[Early measurement of portal pressure as a deciding factor in the treatment of ruptured oesophageal varices (author's transl)].

A retrospective study of 51 patients with ruptured oesophageal varices showed a clear-cut difference in mean portal pressure between those who underwent elective surgery (33.7 cm/H2O) and those who were operated upon while still bleeding after failure of medical treatment (41.3 cm/H2O). It would appear that medical treatment is likely to be unsuccessful in 75% of the cases when portal pressure exceeds 35 cm/H2O. These findings tend to indicate that a rapid therapeutic decision would improve the results of emergency treatment. Early measurement of portal pressure by jugular vein catheterization seems to be essential, but other diagnostic procedures, such as laboratory tests and liver biopsy, are also important. A prospective study of 11 recent cases where these procedures were carried out appears to confirm their value as deciding factors in the treatment of ruptured oesophageal varices.

[Post-operative cobalt irradiation and chemotherapy in cancer of the oesophagus (author's transl)].

Oesophago-gastrectomy through combined abdominal and right thoracic incision was performed on 271 patients with epidermoid carcinoma of the thoracic oesophagus. The overall post-operative mortality rate of 16.6% dropped to 4.8% during the last 3 years. The global survival rate at 5 years was 9.3% but could be as high as 28% in patients with good prospects of cure. At the moment, the resectability rate is 74% and the "curability index" approximately 27%. The authors emphasize the need for a post-operative medical treatment capable of improving upon the results obtained with surgery. After reviewing the benefits provided by various therapeutic measures, they have adopted a treatment combining cobalt irradiation and chemotherapy. The results over a period of 2 1/2 years and in a limited number of cases appear to be encouraging.

[Carcinoma of transverse colon (author's transl)].

A personal series of 83 cases of carcinoma of transverse colon has been analysed, as the recent works of international literature. The low percentage of success of these cancers is justified by the anatomical conditions and the pathological findings. Particularly, this prognosis is made worse by the high percentage of multivisceral cancers, hepatic metastases and emergency cases. The operative technics are very different. Beside the typical resections (right or left hemicolectomy), there remains one more important place for segmental colectomy. At least the subtotal colectomy gets some more indications, especially in the high risk patients. Only 28% of the resected cases shows a 5 years survival rate. This percentage is nearly the same as most of the recents statistics.

[Several cases of emergency repair of loss of skin substance in the child (author's transl)].

Traumatic loss of skin substance in the child is uncommon. There are three main causes: dog bites, scraping lesions and bullet wounds. In all cases, great attention is paid to initial and early treatment of the wounds (exploration, wound care) under general anaesthesia. Provided the operator is competent, dog bites may be completely repaired from the outset. Lesions due to scraping or bullet wounds should be treated more cautiously with emergency treatment not necessarily seeking to achieve closure at any price.

[Endocrine polyadenomatosis associated with prolactin pituitary adenoma and an intrathyroidal parathyroid adenoma].

A pituitary adenoma was removed transsphenoidally from a 20-yr-old woman with secondary amenorrhea, galactorrhea, and hyperprolactinemia. Light and electronic microscopy, immunocytology characterized a prolactin cell tumor. The patient also underwent three surgical explorations for hyperparathyroidism. Only after selective catheterization of thyroid veins with radioimmunoassay for parathormone, an intrathyroidal parathyroid adenoma was found. No other case of proven prolactin adenoma in Wermer's syndrome has been reported.

[Value of conservative methods in the surgical treatment of peptic stenosis of the oesophagus in adults (author's transl)].

Out of a series of 103 patients operated on for peptic stenosis of the oesphagus, we carried out 63 conservative operations, 29 by the thoracic route, 34 by the abdominal route. The operation consists of retrograde dilatation, by finger or by instrument, combined with an anti-reflux arrangement. Among the various technics of the latter, plicature of the fundus has proved the most effective. Gradually, the indications for the abdominal approach have become dominant, even if the cardia cannot be lowered. One may then use a sleeve on the gastric cone, which until now, in 23 cases has given excellent results. One may thus carry out by an extremely benign operation without any mortality, a surgical cure not only of supple stenoses, but also of certain tight fibrous stenoses, considered insuperable. Postoperative dilation was rarely necessary. The results of which some are 8 years old, seem stable in the vast majority of cases. The proportion of excellent results was 81 p. cent of the series. It was higher in the group of plicatures of the fundus where there were only two failures. No case of cancer was observed in the remaining oesphagus.
