[Does Ming's classification of gastric carcinomas have epidemiologic or prognostic value?].

The population based registry of digestive tract tumours of the country of Cote-d'Or was used to assess the epidemiological and prognostic value of Ming classification. The 211 gastric carcinomas operated or autopsied between 1976 and 1981 in this population were reviewed on a pathological basis. The expansive type (46.0%) was nearly as frequent as the infiltrative type (47.9%). In 6.2% of the cases it was impossible to classify carcinomas in one of these two types. The male/female ratio was higher in the expansive type (3.0) than in the infiltrative type (2.2) and the mean age of patients at the time of diagnosis was higher in the expansive type (69.5 years) than in the infiltrate type (66.4 years p less than 0,05). Ming classification was difficult to use with superficial carcinomas: 9 out of 12 could not be classified. Among invasive cancers only 1.5% were not classified in one of the two types. Polypoid and fungating carcinomas were more often of the expansive type (70.1%). On the contrary infiltrative carcinomas (ulcerated or not) corresponded to the infiltrative type of Ming in 65.6%. It was not possible to extrapolate the WHO classification to the Ming classification. Infiltrative carcinomas were more often associated with hyperplastic stroma (38.1%) than did expansive carcinomas (5.2%) the proportion of carcinomas limited to the digestive wall was higher in the expansive type (21.7%) than in the infiltrative type (5.0%). The survival rate was better in the expansive type (41.4%) than in the infiltrative type (30.1%) but the difference was not significant. The prognosis depended partly on the time of diagnosis. The results suggest that Ming classification has an epidemiological interest. It allows us to distinguish two types of carcinomas that appear quite different on basis of their growth and penetration capacity. However its prognostic interest appears limited.

[Application of derivative spectrophotometry in biochemistry].

Derivative spectrophotometry is used to increase the specificity of spectroscopic measurements. The main advantages of this method are a better resolution of overlapping bands and a reduction in the effects of turbidity and coloured background. Its biochemical applications of this method are reviewed.

[Morphological study of the muscle of Treitz (m. suspensorius duodeni)].

The differences of opinion concerning the morphology of the human suspensory muscle of the duodenum conducted the authors to perform a study upon 31 cadavers including 21 adults and 10 foetuses. In most cases they describe a single muscle arising from the duodeno-jejunal flexure and reaching the right side of the diaphragm oesophageal opening. Microscopic examination confirms that it consists of unstriped muscle fibers except cranially near from his diaphragmatic insertion where striped muscle fibers can be observed.

[Lipoprotein lipase activity in human milk; inhibition in vitro of the glucuro-conjugation of bilirubin (author's transl)].

The lipolytic activities of esterase, lipase and lipoprotein lipase were measured in 17 samples of human milk. Lipase and esterase activities were the same in all samples. However, lipoprotein lipase activity was increased only in samples provided from mothers whose infants had prolonged neonatal jaundice; after storage these samples inhibited the glucuro-conjugation of bilirubin in vitro and their concentration in non-esterified fatty acids was high. The mechanism of action of this enzyme is discussed in relation to the physico-chemical state of fat globules.

[Automated determination of serum BSP. Improvement of dialysis by sodium salicylate].

Because of the great difficulty of BSP dialysis in automatized determinations in serum the authors investigated the effect of sodium salicylate. They proved its good performance with aqueous and proteic solutions of BSP. About 70 mul of patient sera is used to perform the analysis with Technicon Autoanalyser. Comparison of data from about 220 serum evaluated by their proposed method and manual technique are studied.

Determination of hydroxocobalamin and cyanocobalamin by derivative spectrophotometry in cyanide poisoning.

Hydroxocobalamin (OHCo) and cyanocobalamin (CNCo) are determined directly in biological media, without extraction, by using first derivative spectrophotometry. We diluted 200 mL of plasma, urine, or standards with 1.8 mL of pH 6 buffer (boric acid, potassium dihydrogen orthophosphate, and potassium hydroxide). The first derivative spectra of the dilutions were plotted between 320 and 400 nm. At the exact zero-crossing point for hydroxocobalamin, the derivative values of cyanocobalamin concentration were determined. The same procedure was followed for hydroxocobalamin at the zero-crossing point for cyanocobalamin. The derivative values of the concentration curves are linear in the range 5-100 microM. The minimum detection limit is approximately 5 microM for hydroxocobalamin of cyanocobalamin on the determination of hydroxocobalamin or vice versa, although the spectra strongly overlap. The method is fast and simple to use, thus making it easy to assess the in vivo transformation of hydroxocobalamin into cyanocobalamin after the administration of high doses of hydrocobalamin in cyanide poisoning.
