Theoretical mechanisms of dietary calcium's antihypertensive action.

Theoretical mechanisms underlying dietary calcium's antihypertensive action are reviewed. Based upon known defects in the regulation of membrane Ca2+ transport and regulation of intracellular free Ca2+ concentration, we conclude that maneuvers that favorably modify calcium homeostasis such as dietary calcium supplementation or 1,25 (OH)2 vitamin D3 administration appear to favorably modify these defects. Consequent improvements in vascular smooth muscle function may mediate the reductions in blood pressure that follows chronic dietary calcium supplementation in experimental hypertension and in the clinical setting.

[Pre and postoperative chemotherapy of osteosarcoma with an adriamycin-cisplatin combination. Risks of a neoadjuvant chemotherapy which is not sufficiently effective].

The authors evaluated a new protocol of neoadjuvant chemotherapy for osteosarcoma, easier to manage and different from T10. The good results obtained with the postoperative ADR-CDDP association led us to undertake a pilot study between 1982 and 1984, using ADR-CDDP as preoperative chemotherapy. The records of sixteen patients were available for follow-up. The average age of the patients was 19.9 years. Patients received two or three preoperative courses, and a total of six identical courses. Tolerance was good. Pain usually disappeared but this was often misleading because associated with radiological and/or clinical tumor progression, low histological necrosis or poor outcome. The continuous disease-free survival actuarial rate was less than 57 and 40% at 18 months and two years respectively. The actuarial survival rate was 87% at one year and 65% at two years respectively. Disappointing results of this preoperative protocol, compared to results with the SO4 78 or T10 protocols for example, led to publish these data early in order to underline their potential dangers. As a result, we stopped our study. The charter of pilot studies justifies this publication. As well, these data point out the necessity of very close follow-up of neoadjuvant chemotherapy by sophisticated medical imaging. Neoadjuvant chemotherapy, if ineffective, must be stopped early, and should lead to surgery, followed by adequate postoperative chemotherapy.

Metastatic calcification in the thyroid gland demonstrated on bone scan in a patient with primary hyperparathyroidism.

Bone scanning agents are known to accumulate in extraskeletal sites. We report the case of a patient with primary hyperparathyroidism whose bone scan performed with [99mTc]HMDP revealed not only the classic pattern described in hyperparathyroidism, but also a striking visceral uptake in the lungs, heart, stomach, and thyroid gland. Metastatic calcification was found on histologic examination of the thyroid.

[Medical treatment of soft-tissue sarcomas in adults].

Soft tissue sarcomas are rare tumors. There is a lot of histologic varieties but the natural history and the prognosis are very similar. Surgery remains the first step of the treatment policy. Radiotherapy, given additionally, can prevent local relapse. Till now, chemotherapy was given to patientes with advanced diseases. Except doxorubicine and ifosfamide much drugs act poorly in the solft tissue sarcomas. CYVADIC is the best combination available. One randomized study has established the value of adjuvant chemotherapy in soft tissue sarcomas of the extremities. The real place of chemotherapy, probably will be in the next years, beside surgery and radiotherapy, in a multidisciplinary combined approach.

[Diabetes and pregnancy: our experience with the insulin pump].

The author describes a retrospective study on the use of the insulin pump or continuous sub-cutaneous insulin injection in 12 insulin-dependent diabetic women during the last six months of pregnancy. Use was found to be easy and the control of glycemia was significantly better (m = 5.81 mmol/l with continuous subcutaneous insulin injection as opposed to 7.66 before continuous sub-cutaneous insulin injection, p less than 0.01) than with the standard treatment of 3 discontinuous insulin injections. Although not significant, the number of episodes of hyperglycemia was found to be lower. However, no reduction in hypoglycemia was noted. Maternal complications were rare and the children did not present any serious abnormalities. The results encourage the use of the insulin pump with all insulin-dependent diabetics right from the onset of pregnancy.

[Disseminated lupus erythematosus and pregnancy. Apropos of a case].

The authors report a case of second pregnancy in a woman with dermatologically active disseminated lupus erythematosus which was well stabilised on steroid therapy. They stress the need for repeated assays of the complement C3 and C4 fractions and the antinuclear and anti-native DNA antibodies and careful monitoring of the renal function in order to be able to assess the activity of the disease and to evaluate the effectiveness of steroid treatment, which may even be modified on the basis of these parameters. Independent of the degree of visceral involvement, pregnancy in a woman with lupus is always a high risk pregnancy, for the mother, for the foetus in utero and for the neonate, which must be investigated for the presence of transmitted lupus, atrio-ventricular block or even familial lupus, which may not present until later.

[Cutaneous localization of Hodgkin's disease. Description of 4 cases and review of the literature].

Cutaneous involvement is uncommon in Hodgkin disease. Among 167 Hodgkin patients, we observed skin lesions in four. Clinical and histopathological features are discussed and compared with those of 40 other well-documented cases previously reported in the literature. The wide spectrum of clinical patterns and variability of outcome are underlined. Skin biopsy is mandatory and provides cardinal evidence for diagnosis; however, the Reed Sternberg cell is not absolutely specific and caution must be taken in affirming a disease which implies major therapy. In most observations, skin involvement obviously has no bearing on prognosis. We believe that the reality of specific Hodgkin skin lesions is subject to major reservations.

Weight gain and movement patterns of very low birthweight babies nursed on lambswool.

34 very low birthweight babies (mean 1143 g) in incubators were randomly assigned to be continuously nursed on lambswool (n = 17) or ordinary cotton sheets (n = 17). The weight gain for the periods when babies were well was significantly larger for the wool group, 22.7 g/day vs 18.6 g/day for cotton control (p less than 0.02). The overall weight gain (which included weight change during periods of illness) revealed a similar picture in favour of the wool group, 21.5 g/day vs 18.2 g/day (p less than 0.05). Movement patterns for the two groups showed no differences, but for all babies a strong correlation was noted between moving and lying suspine (p less than 0.001), having eyes open (p less than 0.001), a cooler incubator (p less than 0.01), and faster weight gain (p less than 0.01). Lambswool seems to have advantages over cotton sheets as a bedding material for very low birth weight babies.

Melanin-containing peripheral carcinoid of the lung.

A case report of a patient with peripheral bronchial carcinoid is presented. The tumor cells contained characteristic neurosecretory granules, filopodia and microvilli. Some cells contained both neurosecretory granules and the granular variant of melanosome. We suggest that the demonstration of melanosomes does not necessarily confirm the diagnosis of melanoma and that melanin be added to the long list of substances contained in carcinoid tumors.

[Intensifying screens: speed, definition, afterglow and abrasion resistance (author's transl)].

Main characteristics of radiological intensifying screens were measured. Speed and definition under conditions described before. The different screens-film systems were ranged after their speed factor, at 40, 70 and 120 kV with mention of definition of these systems when measured. Afterglow and abrasion resistance were measured under experimental well definite conditions. Results show that are extant rare earth screens allowing a decrease of X-ray exposure without too great afterglow or mechanical frailty.
