Neutrophil activation and morbidity in young adults with cyanotic congenital heart disease.

Young adults with cyanotic congenital heart disease have a high incidence of respiratory and haemostatic problems. Activated neutrophils release vasoactive and chemotactic factors which result in endothelial injury, lung parenchymal damage and the activation of platelets and coagulation pathways. To investigate the contribution of neutrophil activation to morbidity in young adults with cyanotic congenital heart disease, plasma neutrophil elastase levels were measured in 25 cyanotic patients and the results compared to patients with acyanotic heart disease and normal controls. Neutrophil elastase levels were significantly elevated in the group with cyanotic congenital heart disease (P < 0.001). Platelet activation was significantly increased in the patients with cyanotic heart disease (P < 0.001). Platelet aggregation was impaired only in those with haematocrits greater than 0.50 (P < 0.02). Whole blood coagulation, as determined by thrombelastography, was within normal limits. The reason for neutrophil activation in patients with cyanotic congenital heart disease is unclear, but activated neutrophils may contribute to the respiratory and haemostatic problems common to these patients.

Hepatitis C in sickle cell anemia.

We evaluated the contribution of hepatitis C virus infection to liver disease in patients with sickle cell anemia. Antibody to hepatitis C virus (anti-HCV) by commercial enzyme immunoassay and a second confirmatory assay were assayed in 121 consecutive patients with sickle cell anemia. Anti-HCV was detected in 25 of 121 patients (20.7%). Of patients transfused > 10 units of blood products, 30.3% were anti-HCV seropositive, whereas 8.6% of those patients who transfused < 10 units were seropositive. In 11 of the 121 patients, serum alanine aminotransferase levels were repeatedly elevated. Nine of these 11 patients were anti-HCV seropositive, one was positive for hepatitis B surface antigen, and one was negative for all viral markers. In contrast, of 110 patients with normal serum alanine aminotransferase levels, only 14% were anti-HCV seropositive. In patients with sickle cell anemia, exposure to hepatitis C is common, related to the number of previous transfusions, and the most likely cause of persistently elevated aminotransferase levels.

Spinal cord stimulation for chronic intractable pain: nursing implications.

Spinal cord stimulation (SCS) became available in the 1970s as a treatment option for patients with constant or recurring pain. This invasive procedure is performed after all other efforts have failed to prove effective in controlling pain. Advanced technology enables the patient to perform adjustments to maximize comfort with the use of a magnet or portable programmer. Nursing care is guided by a specific plan of care for the SCS patient population. A patient population-specific standard may be useful.

A microsatellite within the bovine kappa-casein gene reveals a polymorphism correlating strongly with polymorphisms previously described at the protein as well as the DNA level.

The polymorphism of a (TA)n(CA)n repeat microsatellite present in the third intron of the bovine kappa-casein gene (CASK) has been investigated. The existence of six alleles differing only in the number of dinucleotide repeats has been established. A total of 330 animals belonging to nine different pure bred Bos taurus French breeds or to a cross-bred Bos taurus x Bos indicus population (Créole) were genotyped. The distribution of the microsatellite alleles was examined and clear breed differences were noted. Genotyping of animals by isoelectric focusing (IEF) or restriction fragment length polymorphism (RFLP) (TaqI) was performed, in order to examine the relationship of the microsatellite polymorphism to other previously described CASK polymorphisms, at the protein and DNA levels. Strong correlation was seen, indicating that evolution of the various polymorphisms was not independent, and nine CASK haplotypes were observed.

The Carmarthen audit project.

Experience within East Dyfed Mental Health Services of using externally designed audit tools has indicated a lack of ownership of the audit process by clinical staff. A project has been developed to initiate standard setting and measurement at clinical level, with an assumption that the development of an audit culture through an audit process is synonymous with ownership. An external appraisal system is being developed which can validate internal standard setting and measurement.

Effect of step length optimization on the aerobic demand of running.

To assess whether distance runners displaying uneconomical freely chosen step lengths (FCSL) could be trained to shift FCSL toward a more optimal setting, six males and three females who exhibited uneconomical FCSL [mean optimal step length (OSL) = -9.81% of leg length from FCSL; mean change in oxygen uptake (VO2) (FCSL - OSL) = 1.46 ml.kg-1.min-1] comprised an experimental group that completed 15 treadmill sessions (30 min/day, 5 days/wk, 3 wk) of OSL training at individually determined running velocities (2.87-3.74 m/s). Training sessions featured alternating 5-min periods of combined audio and visual feedback matching OSL and no feedback. A control group of three subjects with uneconomical FCSL (2 males, 1 female) performed 3 wk of treadmill running without feedback. The extent of step length optimization was evaluated by comparing pre- and posttraining differences between FCSL and OSL and between pre- and posttraining VO2. Compared with the control group, the experimental group demonstrated a significantly (P < or = 0.05) greater relative shift in FCSL toward OSL and a marked reduction in FCSL VO2. Taken together, these results suggest that short-term audiovisual feedback training can be effective in optimizing step length and producing a decrease in aerobic demand among distance runners exhibiting uneconomical FCSL.

Prolactin gene expression in ovine and caprine mammary gland.

The presence of prolactin (PRL) mRNA in the mammary gland of lactating goats and sheep was demonstrated by Northern analysis and RT-PCR. This provides evidence that the PRL gene is transcribed in this tissue. This ectopic expression is not restricted to the lactational period, as PRL transcripts were also found during the last third of pregnancy. Comparison of mammary and pituitary PRL mRNAs showed that they are similar in size but less abundant in mammary gland. In addition, an 847-bp cDNA fragment amplified from mammary retrotranscripts, containing the entire coding region and the major part of the 5' and 3' untranslated regions (UTRs), was found to be identical in sequence to its pituitary counterpart. Primer extension analysis, performed to obtain further information on the structure of the mammary PRL mRNA, has shown that the 5' UTR is 56 nucleotides (nt) long for both species. This is comparable with the size (53 nt) found using the caprine pituitary RNA as template. These results strongly suggest that the PRL gene is not transcribed from a different promoter in mammary gland, as has been demonstrated for placental and lymphocyte cells, but is more likely transcribed from the pituitary-specific promoter. Finally, the presence of PRL mRNA in polysomal fractions suggests that PRL is synthesized in mammary cells.

High-dose fractionated total-body irradiation, etoposide, and cyclophosphamide followed by autologous stem-cell support in patients with malignant lymphoma.

PURPOSE: To evaluate a high-dose treatment regimen of fractionated total-body irradiation (TBI), etoposide, and cyclophosphamide (Cy) followed by autologous stem-cell transplantation (ASCT) in patients with malignant lymphoma. PATIENTS AND METHODS: Fifty-three patients with non-Hodgkin's lymphoma (NHL; n = 43) or Hodgkin's disease (HD; n = 10) received 12.0 Gy of fractionated TBI, etoposide 60 mg/kg, and Cy 100 mg/kg followed by infusion of autologous hematopoietic stem cells. RESULTS: Thirty-one of 53 patients are alive a median of 643 (range, 177 to 1,144) days after transplant. The 2 year Kaplan-Meier (K-M) estimates of survival, event-free survival (EFS), and relapse for all 53 patients were 54%, 45%, and 43%, respectively. Sixteen of 24 patients with less advanced disease and 10 of 29 patients with more advanced disease survive free of disease for K-M estimates of EFS of 61% and 31%, respectively (P = .006). The K-M estimates of relapse were 34% for patients with less advanced disease and 53% (P = .05) for patients with more advanced disease. The K-M estimates of dying from causes other than relapse were 8% in patients with less versus 25% in patients with more advanced disease (P = .09). CONCLUSION: These data indicate that approximately 60% of patients transplanted early after failure of initial therapy for malignant lymphoma are projected to be disease-free more than 2 years after treatment with fractionated TBI, etoposide, and Cy and infusion of autologous hematopoietic stem cells. The transplant-related mortality rate is low and relapse is the main cause of treatment failure in patients with less advanced disease. For patients with more advanced disease, the K-M estimates of both transplant-related deaths (25%) and relapse (53%) remain major problems.

Macrophage recruitment during limb development and wound healing in the embryonic and foetal mouse.

Macrophages play a pivotal role in the adult inflammatory response to wounding. They are directly responsible for cellular debridement and, by providing a source of growth factors and cytokines, they recruit other inflammatory and fibroblastic cells and influence cell proliferation and tissue remodelling. In this paper we investigate the role of macrophages in clearing areas of programmed cell death in the developing embryo and also their role in embryonic and foetal wound healing. Immunocytochemistry using the monocyte/macrophage-specific monoclonal antibody, F4/80, reveals a close association between areas of programmed cell death in the remodelling interdigital regions of the mouse footplate and of F4/80-positive cells, suggesting that monocyte-derived macrophages, and not locally recruited fibroblastic cells, as previously believed, are responsible for phagocytosing and clearing areas of interdigital apoptosis. Our studies of wound healing reveal that macrophages are not recruited to, and therefore cannot be playing an active role in the healing of, excisional wounds made in the mouse embryo at any stage up until E14.5. Beyond this transition stage we see a significant recruitment of macrophages within 12 hours of wounding. We find that macrophages can be attracted to wounds in earlier embryos if the wound results in significant cell death such as after burning.

The influence of rural/urban residence on health in the oldest-old.

Differences between rural (n = 18) and urban (n = 66) centenarians are examined across the following variables: physical health, activities of daily living, mental health, and life satisfaction. Results demonstrate higher levels of morale in rural residents and higher levels of functional health as exhibited by urban elders. Qualitative data support trends of increased degrees of independence among the rural participants. The absence of robust differences in rural and urban centenarians is discussed in terms of a leveling-off effect.

[Drug treatment of schizophrenic psychoses in puerperium].

Three schizophrenic patients and one schizophreniform patient, all experiencing puerperal psychosis, required a drug-induced delactation (bromocriptine) simultaneously to neuroleptic treatment. Taking into account the pharmacodynamic effects, an exacerbation of symptoms following bromocriptine (a D-2 receptor agonist) and an impairment of delactation following neuroleptic treatment (bringing about the blockade of D-2 receptors) are to be expected. In three cases, we carried out a combined bromocriptine-haloperidol treatment and, in one case, a bromocriptine-clozapine treatment. The above mentioned complications were not observed in any of the cases. The problems which may result from using clozapine during the puerperal period are also discussed.

The gene encoding lactoperoxidase is expressed in epithelial cells of the goat lactating mammary gland.

The localization of lactoperoxidase (LP), which catalyzes in milk the peroxidation of endogenous thiocyanate in the presence of peroxide, was investigated in the goat lactating mammary gland, using immunofluorescence and hybridization techniques. In situ hybridization experiments have demonstrated the presence of LP mRNA within the cytoplasm of alveolar epithelial cells (acini) where LP has been detected by immunofluorescence labelling with anti-LP polyclonal antibodies. Taken together, these data provide the first direct evidence for expression of the LP gene within secretory cells of the lactating mammary gland.

The reliability of the Periotron 6000 in the presence of plaque.

The aim of this study was to evaluate if the presence of supragingival plaque can influence measurements of gingival crevicular fluid flow (CFF) taken with the Periotron 6000 unit. Sixty-seven experimental sites and 67 contralateral control sites were randomly chosen. Crevicular fluid was collected on a filter paper strip for 15 seconds and the CFF was measured using the Periotron 6000 unit: CFF1. The supragingival plaque was removed from the experimental site with a Gracey curette and the plaque index (PI) was estimated. Five minutes later, a second CFF measurement was taken: CFF2. On the control tooth, the plaque was not removed, and the PI was established after the second CFF measurement. There was no significant difference between the CFF1 and the CFF2 values for the control teeth. In the experimental teeth, a PI of 3.0 increased the CFF1 (pre-plaque removal) compared to the CFF2 (post-plaque removal) value by 20.6 Periotron units (PU) (p = 0.0004). A PI of 2.0 caused a 7.3 PU increase (p = 0.0370). There were no significant changes for a PI of 1.0 or 0. In this type of experimental protocol, the results also indicated that CFF differences were not affected by the gingival index (GI) for either the experimental or control groups. These results indicate that large amounts of supragingival plaque can significantly elevate CFF measurements, as determined by the Periotron 6000 unit.

Comparison of morbidity and mortality after marrow transplantation from HLA-genotypically identical siblings and HLA-phenotypically identical unrelated donors.

The nature and incidence of life-threatening or fatal toxicity after marrow transplantation were analyzed in 52 patients who received HLA-A, B, DR, Dw-phenotypically identical marrow transplants from unrelated volunteer donors (URD). An age and disease-matched cohort of 104 patients transplanted from HLA-genotypically identical siblings was used as a comparative group. The actuarial probability of grade 3 or 4 regimen-related toxicity was 31% after URD transplants and 21% after matched sibling transplants (log-rank p = 0.1041). The median duration of first hospitalization was 33 days for recipients of genotypically-identical marrow and 36 days for recipients of URD marrow (p = 0.0244). Sixty percent of genotypically identical marrow recipients and 51% of unrelated volunteer donor marrow recipients were discharged home from Seattle free of disease at a median of 99 days post-transplant (p = 0.5095). The percentage of patients requiring readmission to hospital and the actuarial probability of requiring either hemodialysis or mechanical ventilation were not statistically different between the two groups. We conclude that matched URD marrow transplantation is not associated with significantly greater regimen-related toxicity or mortality than transplantation from HLA-genotypical siblings.
