[Cataract surgery in 2000].

The authors evaluate a questionnaire survey concern to cataract surgery in the Czech Republic. They present the results of all 58 ophthalmological departments where cataract is operated. In the CR in 2000 a total of 49,804 eyes were operated. In 94.4% of operations the technique of phacoemulsification was used. Primary implantation of an intraocular lens was used in 99% of the operated patients. In more than 27% for correction of aphakia a soft intraocular lens was used. Almost 30% of the operated patients had one-day surgery. In coming years a greater need for these operations may be foreseen. To avoid longer waiting periods more financial means will be needed for new types of materials or a solution of the problem of financial participation of the patient in meeting the costs of surgery.

[Primary polypseudophakia in patients with cataracts and hypermetropia].

The authors present clinical results of 22 cataract surgeries of hypermetropic eyes in 18 patients (mean age 74 years, range 58-95 years) with primary piggyback implantation of two PMMA IOLs both in capsular bag. The surgeries have been performed in The Eye Department of The Municipal Hospital in Ostrava. Intraocular lens calculations were performed using the SRK II formula. The follow-up period was 5 to 36 month (mean 15 month). The mean stabilised postoperative refraction was -0.24 D (range -3.5 D to +3.12 D). The mean best corrected visual acuity, that reached 0.23, is limited especially by amblyopia and other eye diseases. We noticed four secondary cataracts in two patients. There was found no interlenticular opacification. Primary implantation more posterior chamber intraocular lenses is a good choice for correction hypermetropia during cataract surgery.

[Cataract surgery in the Czech Republic 1988-1997].

The author presents the results of a questionnaire survey where all ophthalmological departments in the Czech Republic which operate cataract were addressed every year with eight or fifteen questions resp. pertaining to the frequency of operations, surgical technique and implantation of intraocular lenses. The results covering 10 years were evaluated. The number of cataract operations increased during the follow-up period from 11,339 to 46,175 eyes. The number of implantations of intraocular lenses increased from 13.50 to 98.80%. The number of operations made by the phacoemulsification technique was in 1991 283 eyes (1.80%) and increased to 36,543 eyes (79.10%) in 1997. Of the contemporary 137 phacosurgeons 12 are so-called high volume surgeons, i.e. they operate more than 50 cataracts per month.

[Development of cataract surgery and implantation of artificial intraocular lenses in Czecho-Slovakia 1988-1992].

After a brief introduction on the history of implantations of synthetic intraocular lenses in Czechoslovakia the authors present an analysis of surgery of cataract and implantation of artificial intraocular lenses during the past five years in Czechoslovakia. The number of operations of cataract increases every year, a substantial increase was recorded in particular in 1992. During the other years there is also a change from intracapsular to extracapsular extraction of cataract and a slow onset of phacoemulsification. The number of departments where artificial lenses are implanted is increasing as well as the ratio of implantations in relation to the total of cataract surgery. In particular during the remaining three years a marked rise of implantation of posterior chamber lenses was recorded and a decline in implantations of iris-clip lenses. Almost 95% of implantations of posterior chamber lenses is considered satisfactory, while less than 60% implantations from the total number of cataract surgery is still insufficient.

[Personal experience with modern extracapsular cataract extraction].

In the group of 42 eyes with extracapsular cataract extraction the postoperative results were very favourable differing conditions. There did not occur any more serious postoperative complications. Authors verified that for a skillful ophthalmic surgeon this operation is easily performed, on the other hand many of them have to overcome some accustomed aversion and to use at present time the needed extracapsular extraction, in spite of the previous routinely used intracapsular technique, which was successfully performed for many years. Present trend of extracapsular cataract extractions is based on the aim to decrease the number of the postoperative complications and to make possible the implantation of the intraocular lenses into the posterior chamber of the eye.

[Fluorescein and its adverse effects].

The authors investigated physical and chemical properties of fluorescein, a substance widely used in ophthalmology. In view of the increased application of fluorescein angiography in the diabetological programme, the authors draw attention to possible undesirable effects of fluorescein and submit an account of complications recorded after application of this substance to 576 patients examined by this method at the Ophthalmological Clinic in Brno.
