Smoking and menopause.

A retrospective study on 293 women who attended the Physiopathology of Human Reproduction menopausal out-patient clinic at Bari University Hospital throughout 1995, has been carried out to evaluate correlations between smoking and climacteric syndrome. From the data we have discovered an earlier onset of menopause in the smokers. Fewer smokers were married and they worked more, drank more, and were more educated. The incidence of climacteric syndrome did not differ between smokers and non-smokers.

[Delivery in nulliparous women aged < or = 18 years].

METHODS: A retrospective study of 733 nulliparous women who gave birth at the Department of Obstetrics and Gynecology at the University of Bari, in the period from 1-1-1994 to 31-12-1995, was performed to evaluate the outcome of pregnancy and neonatal outcome in adolescents < or = 18 years old. The sample was subdivided into two subgroups; the first of 114 girls < or = 18 years old; the latter of 619 women 22-34 years of age, and they were compared. RESULTS: We have found a more lengthy term of pregnancy (39.66 weeks vs 39.27 weeks p = 0.008) and a greater incidence of vacuum extraction (VE) in the group of < or = 18 years old (7.89% vs 3.23% p = 0.03).

Sexuality in the climacterium.

A retrospective study on 421 women who spontaneously attended the Menopausal Center of Physiopathology of Human Reproduction at Bari University has been carried out to evaluate the incidence of the decrease of libido in climacterium and factors that affect it. The results show a high incidence of decreased libido (46%) and a correlation with age, dyspareunia and housewife status.

Climacteric syndrome and body-weight.

A retrospective study on 181 women who attended the menopausal out-patient clinic of Physiopathology of Human Reproduction at Bari University Hospital throughout 1995, has been carried out to evaluate the relation between body-weight (valued as BMI) and the climacteric syndrome. From the data we have discovered that hot flushes and sweating are more common in women with greater body-weight. Also, women with BMI > 23.8 were found to have a lower level of education.

Ursodeoxycholic acid therapy in pregnant women with cholestasis.

OBJECTIVE: To test the efficacy and safety of ursodeoxycholic acid (UDCA) in the treatment of intrahepatic cholestasis of pregnancy (ICP) by a randomized, double-blind, placebo-controlled trial. METHOD: Eight patients with pruritus and abnormal liver function tests received 600 mg/day of UDCA in two doses for 20 days, compared with a control group of eight patients who received placebo for 20 days. RESULTS: No adverse reactions were detected in any patient. During UDCA therapy a statistically significant improvement in pruritus, serum bile salt levels, serum glutamic pyruvic transaminase, alkaline phosphatase, gamma-glutamyl transpeptidase and total and direct bilirubin was observed. Only two patients treated with UDCA underwent cesarean sections due to reasons unrelated to the therapeutic trial. All newborns had an Apgar score >7 and normal postnatal growth. CONCLUSION: Although the number of patients in this study was very small, we suggest that UDCA treatment has a role in improving clinical and biochemical results in ICP.

Primary carcinoma of the fallopian tube: report on two cases.

Two cases of carcinoma of the Fallopian tube were treated by total abdominal hysterectomy, bilateral salpingo-oophorectomy and omentectomy followed by chemotherapy consisting of several courses of a cisplatin, epirubicin and cyclophosphamide combination. One patient received three additional courses of combined cisplatin-taxol. At 60 months from onset of therapy one patient (Stage IIa-Grade 1 tumor) is alive and well, while the other (Stage III-Grade 3 tumor) died of disease 26 months from initial treatment. Second-look laparotomy proved useful in assessing response to chemotherapy. CA-125 blood levels helped monitoring tumor activity. Anamnestic data relevant to tuberculous salpingitis on one case, and bilateral salpingoplasty for tubal occlusion, in the other case, may stand for some remarks.

The diagnosis of benign uterine pathology using transvaginal endohysterosonography.

Twenty-nine women believed to be affected with benign uterine pathologies underwent transvaginal hysterosonography insertion of a physiological solution into the cervical cavity by means of a catheter positioned in the cervical canal. This new technique, due to the acoustic window created by the fluid, made it possible to find: one cervical polyp, six endometrial polyps, one endometrial synechia, five submucosa myomas and one uterine malformation, the presence of which, with traditional ultrasonography can only be suspected. The subsequent hysteroscopic check-up confirmed the high diagnostic reliability of hysterosonography which is also easy to carry out, safe and costs little.

Fetal malformations in an epileptic pregnant woman treated with carbamazepine.

We report a case of an epileptic pregnant woman at 20 weeks of gestation treated with carbamazepine, whose fetus was affected by major malformations probably due to the drug teratogenicity. After a therapeutic abortion minor malformations were also found. After seven months the patient was pregnant again, at 14 weeks of gestation she aborted spontaneously a fetus with no malformations. During this pregnancy the patient took a reduced dosage of carbamazepine.

[Pregnancy in women over 35 years of age. What risk?].

A retrospective study of women who are delivered at the Dept. of Obstetrics and Gynecology at the University of Bari, in the period 1991-1994, was performed to evaluate if over 20 week pregnancies in women aged 35 years and over were at higher risk for maternal and neonatal complications than pregnancy in women aged 20 to 29 years. The results of these 4 years show that pregnancy in women aged 35 and over is burdened with a greater incidence of maternal complications and cesarean sections, but age is not a risk for neonatal outcomes. Moreover, most working women were obliged to delay the first birth child, but work does not increase the number of complications.

[Incidence of climacteric syndrome].

A retrospective study on 300 women who attended the menopausal out-patient clinic of Physiopathology of Human Reproduction at Bari University Hospital from 1994 to 1995, was carried out to evaluate the incidence of climacteric syndrome in premenopause together with its correlations with the social and cultural environment. The results don't show differences between spontaneous and surgical menopause, and minor differences with premenopause. No relationship has been discovered between the onset of the menarche and the onset of spontaneous menopause. The hot flushes are more frequent in women with low level of learning.

Development of melanoma throughout two close pregnancies. Case report.

Primary melanoma is the most malignant and the least frequent of all cutaneous tumors. Its incidence in pregnancy varies from 0.1 to 2.8/1000. Up to today, impact of pregnancy on melanoma's course is still a much debated question in the literature. Some earlier reports suggested a particularly serious prognosis for melanoma associated with pregnancy, while more recent studies show that pregnancy may influence relapses without significantly altering survival. This paper reports the case of a woman affected by melanoma, whose clinical conditions became more and more serious during her second pregnancy and the following puerperal period. The progressive impairment of her clinical condition has suggested a correlation between the two close pregnancies and the unfavourable course of her disease.

Long-term follow-up of women with amenorrhea-galactorrhea treated with bromocriptine.

The present study is aimed at investigating whether long-term use of bromocriptine on patients affected by amenorrhea and galactorrhea may improve the clinical picture after discontinuation of treatment. For this reason 26 patients with amenorrhea and galactorrhea have been studied. Sixteen had high PRL values and 10 were normoprolactinemic. The normoprolactinemic patients underwent a TRH test. All the patients underwent computed skull axial tomography (CT scan) and were treated with bromocriptine, at a daily dosage variable from 2.5 to 10 mg for an average period of 26 months. After discontinuation of treatment, follow-up was carried-out for 20 months. Eighty-seven percent of the patients affected by amenorrhea, galactorrhea and hyperprolactinemia had regular menses, in 75 of the patients galactorrhea completely disappeared. Of the ten patients with normoprolactinemic amenorrhea and galactorrhea, only those who positively responded to the TRH test had regular menstruation and showed disappearance of galactorrhea. Upon discontinuation of treatment amenorrhea recurred in 68% of the cases whereas galactorrhea recurred in 80%. CT scans revealed disappearance of 3 microadenomas and reduction in size of the macroadenoma. Long-term use of bromocriptine represents the first choice treatment for the syndrome of galactorrhea-amenorrhea. In case of relapse, treatment must be continued for an undefined period of time.

Induction of multifollicular growth in patients non responding to clomiphene citrate and gonadotropins or gonadotropins only.

OBJECTIVE: We aimed at inducing multifollicular recruitment in patients with chronic anovulation-resistant to Clomiphene Citrate (CC) and/or exogenous gonadotropins. DESIGN: At the II Department of Obstetrics and Gynecology we treated our patients with exogenous gonadotropins and concomitant endogenous gonadotropin suppression obtained by the use of a GnRH analog (Buserelin). PATIENTS: We studied 14 patients with chronic anovulation due to either hypothalamic dysfunction or polycystic ovarian syndrome. MEASUREMENTS: We monitored the follicular growth ultrasonographically from the eighth day of the menstrual cycle, and assessed E2 and LH daily dosage from the tenth day. RESULTS: With this protocol we obtained 2 or 3 mature follicles, and reached an ovulatory rate of 80.7% and a pregnancy rate of 71.4%. Premature luteinization never occurred, and progesterone increase took place only after human chorionic gonadotropins administration.

Analgesic properties of electromagnetic field therapy in patients with chronic pelvic pain.

AIM: Demonstration of analgesic effects of electromagnetic field treatment in cases of chronic refractory pelvic pain. STUDY DESIGN: Prospective non-controlled trial, 64 women complaining about pelvic pain of at least 6 months duration, resistant to standard therapies, submitted to electromagnetic field applications on both iliac regions by Thelf Systems apparatus by two applications daily lasting 2 hours each for 20-40 days. Control visit after 3 months. RESULTS: Complete subsidence of pain in 39 cases (61%), in 15 patients (23%) relief during treatment, then mild endopelvic tension after a 3-month control; in 10 cases (16%) symptoms reduced only during application hours, unchanged at follow-up. Outcome of treatment appears to be independent of pre-existent psychosocial variables. CONCLUSION: Magnetic therapy shows a real analgesic effect on pelvic pain, and seems to contribute to resolution of complex interactions between somatic nociceptive stimuli and psychosocial implications affecting pain perception in these patients.

Neoplastic association of leiomyoma, bizarre leiomyoma and leiomyosarcoma uteri. Case report.

Leiomyoma uteri is the commonest mesenchymal neoplasia of this organ. Leiomyosarcoma uteri is the most frequent histologic variant of all sarcomatous forms. Among these there is bizarre leiomyoma, a borderline lesion with potential malignity, whose differential diagnosis is very difficult. This paper reports the case of a 44 year old woman who was simultaneously diagnosed with these anatomicoclinical lesions. The singularity of this case is represented by the coexistence in the same patient of three different lesions that, according to our data and those reported in the latest literature, do not seem to be correlated.

Ultrasound diagnosis of heterotopic pregnancy with viable fetuses.

Combined pregnancy is an under-estimated and increasing clinical condition. Its recognition is usually made post-operatively. Ultrasound findings are not specific. Diagnosis is possible only when the cardiac activity of the extrauterine and intrauterine fetus can be detected. In the literature, 18 pre-operative sonographic demonstrations of such cases are reported. A correct echographic assessment can show a definite diagnosis or indicate a diagnostic laparoscopy minimizing maternal morbidity and mortality and improving the prognosis of the intrauterine fetus. In this report, ultrasound diagnostic aspects are discussed.
