Platelet aggregation and haemostatic response in dogs naturally co-infected by Leishmania infantum and Ehrlichia canis.

Haemostatic alterations in dogs naturally infected by ehrlichiosis and/or leishmaniasis were studied. Platelet count, ADP and collagen-induced platelet aggregation, prothrombin time, activated partial thromboplastin time (APTT) and plasma fibrinogen concentration were measured. An evident reduction of platelet aggregation response was shown for Leishmania-Ehrlichia co-infected dogs where platelet aggregation was lower in comparison with control and leishmaniotic dogs (ADP and collagen, P < or = 0.01) and ehrlichiotic dogs (ADP 10 and 7.5 microm, P < or = 0.05). Moreover, a significant increase in APTT as well as a reduction of the albumin/globulin rate (A/G) for leishmaniotic and co-infected dogs versus control and ehrlichiotic dogs was detected. The hypothesis of a synergism between leishmaniosis and ehrlichiosis in altering platelet function by different pathways is discussed.

Oil spill in the Rio de la Plata estuary, Argentina: 1. Biogeochemical assessment of waters, sediments, soils and biota.

Aliphatic (ALI) and aromatic (ARO) hydrocarbon concentrations, composition and sources were evaluated in waters, sediments, soils and biota to assess the impact of approximately 1000 tons of oil spilled in Rio de la Plata coastal waters. Total ALI levels ranged from 0.4-262 microg/l in waters, 0.01-87 microg/g in sediments, 5-39 microg/g in bivalves, 12-323 microg/g in macrophytes to 948-5187 microg/g in soils. ARO varied from non-detected 10 microg/l, 0.01-1.3 mug/g, 1.0-16 microg/g, 0.5-6.9 microg/g to 22-67 microg/g, respectively. Offshore (1, 5, 15 km) waters and sediments were little affected and contained low background hydrocarbon levels reflecting an effective wind-driven transport of the slick to the coast. Six months after the spill, coastal waters, sediments, soils and biota still presented very high levels exceeding baseline concentrations by 1-3 orders of magnitude. UCM/resolved aliphatic ratio showed a clear trend of increasing decay: coastal waters (3.3) < macrophytes (6.7) < soils (9.4) < offshore sediments (13) < coastal sediments (17) < clams (52). All environmental compartments consistently indicated that the most impacted area was the central sector close to Magdalena city, specially low-energy stream embouchures and bays which acted as efficient oil traps. The evaluation of hydrocarbon composition by principal component analysis indicated the predominance of biogenic (algae, vascular plant cuticular waxes), background anthropic, pyrogenic and diagenetic hydrocarbons, offshore and in non-impacted coastal sites. In contrast, polluted stations presented petrogenic signatures characterized by the abundance of isoprenoids, low molecular weight n-alkanes and methylated aromatics in different stages of alteration. The petrogenic/biogenic ratio ( n-C23) and petrogenic/pyrogenic relationship (methylated/unsubstitued PAH) discriminated the samples according to the different degree of impact. The following paper present the results of the study of the progress of hydrocarbon disappearance in sediments and soils 13 and 42 months after the spill.

The role of beating heart coronary artery bypass grafting in the current era: a point of view.

Despite coronary artery revascularization on the beating heart was originally introduced in the 1960's, its further development was halted for several decades by advances in medical technology and by the development of the extracorporeal circulation. It is only in recent years that coronary artery bypass grafting on the beating heart has gained a secure role in the armamentarium of cardiac surgeons, mainly as a result of the popularization of new techniques of coronary exposure and stabilization. In later years, the expansion of interventional cardiology and the refinement of catheter-based techniques for treating coronary artery disease have corresponded to a substantial decline in the overall number of patients referred for surgery. Additionally, more and more patients have extensive disease and concomitant risk factors. These factors have made the future of coronary artery surgery uncertain. As these trends are likely to persist, and probably become more prominent in the future, it is essential for surgeons to embrace changes that may lead to improved surgical outcomes after coronary revascularization. In this article the authors review some of the aspects that have contributed to the popularization of off-pump coronary artery bypass and speculate on the importance of embracing innovative techniques in the field of coronary artery surgery.

Sludge bed development in denitrifying reactors using different inocula-performance and microbiological aspects.

Aerobic and methanogenic consortia were evaluated as inocula for laboratory scale denitrifying reactors, fed with a synthetic wastewater with acetate as the main electron donor. The denitrifying microflora of inocula and reactors was evaluated by specific denitrifying activity, enumeration and isolation of denitrifiers, which were screened by amplified ribosomal DNA restriction analysis. Reactor performance was monitored by COD and nitrate removal efficiencies and granule size. The aerobic sludge failed to form granules, probably due to the development of a filamentous, nitrate-reducing organism which was characterised by 16SrDNA sequencing as Bacillus cereus. The methanogenic sludge showed denitrifying activity and adapted very rapidly to denitrifying conditions in the two reactors seeded with granules of different sizes. Denitrifiers grew around the granules, increasing the specific denitrifying activity of the sludge over 10-fold. Exopolymer-forming organisms, belonging to the same species, were isolated from both reactors. Granule size increased during operation, but flotation of the aggregates, related to gas retention was observed.

Evaluation of buffalo colostrum quality by estimation of enzyme activity levels.

A study to evaluate the value and potential use of colostral enzymes as markers for the evaluation of buffalo colostrum quality was conducted. The enzymes gamma-glutamyltransferase (GGT), lactic dehydrogenase (LDH), and alkaline phosphatase (ALP) in buffalo's colostrum were measured spectrophotometrically, and their activities were correlated with the gamma-globulin content. Gamma-globulin concentration was determined following the electrophoretic separation of the colostral proteins and quantified with a densitometer. Colostrum was obtained from 15 dams, soon after calving. Means, standard deviations, correlation coefficients, and degree of significance were calculated using the general linear model procedure of the Statistical Analysis Systems program. The activity of GGT in the colostrum was the highest, followed by LDH and ALP. A significant correlation (r = 0.86; P < 0.001) was seen between GGT and gamma-globulin concentration in the colostrum, supporting the suggestion of using this enzyme as a marker for the evaluation of colostrum quality.

Prevention of early postmenopausal bone loss with cyclical etidronate.

Cyclical etidronate has been shown to be effective in the treatment of established postmenopausal osteoporosis but less is known about its effects on early menopausal bone loss. The aim of the study was to establish the effects of cyclic etidronate therapy on spinal and proximal femoral bone mineral loss in early postmenopausal women. One hundred and seven women who were within 6 months to 3 years of the menopause were recruited into a 2-year, randomised, placebo-controlled, double-blind trial. Spinal bone mineral density was within 2 SD of the age-matched mean reference value at baseline. Bone mineral density in the lumbar spine and proximal femur was assessed by dual energy X-ray absorptiometry at baseline and thereafter at 6 monthly intervals for 2 years. Urinary collagen cross-links (deoxypyridinoline and pyridinoline) were measured at the same time points. Seventy-seven women completed the study. At the end of the treatment period, the mean bone mineral density change from baseline in the treated group was +0.14% and -0.06% in the lumbar spine and femoral neck, respectively, compared to -1.49 and -2.22 in the control group. Overall, there was a significant difference between the two groups at both these sites (p=0.01 and 0.001, respectively). No significant differences between the groups were demonstrated at the greater trochanter or Ward's triangle. The conclusion was that cyclical etidronate therapy prevents bone loss in the spine and femoral neck in early postmenopausal women. It provides a safe and effective therapeutic option for the prevention of postmenopausal osteoporosis in women who are unwilling or unable to tolerate hormone replacement therapy.

The knee joint in rheumatoid arthritis.

Rheumatoid arthritis may present in the knee within a wide spectrum of manifestations. Disease progression, treatment protocols, surgical treatments and the possible complications of these treatments differ distinctly from those of osteoarthritis.

Modulation of anthracycline activity in canine mammary tumour cells in vitro by medroxyprogesterone acetate.

Failure of chemotherapy with anthracyclines as a result of drug resistance and toxicity is a major problem in the clinical management of neoplasia. The aim of the present study was to evaluate the activity of medroxyprogesterone acetate (MPA) as a chemosensitiser on anthracycline cytotoxicity. The study investigated whether such an effect could be related to an increase in lipid peroxidation, nitric oxide production, membrane fluidity and intracellular anthracycline concentration. The results showed that anthracyclines decreased nitric oxide production but increased membrane viscosity (polarisation constant) and lipid hydroperoxide formation in canine mammary tumour cells. Moreover, it was found that both drug-induced cytotoxicity and membrane viscosity increased in the presence of MPA. Conversely, lipid hydroperoxides decreased in MPA-supplemented cells. Medroxyprogesterone acetate did not show any effect on nitric oxide production. The two anthracyclines used (doxorubicin and idarubicin) showed differential intranuclear accumulation in canine mammary tumour cells, and MPA significantly modified intracellular concentration of anthracyclines.

Buffalo-milk enzyme levels, their sensitivity to heat inactivation, and their possible use as markers for pasteurization.

The activities and rates of inactivation of four enzymes in raw buffalo milk were measured in relation to the process of heating to determine the value of these enzymes as markers for the evaluation of milk pasteurization. The activities of the enzymes alkaline phosphatase (ALP), lactic dehydrogenase (LDH), gamma-glutamyltransferase (GGT), and aspartate aminotransferase (AST) were measured before and after heating at 50, 60, 70, and 80 degrees C for 1, 3, 5, 10, 20, and 30 min. The enzyme GGT showed the highest activity (712 +/- 601 IU/liter), followed by LDH (386 +/- 183 IU/liter), ALP (295 +/- 164 IU/liter), and AST (18 +/- 4 IU/liter). Heating the milk at 50 degrees C for 1 to 30 min resulted in no effect on the activity of any of the enzymes. At 60 degrees C, ALP showed the highest sensitivity to heat inactivation, whereas all other enzymes showed resistance. At 70 degrees C, ALP activity was abolished completely after 1 min, whereas GGT and LDH lost most activity after 10 min, and AST still maintained 50% activity even after 30 min. At 80 degrees C, the activities of LDH and GGT were lost, whereas AST still retained some of its activity. The results suggest that in addition to ALP, LDH and GGT, but not AST, are potential markers for heat denaturation in buffalo milk, with GGT having the advantage that its concentration is the highest.

Deep vein thrombosis prophylaxis: a comprehensive approach for total hip and total knee arthroplasty patient populations.

One of the most catastrophic complications after total joint arthroplasty is a fatal pulmonary embolism. Thromboembolic disease is particularly a problem in lower extremity joint arthroplasty secondary to the development of deep vein thrombosis (DVT) and proximal propagation of the thrombus. The environment created during total hip and knee arthroplasty fulfills the criteria for DVT formation: vessel wall damage, venous stasis, and a hypercoagulable state. Evidence that suggests the insult and primary event in thrombogenesis occurs during surgery. Until recently, however, the main thrust of DVT prophylaxis has concentrated on the immediate postoperative period. A more global approach to patient care during the 6-week period beginning with surgery may result in more effective DVT prophylaxis. Operative interventions that have proven to be effective include hypotensive epidural anesthesia and intravenous administration of heparin. Postoperative pharmaceutical interventions range from standard doses of aspirin or warfarin to recently studied dosing regimens of low-molecular-weight heparins, antiplatelet agents, and antithrombotic agents. Mechanical prophylaxis has also proved to be a valuable adjunct in DVT prophylaxis during these periods. It is hoped that a more comprehensive approach incorporating several of the aforementioned treatments into a strategy that encompasses the intraoperative and early and late postoperative periods will maximize the effectiveness of DVT prophylaxis.

Liquid chromatographic-mass spectrometric determination of phenolic compounds using a capillary-scale particle beam interface.

A capillary-scale particle beam interface was used to detect 18 phenolic compounds in red wine samples. This technique allows reproducible, library searchable electron ionization spectra at only 1 microliter/min mobile phase flow-rate for a sensitive detection of the analytes in complex matrices. The method makes use of a narrow bore, reversed-phase packed capillary column for sample separation. Detection limits were in the low picogram range for most compounds. Sensitivity and response linearity were evaluated for eight phenolic acids, which are often encountered in red wines. The phenolic compound composition was outlined in two red wines obtained using different aging processes.

Extracorporeal right to left atrial bypass to treat right ventricular failure.

Graft right ventricular failure after heart transplantation, secondary to preoperative functional pulmonary hypertension, was successfully managed in a 49-year-old patient using an extracorporeal right to left atrial bypass. We comment on the case and discuss the type of mechanical assistance used.

Ovarian function suppression with a GnRH analogue: D-ser(But[t])[6]-Arzgly[10]-LHRH (Goserelin) in hormone dependent canine mammary cancer.

Hormones and hormone level modifying substances have long been used to treat hormone-dependent tumours in humans. Recently, attempts have been made to use hormone manipulation regimens for the treatment of these tumours in veterinary medicine. The aim of the present study was to evaluate the activity of the luteinizing hormone-releasing hormone (LHRH)-agonist, D-ser(But[t])[6]-Azgly[10]-LHRH (Goserelin) in hormone-dependent mammary cancer in dogs. Eighteen female dogs with hormone-dependent mammary cancer (T2-T4, N0, M0 according to TNM clinical staging classification) were selected and allocated into two groups: nine dogs not treated with Goserelin (Group 1) referred to as control; and nine dogs treated with 60 microg/kg depot Goserelin every 21 days for 12 months (Group 2). Goserelin treatment decreased circulating levels of oestradiol and progesterone and reduced the size of mammary tumours; all the animals showed objective response (OR) to treatment after 3 months, and the relapse-free survival after 2 years was 88%. Haematology and blood chemistry parameters, measured every month from the beginning of treatment, as well as physical examination, showed that the drug was without toxic effects. This suggests that, at the dose administered, Goserelin blocks the hypothalamus-pituitary-ovary axis, and consequently can be useful to treat hormone-dependent mammary tumours in female dogs.

[Integration of computerized tomography imaging with single photon emission in a commercial system for developing radiotherapy fields: application to conformational irradiation for lung carcinoma].

PURPOSE: Single photon emission computed tomography (SPECT) of lung perfusion permits to map functioning lung parenchyma with higher sensitivity than CT. Delivering higher radiation doses is used to increase local control in lung carcinoma; this strategy is based on radiobiological and clinical studies. Lung parenchyma is a dose-limiting tissue in patients irradiated for lung cancer. Functional mapping based on SPECT and CT findings permits to design radiation beams such as to minimize irradiation of functioning lung. MATERIAL AND METHODS: CT and SPECT were used to examine a patient with non small cell lung carcinoma (stage IIIB, T4N0, left lung) candidate to conformal irradiation. Images were spatially correlated based on lung contours and using CT findings as reference. SPECT images were normalized to mean right lung value and expressed as perfusion (functional) contours. CT images and perfusion contours were transferred to the treatment planning system (Cadplan V 2.79, Varian-Dosetek Oy): in this way both functional (SPECT) and anatomical (CT) data were available for planning. A comparison was made between two irradiation techniques defined at TPS with (technique B) or without (technique A) SPECT contour information. The prescribed dose was 70.2 Gy. Rival plans were compared using dose volume histograms of target and risk organs. Both functional and anatomical regions were considered in the lung, together with single lung(s) and lung parenchyma. A second perfusion SPECT was obtained 5 months after irradiation and correlated with pretreatment CT images. RESULTS: SPECT lung scans showed marked heterogeneity in the left lung, which was found neither at CT nor at classic lung function tests. The lung volume with perfusion exceeding 80% of average corresponds to about 70% of the anatomical volume. Mean doses to anatomical and to functional lung parenchyma were 24 Gy and 19 Gy, respectively, with technique A and 23 Gy and 18 Gy, respectively, with technique B. Thirty-five percent and 20%, respectively of anatomical and functional lung parenchyma received > or = 25 Gy (V25) with technique B. The figure for functional lung parenchyma was reduced by 5% with technique B. Optimal design of irradiation field geometry decreased the area of functional parenchyma given high doses, which sparing was greater with smaller irradiation volumes. CONCLUSIONS: We have integrated the functional data provided by SPECT lung perfusion into a commercial irradiation planning system. Lung function mapping permits to design irradiation portals sparing larger areas of functional lung parenchyma.
