to 32% for those that did not (p=0.003). All patients developed serological evidence of human anti-mouse antibody (HAMA). This study shows a likely survival benefit for patients with ovarian cancer who receive intraperitoneal radioimmuno-therapy in the adjuvant setting.
The impact of different data feedback methods on provider response to the Cooperative Cardiovascular Project.
OBJECTIVE: To compare the effects of feedback methods on the type and frequency of responses received after dissemination of Cooperative Cardiovascular Project data. METHODS: Feedback visits were conducted throughout Iowa by means of two approaches; in-person (on-site) visits and telephone conference calls. The content of the presentations was the same for both approaches. This study examined the efficacy of the feedback methods used as determined by the response rates of facilities. A standardized questionnaire was requested from each facility to document the types of action taken by the facilities as a result of the presentations. The Cooperative Cardiovascular Project data were presented to 106 Iowa facilities. On-site presentations were held at 56 facilities; telephone conferences were held with 50 facilities. RESULTS: Facilities that were visited on-site responded with definite quality improvement plans more frequently than did facilities that participated in telephone conferences. Improvement plans were received from 36 of the 56 facilities (64%) visited on-site and from 20 of the 50 facilities (40%) participating in telephone conferences. CONCLUSIONS: On-site visits appeared to be more effective than telephone conferences in promoting quality improvement efforts through facility-specific feedback presentations.
